Exploring the major syntactic phenomena of German, this book provides a state-of-the-art account of German syntax, as well as an outline of the key aspects of Chomsky's Minimalist Program. It is one of the first comprehensive studies of the entire syntactic component of a natural language within the Minimalist Program, covering core issues including clause structure, binding, case, agreement, control, and movement. It introduces a phase-based theory of syntax that establishes Remove, an operation that removes syntactic structure, as a mirror image of Merge, which builds syntactic structure. Calum Carmichael presents a new perspective on how parables unique to Luke's Gospel were composed. These parables took up moral issues that arose out of conflicts among figures such as Jacob and Esau, Joseph and his brothers, Judah and Tamar as portrayed in Genesis narratives. Providing literary and linguistic analyses, Carmichael demonstrates how Luke, like many of his contemporaries, absorbed the narrative legacy of the Hebrew Bible and used it to express ideas about Jesus. The Joseph story was of particular interest to Luke because Joseph's role during the Egyptian famine resulted in the rescue of his family, thereby giving the Israelite nation a future. This book examines the ways in which contemporary Arab authors communicate with two major sources of inspiration: the first, is the rich Arabic literary heritage whether it has been embodied in texts or concrete experiences, real or imaginary. The subject of elliptic curves is one of the jewels of nineteenth-century mathematics, originated by Abel, Gauss, Jacobi, and Legendre. Solid Mechanics, as compared to Mechanics of Materials or Strength of Materials, is generally considered to be a higher-level course. It is usually taught in higher semesters to senior undergraduate students. However, there is no suitable textbook on this subject. The book is primarily aimed at this group of students and the text is an attempt to bridge the gap between complex formulations in the theory of elasticity and elementary strength of materials in a simplified manner. This Element explores the life, teaching, and legacy of philosopher and spiritual teacher Jiddu Krishnamurti. From an obscure childhood in south India, he was 'discovered' at age fourteen by the Theosophical Society as the vehicle for the prophesied World Teacher of this cosmic age. At age 34, he disaffiliated from the Society, became an independent teacher, and, for sixty years, traveled widely and addressed thousands of audiences on the need to develop awareness and attention for transformation of consciousness. His teaching defines the human condition as perilous, dominated almost completely by cultural and personal conditioning, fear, and negative emotions. Freedom from these perils, his teaching states, occurs through rigorous self-observation and inquiry in the search for truth. While extremely popular, Krishnamurti rejected the mantle of authority invariably attributed to spiritual masters and teachers. He created schools in his name to implement his pedagogy of non-authoritarianism and freedom from conditioning.
The search for a new scientific theory is typically prompted by an encounter with something in the world that cannot be explained by current theories. This is not the case for the search for a theory of quantum gravity, which has been primarily motivated by theoretical and philosophical concerns. This Element introduces some of the motivations for seeking a theory of quantum gravity, with the aim of instigating a more critical perspective on how they are used in defining and constraining the theory sought. These motivations include unification, incompatibilities between general relativity and quantum field theory, consistency, singularity resolution, and results from black hole thermodynamics.
Metrical systems differ in patterns of stress assignment, the domains over which those patterns are built, and acoustic manifestations of stress. It has been widely debated in the phonological/phonetic literature how stress should be represented, what mechanisms govern its assignment, and whether the phonetic underpinnings of primary/secondary stress exist independently of other prominence effects (e.g. boundary strengthening, pitch accents). This Element addresses these fundamental issues on the basis of an in-depth study of a hybrid (lexical-grammatical) metrical system of Ukrainian. It synthesizes previous results with new findings, focusing on the phonetic as well as formal description of the Ukrainian system. The lexical-grammatical stress interactions in Ukrainian pose a challenge for current metrical theories, shed light on the relation between the lexical and grammatical stress domains, and the relationship between categorical and gradient aspects of the metrical system. While there has been a great deal (comparatively) written about Rus as part of medieval Europe in the last twenty years, the popular perception of it remains a part of the so-called 'Byzantine Commonwealth.' This traditional framing discounts Rusian ties to the west and exaggerates those to 'Byzantium,' elevating the rhetoric used by Russian nationalists to separate Russia from Europe. This Element provides an accurate historical account of medieval Rus that corrects the modern misuse of medieval history: a resource for academics interested in the results of current research on the place of the Kingdom of Rus in the medieval world. It brings together and synthesizes existing scholarship on Rus to present a complete picture of the kingdom of Rus, and its orientation within the wider medieval world, with a particular focus on the eleventh and twelfth centuries.
This study explored bereaved relatives’ experiences of end-of-life care (EoL care) in the last 3 days in an acute private hospital in Australia.
Methods
An interpretative qualitative study was conducted. Semi-structured interviews with 8 bereaved relatives whose family member had died at an acute private hospital shared their experiences of the EoL care during the last 3 days of life. The transcribed interviews were analyzed using inductive thematic analysis.
Results
Bereaved family members had mixed experiences, and their primary concerns related to the need for improvements in support for the family; communication; and clinicians partnering with families. The need for family support encompassed care for the person dying and the bereaved relatives, before and during the last days of life, and after death. Bereaved relatives perceived that hospital based EoL care could be positive when the care was collaborative with health professionals, patients, and relatives and there was effective communication.
Significance of results
A patient- and family-centered approach to EoL care should be provided in hospitals, and it requires understanding of the needs of both patients and family members, including informational requirements, communication approaches, and care delivery. Health-care organizations have a responsibility to care for families and this must be considered as part of organizational readiness and ongoing assessment to determine if the standards for EoL care are met. The findings serve as a guide for evidence-informed practice and may contribute to the development of resources and guidelines for delivery of quality EoL care.
Childhood morbidity is a precursor and contributor to under-five child mortality. Community-based primary healthcare programs are culturally responsive and low-cost strategies for delivering maternal and child health services in rural communities.
Aim:
To evaluate the equity effect of the Ghana Essential Health Intervention Program (GEHIP) – a five-year community-based primary healthcare program – on childhood morbidity.
Methods:
GEHIP was implemented in the Upper East region of Northern Ghana. Household baseline and end line surveys conducted in 2010/2011 and 2014/2015, respectively, from both intervention and comparison districts were used to assess three childhood morbidity conditions: maternal recall of neonatal illness, the incidence of diarrhoea, and fever. Difference-in-differences analysis, mean comparison test, and multivariate logistic regressions are used to assess the effect of GEHIP exposure on these three childhood morbidity conditions.
Results:
Baseline sample data of 2,911 women and end line sample of 2,829 women were included in this analysis. There was generally more reduction in all three childhood morbidity conditions in intervention communities relative to comparison communities. Diarrhoea and fever had a statistically significant treatment effect (AOR = 0.95, p-value<0.01 and AOR = 0.94, p-value<0.001). Results of equity analysis indicate significant mean reductions for both the poor and non-poor for neonatal illness and diarrhea, while only the intervention group had a significant reduction for both poor and non-poor for fever. Regression analysis shows no significant equity/inequity effects of GEHIP on the incidence of diarrhoea and fever. Neonatal illness, however, shows significant effects of wealth within the intervention group.
Conclusion:
This study shows that GEHIP contributed significantly to childhood morbidity reduction. This implies that community-based strategies have the potential to improve child health and contribute to the attainment of the United Nations sustainable development goal related to child health. Specific targeted measures are recommended to ensure both the poor and relatively better-off benefit from interventions.
This article explores the socio-political landscape of Donbas through a lens of post-colonial studies, revealing the Russian colonial past and neo-colonial ambition. By uncovering the interplay of cultural, political, and economic challenges the author identifies the key elements of the region’s identity and draws on historical analysis and personal reflections on the Russo-Ukrainian war. The article explores how Russia managed to dominate the discourse in Donbas, as well as the reasons why a significant part of the Donbas people accepted Russian dominance over the region and the creation of self-proclaimed states without great resistance. The study underscores the necessity to work on the decolonization of Donbas’ identity as the pivotal point for fostering reconciliation processes in the long-term occupied territories of Ukraine.
The need for more local technical capacity in Health Technology Assessment (HTA) is a leading challenge to its use in low- and middle-income countries. Zambia has been considering using HTA to support its universal health coverage initiative, which includes health benefits package design and implementation. This study assesses the local HTA capacity for the steering committee tasked with supporting the design and implementation of the national health benefits package in Zambia.
Methods
The study applied a cross-sectional web-based survey design and the consensus-based Checklist for Reporting of Survey Studies. Data were collected from the steering committee of the benefits package working group, tasked with leading the design process of the health benefits package using the Instrument for the Assessment of Skills to Conduct a Health Technology Assessment tool.
Results
The majority of respondents had not served on a selection and reimbursement committee. Clinical effectiveness skills in structuring a search strategy, handling missing data, conducting qualitative evidence synthesis, and grading the certainty of evidence were low. Skills for leadership, networking, conflict management, and project coordination, public and patient involvement were mid-level to low. Most of the respondents were aware of ethical issues with health technologies. Health economics skills in economic evaluations and decision analytic modeling, equity and health system efficiency measurement, budget impact analysis, and quality of life were identified for capacity strengthening.
Conclusion
Available technical capacities to revise and implement the national benefits package were lower in health economics, synthesis for clinical effectiveness evidence, ethics, patient and public involvement, and soft skills, in that order.
The maternal restraint stress animal model is based on a long-term stress paradigm administered to pregnant maternal animals, and these offspring have been shown to exhibit a variety of biochemical defects including obesity. This study aimed to investigate whether maternal restraint stress affects obesity-associated changes in offspring intestinal microbiota and the adipogenic differentiation of mesenchymal stem cells (MSCs).
Pregnant mice were subjected to restraint stress three times daily from gestational Day12 to delivery. Changes in the composition of the intestinal microbiota of mothers (during pregnancy and lactation) and their lactating offspring exposed to maternal restraint stress were analyzed using next-generation sequencing. Maternal stress altered the maternal microbiota, with reduced Bacteroidetes and increased Firmicutes. While similar trends were observed in offspring, these changes were not statistically significant. However, maternal stress notably reduced microbial diversity in the offspring’s intestinal microbiota. Bone marrow-derived MSCs from offspring at weaning were analyzed for adipogenic transcription factors and hormone receptor expression using quantitative PCR. Maternal stress enhanced the adipogenic phenotype of offspring MSCs, as evidenced by increased expression of adipogenic markers (PPARγ, leptin receptor) and a reduced osteogenic phenotype. In vitro induction further confirmed the higher adipocyte differentiation potential in stressed offspring MSCs compared to controls.
Our results revealed that maternal restraint stress altered the maternal intestinal microbiota, leading to reduced microbial diversity in offspring, predisposing their MSCs toward an adipocyte phenotype. These finding suggest that modulating the intestinal microbiota of stressed pregnant women may improve the susceptibility to obesity in their children.
When anomalous left coronary artery from the pulmonary artery is diagnosed in infancy, it is associated with high mortality, but collaterals permit rare adult survival despite coronary steal. We report normalisation of perfusion abnormality after transcatheter proximal left coronary occlusion in a symptomatic adult with inducible ischaemia.
Methods:
A forty-eight-year-old female presented with exertional chest pain; EKG showed sinus rhythm with left axis deviation with intraventricular conduction defect. Echocardiogram diagnosed an anomalous left coronary artery arising from the pulmonary artery with normal ventricular function which was further confirmed by CT. The case was discussed with the surgical team, and it was decided to surgically ligate. The patient did not give consent for the surgical procedure; the patient was taken up for the procedure.
Results:
Device was released safely after confirming the position on echocardiogram after the procedure showed improvement in intraventricular conduction defect and QTc duration, and also no significant ischaemic changes were noted. Myocardial perfusion scintigraphy was done after 6 months of procedure which showed improvement in contractile function and perfusion of left ventricle.
Conclusions:
Device closure in long-standing anomalous left coronary artery from pulmonary artery case is safe, and long-term follow-up is needed.
