Le cristallin est un organe radiosensible. En avril 2011, la Commission Internationale de Protection Radiologique (CIPR) fixait à 500 mGy la nouvelle dose seuil d’apparition d’effets déterministes au cristallin. Les patients qui présentent un anévrysme intracérébral rompu bénéficient d’examens scanographiques de l’encéphale (CT) justifiés, pour le diagnostic et le suivi post traitement, réalisé le plus souvent par embolisation radioguidée. Ces examens itératifs exposent le cristallin aux rayons X. Cet article chiffre l’exposition des cristallins de 18 patients ayant bénéficié en 2011 d’examens scanographiques encéphaliques suite à une rupture d’anévrysme intracérébral, et propose un moyen de diminuer cette exposition. Dans notre étude, 44 % des patients étudiés ont reçu une dose supérieure à 500 mGy au niveau des yeux, générée par les examens scanographiques et en particulier les CT de perfusion, indispensables pour évaluer le risque d’ischémie retardée par vasospasme. Nous avons observé qu’en positionnant les patients en hyper flexion cervicale lors de la réalisation des scanners de perfusion, la dose aux cristallins pouvait être diminuée de près de 50 %.

Une étude radioécologique a été menée sur les canaux rhodaniens du réseau hydraulique régional propriété de la région Languedoc Roussillon, gérés par BRL. Ces canaux transfèrent de l’eau du fleuve Rhône vers les territoires des départements du Gard et de l’Hérault à des fins d’irrigation et de production d’eau potable. Nos résultats montrent que les caractéristiques hydrauliques intrinsèques des canaux de transport d’eau influent sur la distribution solide/solution des éléments traces en transit et par conséquent sur leur transfert vers les milieux récepteurs. Si les concentrations en phase dissoute (eau filtrée) sont conservées, les concentrations en phase particulaire (matières en suspension et sédiments) sont significativement modifiées au cours du transit. Outre la ségrégation granulométrique des particules entre l’amont et l’aval du système, ces résultats sont très probablement liés à la production biologique autochtone (phyto et zooplancton). Ces résultats originaux soulignent le caractère atypique des canaux de transport d’eau quant au transfert des éléments potentiellement contaminants.

Objectif : comparer la dose délivrée aux patients et la qualité d’imageen routine clinique lors de la réalisation d’un scanner abdominal ne disposant pas destechniques de reconstructions itératives (RI) par rapport à un examen réalisé sur unscanner disposant des RI. Matériels et méthodes : il s’agit d’une étuderétrospective incluant 30 patients ayant eu deux examens abdominaux sur un scanner 40coupes (TDM40) et sur un scanner 256 coupes avec RI (TDM256). Les patients, suivis pourune pathologie abdominale chronique, ont eu un examen de même indication sur chaquescanner avec un protocole comprenant une phase abdomino-pelvienne au temps portal. Lalongueur d’acquisition, la dose efficace et le Produit Dose Longueur (PDL) ainsi que desévaluations quantitatives et qualitatives de l’image ont été comparés. Résultats: la dose efficace moyenne pour un examen était de 17,3 mSv avec le TDM40 (PDL :1019 mGy.cm) contre 11,1 mSv avec le TDM256 (PDL : 654 mGy.cm) soit une réduction de 35,8 % (p < 0,001). Les longueurs d’acquisitions et l’évaluation quantitative étaient comparables dans les deux groupes. L’évaluation qualitative étaitlégèrement supérieure sur le TDM40 mais aucun examen n’a été considéré comme sous-optimal.Conclusion : l’utilisation d’un scanner équipé de RI permet uneréduction significative de la dose efficace tout en préservant une bonne qualité d’image.

Classical beta spectra calculations are briefly described, highlighting the usualassumptions and limitations. To go beyond these usual assumptions, the numericalresolution of the Dirac equation for the atomic and beta electrons is necessary. Thisallowed us to determine the parameters λi involved in the theoretical shapefactors exactly. A systematic comparison between theoretical and experimental shapefactors led us to disqualify the usual λi = 1 assumption for all forbiddentransitions. The usual ξ-approximation was proved to be incorrect fornumerous first forbidden non-unique transitions, and for all higherorder non-uniquetransitions. A more accurate screening correction was defined and the atomic exchangeeffect was taken into account, an effect which is always neglected in usual calculations.The beta spectra of 63Ni and 241Pu, recently measured down to very low energies, arewell reproduced in our calculations. The exchange effect was demonstrated to have a greatinfluence on the spectral shape within this energy range.

Un cas exceptionnel de radiothérapie peropératoire intracardiaque a nécessité l’organisation de ce traitement administrativement, mais aussi en terme de radioprotection. En effet, le traitement devait être effectué dans un établissement non prévu pour des irradiations. Nous avons donc dû préparer, informer, simuler avant la réalisation. Avec l’énergie des photons de 50 kVp et les précautions prises il n’y a eu aucune incidence sur le personnel et l’environnement. Ce cas montre qu’il est possible d’envisager une externalisation de la radiothérapie peropératoire sous réserve des procédures d’autorisation et des mesures de radioprotection.

In the last few years there has been significant development of radiation therapy (RT)equipment with advanced imaging and delivery techniques, as well as treatment planningsystems. From this perspective, proactive approaches for risk assessment were identifiedas a powerful tool in modern radiation oncology. A multidisciplinary working group (WG)has been established in the framework of the Italian association for medical physics(AIFM) to promote the use of prospective approaches in the radiotherapy scientificcommunity. This paper describes the main actions carried out by the WG in order to collectinformation about the engagement of Italian medical physicists in the risk managementprocess, in reporting possible incidents in RT and in the procedures of collecting andanalysing near misses. In particular, the main scope of the study was to evaluate theactual level of experience in use of proactive risk analysis tools in modern RT by medicalphysicists. Finally, the measures implemented by the WG in order to promote the use ofsuch approaches, and consequently to contribute to enhancing safety and radiationprotection culture in radiation oncology are described.

For targeted radionuclide therapies, treatment planning usually consists of theadministration of standard activities without accounting for the patient-specific activitydistribution, pharmacokinetics and dosimetry to organs at risk. The OEDIPE software is auser-friendly interface which has an automation level suitable for performing personalizedMonte Carlo 3D dosimetry for diagnostic and therapeutic radionuclide administrations. Meanabsorbed doses to regions of interest (ROIs), isodose curves superimposed on apersonalized anatomical model of the patient and dose-volume histograms can be extractedfrom the absorbed dose 3D distribution. Moreover, to account for the differences inradiosensitivity between tumoral and healthy tissues, additional functionalities have beenimplemented to calculate the 3D distribution of the biologically effective dose (BED),mean BEDs to ROIs, isoBED curves and BED-volume histograms along with the EquivalentUniform Biologically Effective Dose (EUD) to ROIs. Finally, optimization tools areavailable for treatment planning optimization using either the absorbed dose or BEDdistributions. These tools enable one to calculate the maximal injectable activity whichmeets tolerance criteria to organs at risk for a chosen fractionation protocol. This paperdescribes the functionalities available in the latest version of the OEDIPE software toperform personalized Monte Carlo dosimetry and treatment planning optimization in targetedradionuclide therapies.

Radon (222Rn) is aradioactive gas that occurs naturally when uranium in soil and rock breaks down. Long-termexposure to 222Rnincreases the risk of lung cancer. The principal objective of this work was to determinethe 222Rn activityconcentration in an indoor air environment at the El-Dabaa site proposed for a nuclearpower plant project in Egypt using the track etch technique with LR115 detectors. Theannual average indoor 222Rn activity concentration in apartment buildings variesfrom 24 to 77 Bq.m-3, with a mean value of 54 Bq.m-3. The annual effective dosereceived by residents of the studied area was estimated to be 0.41 mSv. The annualestimated effective dose is less than the recommended action level by ICRP(3−10 mSv.y-1). The results from this workprovide a radiological assessment program and update the background of the naturalradioactivity map at the El-Dabaa site.

To go further in the optimization of treatment planning in selective internal radiationtherapy (SIRT), radiobiological aspects can be accounted for with the OEDIPE software andused to design fractionation protocols. Dosimetry was performed using data from99mTc-MAAevaluations of 10 patients treated for hepatic metastases with SIRT. The maximalinjectable activity (MIA) was calculated, using a tolerance criterion onBEDmean,healthyliver equal to 54 Gy2.5, for different fractionationprotocols, varying the number of fractions, the repartition of activity and the time delaybetween fractions. OEDIPE was also used to calculate BEDmean and the EUD to the tumoralliver (TL) that would be delivered with those MIAs. Compared with a single-injectionprotocol, the MIA is increased on average by 23% ± 3%, 36% ± 5% and 45% ± 7% for fractionation protocols with 2, 3and 4 equal fractions, respectively, while BEDmean,TL is increased by 15% ± 2%, 23% ± 4% and 29% ± 5%. EUDTL, calculated for oneevaluation, is increased by 51%, 115% and 159% using 2, 3 and 4 equal fractions,respectively. For this evaluation, the optimal activity repartition for two-fractionprotocols is (3/4 − 1/4) fortime delays of less than 4 days, (2/3 − 1/3) for time delays between 4 and 6 days and (1/2 − 1/2) for time delays superior to 6 days.Finally, this study confirmed that OEDIPE can be regarded as a tool for treatment planningoptimization and fractionation protocol design in SIRT.

Des résultats de mesures de tritium libre et organiquement lié ont été acquis de 2007 à 2012, en France métropolitaine, pour des échantillons de végétaux et de denrées prélevés en milieu terrestre, hors de l’influence potentielle d’un rejet. Le bruit de fond, déduit de ces données, n’évolue pas significativement; il est en moyenne de 1,4 ± 1,2 Bq/L pour le tritium libre et pour le tritium organiquement lié.

L’examen des résultats de mesure de 14C, notamment dans le cadre de lasurveillance environnementale requiert la connaissance de certaines spécificités ducomportement du carbone. Les principaux paramètres susceptibles d’influer sur lesmécanismes d’incorporation du carbone dans les matrices environnementales et donc sur lesniveaux mesurés en 14C sont succinctement rappelés. Des résultats de mesured’échantillons biologiques ont été acquis par l’IRSN dans des zones non directementinfluencées par des rejets d’installations nucléaires; ces chroniques permettent deprésenter les valeurs du bruit de fond naturel et rémanent, exprimé en activité spécifique(Bq 14C/kg C), hors de l’influence contemporaine d’un rejet local de14C.

Modern radiotherapy has increased demand for dose delivery verification. In this paper transmission portal dosimetry was considered. Portal detectors are a promising tool for 2D dosimetric verification and they are nowadays one of the most widely investigated topics. In this study an Electronic Portal Imaging Device (EPID) was positioned below the patient and the transmission images were captured during the irradiation. The principle of this verification consists of comparison of the acquired images with images predicted on the basis of the entrance fluence map and the tissue distribution in the patient. Such verification is not performed at any radiotherapy department in the Czech Republic. There is no system available for the prediction of transmission portal images. Even worldwide, there is still a lack of commercially available solutions. The aim of this paper is to present a new method of prediction of transmission portal images by means of the Monte Carlo (MC) method and the mathematical programming language MATLAB. The MC code EGSnrc (Electron Gamma Shower) was used. The validity of the presented method was verified by comparison of the predicted images with the acquired ones. The acquisition of EPID images was performed at the Hospital Na Bulovce. Three different validation tests were performed. In the first case, the EPID was irradiated by regular and irregular fields while there was nothing present in the beam path. In the second case, a water-equivalent phantom was added to the EPID and was irradiated by a number of irregular fields. In the third case, a real patient was present in the beam path and the EPID images were acquired during the patient’s treatment. The patient was irradiated by 8 treatment fields and the portal images were acquired during 5 treatment fractions. All of the acquired images were compared with the MC predicted ones by gamma analysis with gamma criteria of 3%, 3 mm. The average gamma values were 0.31−0.4, 0.34−0.4 and 0.35−0.61 in the first, second and third case, respectively. The results validate the developed method and demonstrate that MC is an effective tool for portal dose image prediction. MC may be favourably used for in vivo transmission dosimetry.

La scintillation liquide est une méthode d’analyse de choix du suivi radio toxicologiquedes travailleurs susceptibles d’être exposés à des sources émettrices βpurs. Les éléments étant présents à l’état de traces dans les urines, il est nécessaired’optimiser la technique de mesure afin d’obtenir la meilleure sensibilité. Ces dernièresannées ont vu apparaître des nouvelles générations de liquides scintillants, moinspolluants et moins toxiques, destinés à remplacer les liquides scintillants traditionnels.Ces scintillants dits de « nouvelle génération » possèdent cependant un inconvénienttechnique à leur utilisation : une dissipation plus longue de l’énergie émise par lephoton de fluorescence résultant de l’interaction entre le milieu scintillant et lerayonnement. Ce phénomène a un impact direct sur la qualité de la détection et amène àrechercher une valeur du Delay Before Burst optimale qui permette de distinguer lessignaux du rayonnement de la source à mesurer, de ceux venant du milieu environnant. Cetteétude vise à montrer l’importance du choix de la valeur du paramètre Delay Before Burstdans l’optimisation des limites de détection pour les mesures en scintillation liquide.L’influence de ce paramètre a été étudiée sur les différents radionucléides mesurés auLaboratoire d’Analyses Médicales Radiotoxicologiques (LAMR) de l’Institut deRadioprotection et de Sureté Nucléaire (IRSN); le 3H, le 14C et le89Sr. Notre étude compare les rayonnements β mesurés avec unliquide scintillant traditionnel : le Pico-Fluor 40 et un liquide scintillant de nouvellegénération : l’Ultima Gold LTT.

Les traitements utilisant des rayons X de basse énergie continuent à être réalisés maisen faible nombre. Le praticien se trouve en général près du patient pour maintenirl’appareil et expose donc ses yeux. Avec la modification envisagée de la limite maximaled’exposition du cristallin, des mesures ont été jugées nécessaires pour mieux en connaîtrel’exposition. Pour cela une simulation des traitements a été réalisée. Pour la dose d’untraitement superficiel de 42 Gy, l’exposition du cristallin derrière des verres radioatténuateurs se situe entre 130 et 43 μSv pour des distances entre 40 et 60 cm. Pourl’intra cavitaire les valeurs obtenues sont vingt fois plus faibles. L’atténuation par lesverres est supérieure à 90 %. Le port des lunettes avec verres radio atténuateurs permetde minimiser la dose au cristallin de façon significative. L’application de la nouvellelimite maximale n’aura pas d’impact sur le nombre de traitements.

The present work describes the technical information on issues associated with themanagement of disused Ra-226 radioactive sources. The different settings needed tocondition these sources in stainless steel capsules, working area setup, packagingpreparation, Ra-226 source characterization, capsule welding, leakage test and datainformation are completely described. The radium sources addressed in the present work aremainly those which have been used in medicine and for industrial applications, in the formof sealed radiation sources. In Egypt hospitals are among the largest users of sealedradium radioactive sources which are mostly used for brachytherapy. The inventory ofradium sources in large hospitals could be in the hundreds. There is a real risk thatsources may be lost or mishandled, if very restricted procedures are not followed. Anadditional problem with these sources is that they are often leaking, due to internaloverpressure. The small size of the sources, portability and high apparent value (due tothe materials of encapsulation) increase the risk of theft.

This paper presents a comprehensive survey of the tritium content of “reference waters”in real working conditions by French laboratories involved in environmental monitoring.Reference waters are virtually tritium-free waters used to determine blanks for theanalytical system and to check for any contamination. It is therefore crucial to be sureof the low tritium content of these waters as this can limit the sensitivity and accuracyof the measurements. Water from the commercial brand “Abatilles” is widely used in Frenchlaboratories and nine samples were analyzed in this study. Three samples from other deepaquifers were also included. The results all range from 0.004 ± 0.004 Bq/kg to 0.175 ± 0.011 Bq/kg, well below the 0.5Bq/kg advised by the French Safety Authority. The spread of the results can mainly beattributed to contamination through plastic during bottle storage. As expected, twosamples from demineralized tap water showed higher tritium activities (0.3–0.35 Bq/kg).Both waters are suitable as reference water for routine monitoring (DL of about 10 Bq/kg)but should be used with caution for activities in the Bq/kg range.

The ASN launched in July 2007 a system of notification of significant events dealing with ionising radiation, called ESRs. The ESRs notified to the ASN in the medical field have been increasing since 2007 and amounted to a total of 2300. The information showed that the medical activities with the most important implications for radiation protection regarding professionals are interventional radiology, with dose limit overruns, brachytherapy and nuclear medicine, with internal contamination of operators. For patients, deterministic effects were observed in interventional radiology as well as large doses in nuclear medicine, when the process of issuing radiopharmaceuticals is not managed, with the worst ESR being a partial removal of the thyroid. Many events involve leaks of radioactive waste and emphasise the need to strengthen the maintenance and monitoring facilities, and can cause major disruptions of service with delays in the delivery of care. The feedback emphasises the importance of empowering medical physicists and radiation protection officers to implement steps of quality management and risk management as well as clinical audits.
