Discriminant benzamide derivatives (DBD), the prototype of which is DO 710 i.e. N- [(1-propyl 2-pyrrolidinyl) methyl] 5-methyl sulfamoyl 2-methoxy benzamide, were compared to classical neuroleptics such as haloperidol in various behavioral and biochemical tests. Whereas the ID50 (or ED50) of haloperidol for antagonising various apomorphine-induced behavioral responses and producing catalepsy in rats were all around 0.1 mg/kg. DO 710 clearly distinguished catalepsy, HVA increase and apomorphine-induced licking and sniffing in rats (for which the ED50 (or ID50) were 13-54 mg/kg) from apomorphine-induced climbing, yawning and hypothermia (for which the ID50 were 1-2 mg/kg) (fig. 1, 2 and 4). Moreover, DO 710 and other DBD potentiated stereotypies in mice, whereas haloperidol and other classical neuroleptics did not (fig. 3). These features do not seem to be attributable to a heterogenous distribution of DBD in brain, since their ED50 for HVA increase and for inhibiting 3H-N-propylnorapomorphine binding in vivo did not differ in striatum and in limbic regions.

In in vitro binding experiments, DO 710 and other DBD discriminated two classes of 3H-domperidone binding sites in striatum, whereas only one component with a relatively low affinity for DBD could be detected in pituitary (fig.5-6, Table 3). 3H-DO 710 allowed charaterization of a D-2 site with a low affinity for DBD, which was fully sensitive to GTP regulation and present in pituitary and brain, and of a “D-4” site, preferred by DBD, which was little or not sensitive to GTP, present in brain and particularly enriched in olfactory bulb but absent in pituitary (fig. 7). 3H -azidosulpride (i.e. N-[(1-3H- propyl 2-pyrrolidinyl) methyl] 2-methoxy 4-azido 5-methylsulfamoyl benzamide), a photoactivable analog of 3H-DO 710, was used for irreversible labelling of dopamine recognition sites. When subjected to SDS-PAGE, receptor from striatum, pituitary and olfactory bulb co-migrated as a single band of 85 kDa (fig. 8).

These results may suggest the existence of two subclasses of dopamine D-2 receptor as targets for neuroleptic actions, one being preferred by DBD. The recognition subunits of these receptors have similar apparent molecular size.

Cet article présente brièvement les résultats d’une étude épidémiologique unicentrique standardisée avec seulement deux cotateurs entraînés, de l’incidence et des facteurs de morbidité des épisodes dépressifs non psychotiques de la puerpéralité à partir de 483 dossiers standardisés de jeunes accouchées constituant un échantillon au tiers parmi les 1232 accouchements survenus durant une année à la maternité du CHRU de Saint-Etienne. Ce travail a permis de diagnostiquer au moyen d’évaluations psychométriques quantifiées 35 cas de “post partum blues” (incidence 6.6 %). Le dépouillement des réponses aux 35 items (Tableau 1) a permis d’isoler par segmentation non arborescente (figure 1) deux groupes: un de femmes non-à-risque et un de femmes à haut risque de pathologie dépressive post puerpérale défini par la présence de quatre facteurs prédisposants: la présence d’antécédants psychopathologiques personnels dépressifs, la survenue de manifestations anxiodépressives et de traumatismes psychologiques durant la grossesse, la séparation précoce d’avec l’enfant après l’accouchement. Quant aux “post partum blues”, seule la primiparité apparaît avec une plus grande fréquence. Néanmoins leur faible incidence dans notre échantillon peut conduire à s’interroger sur la définition des critères de morbidité de cette pathologie.

Le développement de la recherche psychiatrique en Espagne est étudié dans cet article dans les champs et les domaines suivants : épidémiologie, clinique et psychopathologie, psychologie médicale, psychiatrie biologique, psychiatrie sociale et psychanalyse.

On a par ailleurs répertorié le nombre et les titres des publications scientifiques des cinq dernières années, dans plusieurs revues psychiatriques espagnoles. Cette étude montre une nette prédominance de travaux de psychiatrie biologique et de psychologie médicale.

Dans une troisième partie, sont analysés quelques aspects problématiques de la recherche, comme : les facteurs économiques, les facteurs de “structures de recherche”, les facteurs humains ainsi que les problèmes de compréhension mutuelle en présence de tendances conceptuelles différentes.

L’article se termine par certaines considérations sur les perspectives futures de la recherche, en considérant les nouveaux cadres législatifs, ainsi que la nécessité de s’adapter aux modèles et aux programmes européens de travail, dans l’optique de l’entrée de l’Espagne dans le Marché Commun.

The value of the Dexamethasone Suppression Test (DST) in predicting the therapeutic response to a specific antidepressant is a very controversial topic, probably due to methodological issues. The aim of this study was to evaluate the power of the DST in predicting the response of primary and secondary depressives, diagnosed according to Feighner criteria, to either clomipramine or maprotiline. Patients were screened using standardized psychiatric interview, Current and Past Psychopathology Scales, the Hamilton Rating Scale for Depression (HRSD), and the Beck Depression Inventory (BDI); the Michigan Discriminant Index was determined providing an endogeneity score. A 1 mg DST was performed. Treatment was assigned double blind and at random, according to a fixed schedule with administration of up to 225 mg of either drug for three weeks. After this period patients were again evaluated with the Hamilton Rating Scale for Depression and Beck Depression Inventory, and a DST was again performed.

Treatment reponse was defined in terms of reduction of HRSD and BDI scores. Serum levels of antidepressants were documented in a number of patients.

HRSD score reduction did not yield any significant results.

Insofar as BDI score reduction was concerned, nonsuppression on the DST (i.e. a post-dexamethasone cortisol value greater than or equal to 50 μg/l) predicted a better, or anyway a quicker, response to maprotiline; the DST results did not predict response in patients treated with clomipramine.

Despite some methodological pitfalls (a relatively small group of patients, insufficiently documented serum antidepressant levels) these results seem to warrant further investigation in this field.

• • Die Ablenkung des wissenschaftlichen Nachwuchses auf sozialpsychiatrische Reformpraxis in den 70-iger Jahren ;

• • Die Überlastung und Erschöpfung der Universitätskliniken durch übermäßige Lehraufgaben und kontinuierliche bürokratische Umorganisation, durch ungünstige Lösungen für Probleme der institutionellen und personellen Struktur ;

• • Extensive Auslegung von Datenschutzgesetzen ;

• • Die Sprachbarriere.

Forschungsförderlich sind :

• • Die “kritische Masse” von zwei psychiatrischen Großforschungseinrichtungen (MPIP und ZISG) ;

• • Die finanzielle Förderung und unabhängig-externe wissenschaftliche Kontrolle von Forschungsprojekten (einzeln, in Sonderforschungsbereichen und in Schwerpunktprogrammen) durch die DFG und andere Stiftungen ;

• • Regelmäßige Symposien und Tagungen wissenschaftlich aktiver Arbeitsgemeinschaften in speziellen Forschungsgebieten ;

• • Eine wachsende Zahl von Preisen für wissenschaftliche Leistungen.

Dann werden einige Beispiele patientenbezogener empirischer Forschung zu

• • Methodologie und Diagnostik,

• • Disposition, Pathogenese und Verlauf, und

• • Therapie- und Versorgungsforschung bei psychischen Krankheiten skizziert.

Schließlich wird es als eine zentrale Forschungsaufgabe der Zukunft angesehen, die explosive Entwicklung der Neurowissenschaften für die Lösung von Problemen der psychiatrischen Krankheitsforschung nutzbar zu machen und gleichzeitig die Methoden und Ergebnisse empirischer Forschung aus Psychologie und Soziologie im Sinne einer Mehr-Ebenenforschung zu berücksichtigen. In einer die konventionelle Nosologie übergreifenden Forschungsstrategie werden besonderes Gewicht haben :

• • Erfassung und Differenzierung von Indikatoren für State- und trait-abhängige Variable ;

• • Erfassung und Gewichtung der Entstehungsbedingungen von Dispositionsvariablen ;

• • Analyse der Wirkungsmechanismen psychopathogener Interaktionen zwischen Dispositionsvariablen und peristatischen Ereignissen ;

• • Erfassung von Prädiktoren für den spontanen Krankheitsverlauf sowie den Verlauf unter Behandlung ;

• • Analyse von Wirkungsmechanismen der Verlaufsgestaltung, insbesondere bei Remission und Chronifizierung psychischer Krankheit.

Extrapyramidal side-effects with drug-induced parkinsonism have not previously been reported following treatment with the atypical dibenzazepine-type neuroleptics, clozapine and fluperlapine. We have however observed reversible acute dystonia in a patient following high-dose fluperlapine treatment. The clinical signs of a discrete infantile cerebral palsy observed in our patient are probably one of the risk factors. Although the differential diagnosis of acute dystonia as opposed to tardive dyskinesia was difficult, the patient's symptoms fit more closely the former. Pharmacological aspects of the observed movement disorder are discussed.

Les protocoles du test de frustration de Rosenzweig de vingt et un sujets présentant un phénomène de Raynaud ont été analysés, dans le cadre d’une investigation psychodynamique effectuée à l’occasion d’une hospitalisation pour bilan étiologique et sans aucune sélection préalable. Par la suite, 13 des 21 sujets ont été considérés, d’après des critères physiques cliniques et paracliniques, comme des Raynaud primitifs (RI) et 8 comme des Raynaud secondaires (RII), le plus souvent d’origine sclérodermique. L’âge moyen des RII est plus élevé. L’investigation psychodynamique a été effectuée dans la méconnaissance mutuelle du diagnostic étiologique de la part du patient (à l’exception de 2 cas) et de l’investigateur.

Les résultats évoquent plusieurs différences dans le maniement de l’agressivité en situation de frustration et dans les stratégies de gestion des conflits, entre les RI et les RII. Les premiers extériorisent plus difficilement leur agressivité et ont recours plus fréquemment que les RII à des comportements d’esquive, se montrant plus passifs, plus conciliants mais aussi plus culpabilisés dans une situation de conflit. Les seconds paraissent plus à l’aise pour extérioriser leur agressivité et se montrent tendus vers la recherche d’une solution aux problèmes posés, tout en manifestant plus d’irritabilité que les RI devant la situation frustrante. En revanche, malgré une attitude globalement conformiste, leurs réponses ne concordent pas suffisamment avec les reponses “banales” de la population d’étalonnage ; en outre, l’extériorisation de l’agressivité chez les RII est bloquée lorsqu’elle menace directement le personnage frustrant, surtout si ce dernier joue un rôle parental interdicteur (rôle “surmoïque”). Les diverses attitudes défensives ne sont liées ni à l’âge ni à la durée d’évolution de la maladie chez les RI, alors que les RII extériorisent d’autant plus volontiers leur agressivité que leur maladie évolue depuis longtemps ; cette modification pourrait se faire au prix d’une vulnérabilité accrue face au stress et contribuer ainsi à la survenue d’une affection plus grave, à type de sclérodermie.

We compared patients with DSD or depression spectrum disease (family history positive for alcoholism or antisocial personality disorder in a first degree relative) to other depressives. DSD patients were likely to be diagnosed as having antisocial or borderline personality disorder, to be alcoholics, amphetamine abusers, or hysterics. They had more chronic depressive features, including hostility and self-pitying, bad-tempered, labile, and irritable mood, and they were nervous, worrisome, immature, overly dependent, and had tempestuous relationships. Their depressions were usually described as “reactive”, but their social outcomes were more likely to be poor.

Les propriétés psychostimulantes de trois doses (25, 50 et 75 mg) d’un agoniste partiel des benzodiazépines, la pipequaline (PK-8165, figure 1), ont été comparées à celles mesuràes sous placebo chez 18 volontaires sains.

Les mesures réalisées avant la prise, 2, 5 et 7 heures après la prise, comprenaient une batterie d’échelles visuelles analogiques (tableau 1) ainsi que 4 tests informatisés (discrimination visuelle, “visual scanning”, temps de réaction en champ central et latéral, “digit/digit”).

Les données analysées par analyse de variance à mesures répétées montrent que la pipequaline ne possède pas, aux doses utilisées, d’effet sédatif et que, en général, la dose la plus faible induit une diminution des temps de réaction et une augmentation de la capacité d’attention (figures 4 à 6). Quant aux lignes d’auto-évaluation, les sujets se sentent plus éveillés, plus calmes, l’esprit plus clair, plus adroits, plus énergiques, plus attentifs, plus intéressés et plus réservés, en général sous 25 mg (figures 2 et 3).

Ces résultats confirment ceux déjà observés dans une étude précédente et montrent, de surcroît, la sensibilité et le pouvoir de discrimination des méthodes utilisées.

Cette étude suggère que de faibles doses de pipequaline méritent d’être testées en clinique particulièrement dans une symptomatologie asthénique et, sous toute réserve, dans une symptomatologie anxieuse.

In order to further assess the alterations which might underly behavioral deficits associated with a reduced dopaminergic transmission, the effects of apomorphine at doses thought to stimulate dopaminergic autoreceptors were studied on rat operant behavior.

Low doses of apomorphine caused a reward deficit when animais were shifted from continuons reinforcement to fixed ratio schedules of food delivery (fig. 1). This effect could be accounted for by a decreased ability of secondary reinforcers to sustain responding and/or by a disruption of cognitive processes (Table 1). The apomorphine-induced reward deficit in the fixed ratio 4 schedule was reversed by “disinhibitory” neuroleptics including amisulpride, pimozide, pipotiazine and sulpiride, at low to moderate doses. Conversely, “conventional” neuroleptics such as chlorpromazine, fluphenazine, haloperidol, metoclopramide and thioridazine were found inactive in reversing the deficit caused by apomorphine (fig. 2). Results obtained after lesion of dopaminergic neurons by 6-hydroxydopamine suggested that the behavioral deficit induced by apomorphine was related not so much to a reduction in dopaminergic activity in given restricted areas such as the VTA (fig. 3), the nucleus accumbens (fig. 4) or the prefrontal cortex (fig. 5), as to a functional imbalance between mesolimbic and mesocortical dopaminergic systems.

L’alprazolam et le clobazam ont été comparés au cours d’une étude prospective randomisée en double aveugle d’une durée de quatre semaines, en stricte monothérapie à posologie ajustée en fonction de l’efficacité et de la tolérance du malade, chez 117 patients ambulatoires présentant un trouble névrotique anxieux selon les critères du DSM II, dont l’intensité était comprise entre 18 et 35 à l’échelle d’anxiété de Hamilton.

L’efficacité de l’alprazolam est significativement supérieure à celle du clobazam sur l’appréciation globale de l’efficacité selon le médecin (p = 0.04) et le malade (p = 0.03) sur l’un des symptômes-cibles (p = 0.02), sur la tolérance globale (p = 0.02), ainsi que sur le nombre d’effets secondaires (p = 0.05); il n’existe pas de différence significative entre les deux produits sur l’échelle de Hamilton ou sur la version abrégée à 24 items de l’échelle d’auto-évaluation B.A.T.E. (Bonis Anxiété Trait Etat). Les posologies journalières moyennes utilisées en fin d’essai ont été de 2.95 mg et 28.5 mg pour l’alprazolam et le clobazam respectivement.

Les résultats observés concernant l’efficacité et la tolérance des deux produits sont discutés en termes de comparabilité des groupes, de sous-catégories d’anxiété étudiées, de posologies utilisées, ainsi que des nuances observées selon qu’on raisonne en termes d’évaluation globale ou en termes d’échelles psychométriques.

The prospect for psychiatric research in Britain is bleak. The U.K. government reduced its funding of British Universities by about 10 % between 1980 and 1983 and is now imposing further reductions of about 2 % a year for the foreseeable future. Funding of the Research Councils is also being reduced at a similar rate. As a result many academic and technical posts in our medical schools have already been lost or “frozen” and many more seem destined to disappear before the end of the decade. Although charitable bodies like the Wellcome Foundation are attempting to provide additional funds to offset the damage serious harm is being done to British medical research, and to British science in general.

Psychiatric research suffers along with everything else. For the past generation our strength and our most important achievements have been in social psychiatry. Very few departments have the laboratories or the expertise to mount fundamental biological research and in the present financial climate they have little hope of acquiring this capacity. The Medical Research Council spends its shrinking funds as wisely as it can and there is still a great deal of expertise in our university departments and MRC units, but our capacity to compete with the United States is waning fast. We will do our best to continue to do research which is well designed, innovative and useful. But unless our financial predicament changes we will be responsible for a decreasing proportion of the most important and influential studies, particularly in the biological sphere in which the major developments of the next decade are likely to come.

L'anxiété sociale traverse toute la pathologie psychiatrique et est l’une des plaintes les plus fréquemment rencontrées dans notre Département des Thérapies Comportementales. Les catégories : phobie sociale et personnalité évitante du DSM III s’y réferent spécifiquement. Certains troubles affectifs semblent aussi liés à l’interaction sociale. Les critères diagnostiques et les diagnostics différentiels à l’intérieur de ce groupe hétérogène sont analysés d’après le DSM III et à l’aide d’autres méthodes d’évaluation.

Une évaluation critique de ces méthodes est faite, en vue de les adapter aux critères de trois types de troubles : • la réaction anxieuse psychophysiologique dans la phobie sociale, • les comportements d’évitement dans la personnalité évitante, • les distorsions cognitives dans la dépression atypique.

Une revue de traitements chimiothérapiques, cognitifs et comportementaux utilisés dans les trois types de troubles décrits antérieurement, est présentée.

Neuroleptic malignant syndrome, a dangerous complication of neuroleptic treatment, was first reported in the French literature in the 1960’s. Although long assumed to be rare, the syndrome has beat widely reported recently, particularly in the United States. Does this surge of reports reflect a true increase in the frequency of the syndrome, or merely increased scientific interest in the disorder? A review of available epidemiologic data favors the latter possibility: the syndrome probably continues to affect about 1% of neuroleptic-treated patients, a figure little different from the original French estimate more titan 20 years ago.

L’échelle Manie-Dépression de l’AMDP comporte 20 items dépressifs et 7 items maniaques cotés de 0 à 4 (Figure 2). Le score total de l’échelle se calcule par la différence entre les notes totales de dépression et de manie. Cette échelle nous paraît particulièrement bien adaptée pour évaluer les virages maniaques sous traitement antidépresseur. A ce propos, nous rapportons le cas d’une patiente bipolaire de 29 ans, hospitalisée pour un épisode dépressif majeur avec mélancolie, qui a présenté une inversion maniaque de l’humeur après 8 jours de traitement par adinazolam, une triazolo-benzodiazépine. L’arrêt du traitement a fait réapparaître la symptomatologie dépressive antérieure. Ce cas clinique illustre l’intérêt de l’échelle Manie-Dépression de l’AMDP pour le suivi longitudinal des patients bipolaires.

Certain demographic characteristics, including the limited size and stabiliiy of the population of Denmark, account for the development of genetic, epidemiological and prospective studies in the Danish psychiatric research context. Moreover, despite the predominance of biological psychiatry, psychotherapy arouses the interest of young psychiatrists.

Research is especially concentrated in university centers such as Copenhagen, Aarhus and Odense. The Institute of Psychiatric Demography, directed by Annalise Dupont, is responsible for centralizing data concerning hospitalized patients. It lias thus been possible to carry out longitudinal studies of patients presenting manic-depressive disorders, schizophrenia and mental retardation, as well as surveys on the prevalence of suicide according to region, and genetic studies based on the observation of monozygotic twins. The studies on schizophrenia particularly deal with the incidence of cancer and the influence of life events. The cytogenetics laboratory, directed by Johannes Nielsen, is especially concerned with the follow-up of 25,000 children born in the Obstetrics Department of the Aarhus University Hospital.

There are numerous other areas of research: comparative studies of the ICD 8 and the DSM III; studies dealing with the use of lithium in affective disorders; studies on the response to and modes of action of electroconvulsive therapy; ongoing studies of neurotic patients, conducted for more than 20 years; surveys on the efficacy of psychotherapy in schizophrenic patients.

The existence of benzodiazepine receptors was discovered by the research cenrter of the St Hans Hospital in Copenhagen. Current research at this center is devoted to the study of dopamine receptors, the relations between LSD receptors and serotonine, and study of the GABA System.

Although research is thriving in Denmark at present, it is threatened by a reduction in credits; the author analyzes the reasons behind this threat.
