The relationship between political decentralisation and the welfare state is much studied, and large-scale studies have repeatedly found that decentralised states have less generous welfare states. How do we fit that with other studies that emphasise the potential of decentralisation to raise welfare standards? This article argues that decentralisation, as a variable, is too broad and it is more efficient to focus on the structure of veto players in the central state, intergovernmental relations and intergovernmental finance. Those are the actual mechanisms that connect decentralisation to the welfare states, and they can all vary independently of decentralisation. It uses recent changes in the United States and United Kingdom as examples. The fragmentation and average weakness of the US welfare state is mostly due to a federal government riddled with internal veto points that permits considerable interstate variation and low overall average provision, while tight central control on finances in the UK means that most variation is in the organisation, rather than levels, of social services.

Distributive justice is ordinarily calibrated in monetary terms. But money is not the only resource that matters to people. Talk of the ‘work−life balance’ points to another: time. Control over one's time, the capacity to spend it as one wishes, is another important resource; and its distribution raises another important aspect of justice. Here I describe a new method of distinguishing how much time one has discretionary control over, net of the amount it is necessary to spend in certain ways given one's circumstances. To draw out the distributive-justice implications of these calculations, I contrast the most-to-least privileged, in terms of discretionary time: a person in a dual-earner couple with no children, versus a lone mother. The magnitude of the gap between the discretionary time enjoyed by the best and worst is a measure of temporal injustice. That gap is substantially larger in some countries (such as the US and Australia) than in others (such as Finland and Sweden). Conventional welfare-state interventions – tax and transfer systems, support for child care – contribute pretty similarly to reducing that particular gap across all the countries examined. Differing practices surrounding the dissolution of marriages with children potentially makes a much bigger difference. Differing labour-market policies might make a similarly large difference yet again.

This article presents an historical overview of the changing meaning of the patient-consumer, and specifically the role played by patient groups in constructing the patient as consumer. It is argued that patient groups were central to the formation of the patient-consumer, but as health consumerism was taken on by the state, they lost control of this figure. Competing understandings of what it meant to be a patient-consumer developed, a shift that raises further questions about the unity of claims made in the name of the patient-consumer.

A decade on from the Stephen Lawrence Inquiry, this article examines the contributions of social scientists to the Inquiry on two key issues: the meaning of institutional racism and the police response to racial violence. These academic inputs are characterised as instrumental and reflexive forms of knowledge. While social science applied to social policy is most effective in instrumental mode, rather than reflexively, there are various factors – such as the interpretation of evidence, media debate and the role of prominent individuals – that are more significant in assessing its consequences. The impact of these factors mean that, although academic work on these issues has been influential, the outcome appears to be that institutional racism has run its course and been disowned or downgraded, while racial violence has become subsumed within the broader category of hate crime. It is argued that the relationship between academic knowledge and policy requires a better grasp of the complexities of applying social science, and that is what this article aims to make a contribution to.

Despite the absence of an unemployment grant in South Africa, there is growing concern that other social assistance provision might nevertheless weaken work motivation and create a ‘dependency culture’. This study explores attitudes about the relationship between grant receipt and paid employment in South Africa. Using both quantitative and qualitative data, it examines whether there is evidence of a ‘dependency culture’, the nature and strength of labour market attachment among grant recipients, and the opportunities and barriers to employment they face.

We found that both those in and out of work placed a high value on paid employment. Joblessness had not become ‘normalised’, and all categories of the workless were extremely motivated to get work. Grant recipients did not subscribe to a distinctive culture but to mainstream values and aspirations. While some benefit claimants subscribed to popularly promoted prejudices about other social assistance claimants, our findings appear to counter recent concerns about potential unintended effects of the current grant system. The key factors in reducing people's chances of finding employment seem linked to the structural conditions of the labour market and the wider economy rather than the motivational characteristics of the unemployed and the arrangements of the grant system.

Internationally, traditional approaches to social assistance (welfare) have increasingly been replaced with ‘active’ labour market policies. Alongside other industrialised countries, Canada embraced this shift, with its emphasis on the ‘shortest route’ to paid employment.

There has been little research on the outcomes of these dramatic changes in Canada, especially longer term. This article explores the post-welfare labour market experiences of people who were on social assistance in Canada in 1996. It uses the longitudinal micro-data files of the Survey of Labour and Income Dynamics (SLID) from Statistics Canada, which tracks a panel of recipients over five years. We examine the mixing of work and welfare, the transition from welfare to work, and selected labour market indicators – primarily hours of work and wages – that those in receipt of social assistance face in assuming paid work.

Those leaving welfare for work face precarious employment opportunities. Leavers earn lower wages, work fewer hours and consequently have lower annual earnings than non-recipients. Over time the gap narrows but remains significant, even after six years. Returns to welfare are frequent. Overall, even after six years most social assistance recipients remained marginalised in the periphery of the labour market.

Although health inequalities in England reflect underlying deprivation, there is considerable variation among deprived areas in the extent to which these inequalities are narrowing. Using survey data from 15 local authority areas in North West England, and Ragin's technique of Qualitative Comparative Analysis, contextual features and ways of working in these areas are shown to combine in systematic ways with recent trends in inequalities as measured by premature mortality. For circulatory diseases, a narrowing mortality gap showed a clear association with smoking cessation services that accorded with a best practice description, combined with a local population with relatively more people aged 65 or older. For cancers, a narrowing mortality gap was associated with areas that combined relatively low population mobility with a professional working culture described as one of individual commitment and championing. These findings reveal the complexity of meeting health inequality targets and applying evidence to this endeavour, since both ways of working and context appear to be important to making progress. Both need to be understood case by case if targets are to be locally realistic and evidence applied where local practice is known to matter to the outcome.

This article discusses reforms of the design of the organisational arena for policy implementation and service provision in European welfare states, focusing on the policy areas of income protection and activation. First, it discusses and compares recent reform programmes in four countries: the UK, Germany, Denmark and the Netherlands. The comparison shows that these reforms share some common characteristics: the establishment of one-stop agencies, decentralisation, the introduction of quasi-markets for the provision of activation services, a reduction of the role and influence of social partners and the use of new public management instruments in managing public agencies. Secondly, the article argues that these reforms are not merely reflecting new ways of thinking about organising the public sector and providing public services, but should also be interpreted as responses to policy administration and implementation problems arising in the process of making welfare states more activating.

This article reviews two rounds of pension reform in ten Latin American countries to determine whether they are moving away from individual retirement accounts (IRAs). Although the idea is provocative, we conclude that the notion of ‘moving away from IRAs’ is insufficient to characterise the new politics of pension reform. As opposed to the politics of enactment of IRAs of the late twentieth century, pension reform in Latin America in recent years has combined significant revival of public components in old-age income maintenance with improvement of IRAs. Clearly, the policy prescriptions that were most influential during the first round of reforms in Latin America have been re-evaluated. The World Bank and other organisations that promoted IRAs have recognised that pension reform should pay more attention to poverty reduction, coverage and equity, and to protect participants from market risks. The experience and challenges faced by countries that introduced IRAs, the changes in policies by international financing institutions, and the recent financial volatility and heavy losses experienced in financial markets may have tempered the enthusiasm of other countries from applying the same type of reforms. Scholars and policy-makers around the globe could benefit from looking closely at these changes in pension policy.

People with mental illness can be profoundly disabled and at risk of social exclusion. Transitional models of supported housing have limited effectiveness in improving community participation. Stable, individualised psychosocial housing support programmes have been found to assist in improving mental health and decreasing hospitalisations, but little is understood about whether or how these programmes facilitate social and community participation. This article argues that, if certain supports are available, supported housing models can assist people with high levels of psychiatric disability to participate meaningfully in the community. To make this case, the article uses findings of a longitudinal evaluation of a supported housing model in Australia: the Housing and Accommodation Support Initiative Stage One (HASI). HASI is a partnership between the New South Wales Government Departments of Health and Housing and non-government organisations. It is a coordinated approach that provides clients with housing and community-based clinical support, as well as support with daily living skills and community participation. An analysis of questionnaire, database, interview and clinical data is used to demonstrate how HASI contributes to increased social and community participation. The article concludes with policy implications for supported housing models that aim to facilitate meaningful community participation for people with mental illness.

Introducing individual savings accounts into the system of social security may be an innovative way to reorganise European social security systems. This article examines the merits and drawbacks of this modernisation strategy using the Transitional Labour Market approach as a frame of reference. On the basis of normative criteria derived from this approach, we perform an evaluation of the Dutch life-course scheme (‘Levensloopregeling’). This scheme is a unique and pioneering arrangement that offers employees a fiscally facilitated option to save money to finance periods of unpaid leave. Following the assessment of the Dutch case, we identify several pitfalls of reforms based on individual savings accounts. Finally, we put forward some proposals to overcome these shortcomings.

This longitudinal study examined the association between heavy alcohol use, alcohol- and drug-screening requirements, and social support network variables and mothers' welfare participation in the United States. The study was a secondary data analysis of 3,517 mothers. The sample was extracted from National Longitudinal Survey of Youth data gathered in 1994–2004. Results of logistic regression show welfare participation is not associated with heavy alcohol use or alcohol- and drug-screening requirements, but is associated with a history of reported heavy alcohol use, informal help with childcare, and scant human capital. Results also indicate that alcohol- and drug screening required under TANF may not exclude heavy drinking mothers from TANF participation, and that social support networks do not cancel heavy drinking's association with participation. Policy implications are discussed.

The ways in which government supports families and protects children are always a fine balance. In recent years, we suggest that this balance can be characterised increasingly as ‘child-centric’, less concerned with families and more focused on individual children and their needs. This article charts the changes in families and government responses over the last 40 years, and the way this is reflected in organisational and administrative arrangements. It notes in particular the impact on everyday practice of the introduction of information and communication technologies. Findings are reported from recent research which shows the struggles faced by practitioners who try to manage systems which separate children from their familial, social and relational contexts. As a consequence, we suggest, the work has become increasingly fragmented and less mindful of children's life within families. While the data and analysis draw on research carried out in England, we suggest that similar changes may be going on in other Western liberal democracies.

The recent German welfare state reform with the introduction of Social Code II has created a complex situation for the labour market integration of long-term unemployed people with disabilities. A range of social laws with differing underlying principles is now applicable. In this article, we examine the effects that the implementation of this social code has on long-term unemployed people with disabilities. We show that their integration patterns changed. This is due to the building of new institutions responsible for labour market integration, followed by a temporary destabilisation of work routines at the operational level. Additionally, more persistent consequences occur because the inconsistencies of the relevant laws are creating an area of conflict, which is increasing the risk of marginalising people with disabilities in terms of labour market integration.

In conducting his research for The Last Refuge (1962), Peter Townsend visited 173 public, voluntary and private residential care homes for older people in England and Wales. Drawing on his data, now archived at the University of Essex, we traced the subsequent history of these homes and revisited a sample that were still functioning as care homes in 2006. In this article, we focus on the 42 private homes he visited, some of which remain open and were revisited by us in 2005–06. The pre-1980 history of private sector residential care provision for older people is an elusive and poorly charted topic. Drawing on the two data sets for then and now, this article contributes new insights into this area of UK policy and practice.

The article is focused on the analysis of flexicurity arrangements in the Italian case with a twofold aim. The first aim is substantive: analysing and evaluating recent labour market reforms in Italy with particular reference to the possible emergence of a new flexicurity model. The second goal is to contribute to the debate on labour market development and flexicurity by enriching and refining the available analytical tools. More specifically, we argue that the traditional conceptual ‘lenses’ focusing on the ‘insiders’/‘outsiders’ divide are inadequate to grasp the ongoing changes in the Italian labour market. This leads us to identify a third category of workers, the ‘mid-siders’, who are increasingly relevant in the Italian case and may prove to be relevant in other labour markets too. Moreover, we introduce the concept of ‘selective flexicurity’, referring to the fact that in the Italian case flexibility and security measures have been applied differently across the various categories of workers.
