Two phenotypes of human influenza A(H1N1) virus are currently circulating in Japan. One (group 1) agglutinates both chicken and goose red blood cells (CRBC and GRBC), the other (group 2) agglutinates GRBC but not CRBC. In the 1995/6 season, group 2 viruses accounted for 70% of the H1N1 viruses isolated in MDCK cells. The 1995/6 viruses were located on two branches of the genetic tree. One branch contained both group 1 and group 2 viruses and the other branch contained only group 2 viruses. Group 2 viruses had aspartic acid at residue 225 in the haemagglutinin (HA) protein, the key amino acid residue for group 2 phenotype. The HA protein of group 1 viruses had a change from aspartic acid to asparagine at residue 225 and the expressed HA protein of these viruses adsorbed CRBC. Serial passage of group 2 viruses in MDCK cells or embryonated chicken eggs caused these viruses to gain the ability to agglutinate CRBC. MDCK-adapted viruses had the same amino acid sequences of HA polypeptide as the original ones, but egg-adapted viruses had changed amino acid sequences. The expressed HA protein from one egg-adapted virus that originally belonged to group 2 adsorbed CRBC.

It has been shown that certain strains of Salmonella grown in nutrient broth containing sulphite and bismuth, develop variants that are incapable of reducing sulphite to sulphides in the media of Wilson & Blair.

The altered strains showed no alterations in their antigenic and biochemical characters.

To achieve the elimination of the hepatitis C virus (HCV), sustained and sufficient levels of HCV testing is critical. The purpose of this study was to assess trends in testing and evaluate the effectiveness of strategies to diagnose people living with HCV. Data were from 12 primary care clinics in Victoria, Australia, that provide targeted services to people who inject drugs (PWID), alongside general health care. This ecological study spanned 2009–2019 and included analyses of trends in annual numbers of HCV antibody tests among individuals with no previous positive HCV antibody test recorded and annual test yield (positive HCV antibody tests/all HCV antibody tests). Generalised linear models estimated the association between count outcomes (HCV antibody tests and positive HCV antibody tests) and time, and χ2 test assessed the trend in test yield. A total of 44 889 HCV antibody tests were conducted 2009–2019; test numbers increased 6% annually on average [95% confidence interval (CI) 4–9]. Test yield declined from 2009 (21%) to 2019 (9%) (χ2P = <0.01). In more recent years (2013–2019) annual test yield remained relatively stable. Modest increases in HCV antibody testing and stable but high test yield within clinics delivering services to PWID highlights testing strategies are resulting in people are being diagnosed however further increases in the testing of people at risk of HCV or living with HCV may be needed to reach Australia's HCV elimination goals.

1. That there is reason to believe that sporadic cases of cerebro-spinal fever occurred in Hong Kong previous to the outbreak of the epidemic in 1918.

2. That the epidemic followed a widespread infection of influenza colds.

3. That the mortality out of a total of 1040 cases was 85–48 per cent.

4. That the most susceptible age was found to be the age period under five years.

The younger the individual, the greater the susceptibility, with the added proviso that the extremes of life suffered most.

5. That once the epidemic was established, a drop in the temperature, steadily maintained, accompanied by an increase in the saturation of the air as regards moisture, associated with a lack of sunshine, was found to be followed after a lag of a few days, by a rise in the number of cases. This lag showed a tendency to increase as the epidemic progressed, from three to four days at the beginning to six to seven days at the end of the epidemic.

6. That the number of cases in any given area varied directly with the overcrowding in that area, and this bears out the contention that overcrowding is one of the most important features in the epidemiology of cerebro-spinal fever.

7. That the housing conditions in Hong Kong are such as to necessitate immediate action on the part of the Government, to safeguard the public health of the colony.

8. That new entries to Hong Kong formed a large proportion of the cases, and that this appears to be an important factor in the epidemiology of the disease.

9. That the poorest, most hard worked and badly housed portion of the community suffered most in the epidemic.

10. That the recognition and isolation of carriers is impracticable in an epidemic of any considerable dimensions.

11. That in the present condition of the knowledge of the means by which infection is spread, the wearing of a screen composed of some impervious material such as celluloid would seem to afford complete protection to the wearer against droplet infection.

12. That known carriers should be compelled to wear this screen until such time as they are proved free from the meningococcus. Thus protected, the carrier could pursue his usual avocation and the community be safeguarded from infection at a minimum of expense.

We evaluated the incidence rate of culture-positive central nervous system tuberculosis (CNS TB) in France in 2007 and its time trend between 1990 and 2007. We used a capture–recapture analysis by using data recorded in 2007 by the mandatory notification system and the national network of the National Reference Centre (NRC). The 2007 sensitivity of the NRC was 79·4%. The previous sensitivity for 2000 (75·6%) and that for 2007 yielded a pooled estimate of 77·4% (95% confidence interval 64·8–88·0), which was used to extrapolate the number of culture-positive CNS TB cases from those reported in four surveys (1990, 1995, 2000, 2007). The extrapolated number of culture-positive CNS TB cases fell from 90 to 35 between 1990 and 2007, and the extrapolated incidence rates fell from 1·6 to 0·55 cases/million (P < 0·001). This favourable trend should be closely monitored following the change of the BCG vaccination policy in 2007.

With regard to the influence of malarial infection on the Wassermann test some controversy prevails:

Wassermann and Lange (1913) refer to Meyer and Bonfiglio who, working in Italy, obtained a positive reaction in 80 per cent. of the cases where the blood contained parasites; in some of their cases positive reactions were obtainable for several months after the disappearance of fever and parasites. For this reason Wassermann and Lange advise to put every case, coming from a malaria-infected country and showing a positive reaction, under quinine treatment and to repeat the test after a lapse of six weeks.

In 1956–58 septic lesions in men of the R.A.F. accounted for about 3% of all admissions to sick-quarters or hospital and about 5% of admissions excluding respiratory disease; 1% of all men in the service were admitted with sepsis each year. Incidence fell with age, particularly up to the age of 25 years. The rates for women were about 50% higher.

Recruits experienced more than twice as much sepsis as trained men and boy apprentices. Officers had only half the admission rate of other ranks. These contrasts could have been explained by differences in age. There was no evidence of seasonal variation in the incidence of sepsis, but in recruits and boy apprentices epidemics of sepsis unrelated to season occurred.

This study explored how internet queries vary in facilitating monitoring of pertussis, and the effects of sociodemographic characteristics on such variation by city in Shandong province, China. We collected weekly pertussis notifications, Baidu Index (BI) data and yearly sociodemographic data at the city level between 1 January 2009 and 31 December 2017. Spearman's correlation was performed for temporal risk indices, generalised linear models and regression tree models were developed to identify the hierarchical effects and the threshold between sociodemographic factors and internet query data with pertussis surveillance. The BI was correlated with pertussis notifications, with a strongly spatial variation among cities in temporal risk indices (composite temporal risk metric (CTRM) range: 0.59–1.24). The percentage of urban population (relative risk (RR): 1.05, 95% confidence interval (CI) 1.03–1.07), the proportion of highly educated population (RR: 1.27, 95% CI 1.16–1.39) and the internet access rate (RR: 1.04, 95% CI 1.02–1.05) were correlated with CTRM. Higher RRs in the three identified sociodemographic factors were associated with higher stratified CTRM. The percentage of highly educated population was the most important determinant in the BI with pertussis surveillance. The findings may lead to spatially-specific criteria to inform development of an early warning system of pertussis infections using internet query data.

Streptococcus pneumoniae serotype 35B, a non-vaccine type, is a major contributor to the increase in pneumococcal infection post-vaccination. We aimed to understand the mechanism of its spread by characterizing 35B. The serotype, type 1 pilus (T1P) positivity, and antimicrobial susceptibility of 319 isolates in 2018–2022 were analysed and compared with those of isolates in 2014–2017 to find the changes. 35B accounted for 40 (12.5%) isolates. T1P positivity was notably higher in 35B (87.5%) than in the other serotypes. To confirm the role of T1P, an adhesion factor, we compared adherence to A549 cells between T1P-positive 35B isolates and their T1P-deficient mutants, showing contribution of T1P to adherence. Penicillin-non-susceptible rate of 35B was 87.5%, and meropenem-resistant 35B rate was 35.0%, which increased from 14.5% of 2014–2017 (p = 0.009). Multilocus sequence typing was performed in 35B strains. Prevalence of clonal complex 558, harbouring T1P and exhibiting multidrug non-susceptibility, suggested the advantages of 35B in attachment and survival in the host. The emergence of ST156 isolates, T1P-positive and non-susceptible to β-lactams, has raised concern about expansion in Japan. The increase of serotype 35B in pneumococcal diseases might have occurred due to its predominant colonizing ability after the elimination of the vaccine-serotypes.

Knowing the burden of severe disease caused by influenza is essential for disease risk communication, to understand the true impact of vaccination programmes and to guide public health and disease control measures. We estimated the number of influenza-attributable hospitalisations in Spain during the 2010–2011 to 2019–2020 seasons – based on the hospitalisations due to severe acute respiratory infection (SARI) in Spain using the hospital discharge database and virological influenza information from the Spanish Influenza Sentinel Surveillance System (SISSS). The weekly numbers of influenza-attributable hospitalisations were calculated by multiplying the weekly SARI hospitalisations by the weekly influenza virus positivity, obtained from the SISSS in each season, stratified by age group and sex. The influenza-related hospitalisation burden is age-specific and varies significantly by influenza season. People aged 65 and over yielded the highest average influenza-attributable hospitalisation rates per season (615.6 per 100,000), followed by children aged under 5 (251.2 per 100,000). These results provide an essential contribution to influenza control and to improving existing vaccination programmes, as well as to the optimisation and planning of health resources and policies.

1. The appearance and disappearance of typhoid bacilli in the faeces in these four cases bore no relation to changes in the diet nor to the physical state of the stools, but the later they appeared the shorter was the duration of their appearance.

On the disappearance of typhoid bacilli from the stools the intestinal flora tended to become more simple.

2. B. coli was the only organism invariably present at every examination.

3. Streptococci were very much more abundant in the earlier stages of the disease, when milk formed the greater part of the diet, than in the later stages.

4. In two cases where boils occurred on the body the causative organism had previously been isolated in large numbers from the faeces.

5. With stools slightly alkaline to litmus the flora in these cases was relatively simple and fermentative in type. There is no apparent advantage, therefore, in giving a high carbohydrate diet except in cases of marked alkalinity and putrefaction.

Influenza C virus antigen could be detected by immunofluorescence in chick amniotic epithelium 4 hr. after inoculation with undiluted amniotic fluid. Antigen could be detected in the nucleus of rhesus monkey kidney tissue cultures 10 hr. after infection. Only cytoplasmic fluorescence was observed 17–21 hr. after infection.

The pattern of replication was similar to that reported for group I myxoviruses, indicating that the virus is correctly classified as an influenza virus.

The objective of this study was to determine the optimal number of respiratory samples per outbreak to be tested for institutional respiratory outbreaks in Ontario. We reviewed respiratory samples tested for respiratory viruses by multiplex PCR as part of outbreak investigations. We documented outbreaks that were positive for any respiratory viruses and for influenza alone. At least one virus was detected in 1454 (85·2%) outbreaks. The ability to detect influenza or any respiratory virus increased as the number of samples tested increased. When analysed by chronological order of when samples were received at the laboratory, percent positivity of outbreaks testing positive for any respiratory virus including influenza increased with the number of samples tested up to the ninth sample, with minimal benefit beyond the fourth sample tested. Testing up to four respiratory samples per outbreak was sufficient to detect viral organisms and resulted in significant savings for outbreak investigations.

This report summarizes closed, family, and open studies conducted sequentially over a 10 month period with the Cendehill rubella virus vaccine in more than 16,000 children and adolescents. This strain of rubella was attenuated by serial propagation on primary rabbit kidney cell cultures. Inoculation of the Cendehill vaccine produced seroconversion in 97% of the 3589 susceptible (seronegative) vaccinated persons. There was no spread of the virus to susceptible controls living in close contact with those vaccinated. The vaccine was well tolerated. No arthritis or arthralgia occurred in 860 female subjects 13–18 years of age who were included in the study. The Cendehill vaccine would appear to meet the requirements of an acceptable vaccine.

The cases which from the subject of the following note occurred in the district of Dr Chas. Jackson, M.O.H., of Fulham, and I am greatly indebted to him for so kindly furnishing me with their histories and with samples of the patients' serum.

1. Opinions upon the relationship between anti-rabies immunity and the rabicidal power of rabies-immune serum have been stated.

2. The rabicidal antibody content of the sera of rabbits immunised with etherised virus, living fixed virus and carbolised virus has been determined while at its maximum and compared with the degree of immunity acquired after each method of treatment.

In twenty-five consecutive cases in which nasopharyngeal cultures have been made from patients suffering from bacteriologically proved cerebro-spinal meningitis, meningococci have been obtained.

A striking similarity between the organisms from the two sites has been shown to exist for not only are they of the same agglutinable type but in the majority of instances individual peculiarities of agglutination have corresponded. The closeness of relation has been further demonstrated by the carbohydrate reaction in one case in which the unusual property of bleaching glucose litmus broth to a yellow colour was possessed by both organisms. Nevertheless, in a certain number of instances, though organisms of a different agglutinable type have not been isolated, as by other workers, there have been found certain minor variations of agglutination. That most if not all of these variations are due to alteration of the agglutinable capacity of one of the organisms seems probable, as in the six cases where most marked difference existed (Nos. 1, 3 and 24 of Type I and Nos. 15, 16 and 18 of Type II) the chances of the other alternative, namely that a second infection had happened in each case to be of the same agglutinable type, are remote.

In the twenty-five cases the proportion of the types present has been shown to agree with that found by other workers during the same period. Seven of the infections (28 per cent.) have been due to Type I and eighteen (72 per cent.) to Type II.

Ten of the cases have occurred in children under the age of two years, and the proportion of the two types among them has been approximately the same as among older patients.

Animals protected from fleas by means of a sufficiently broad layer of “tangle-foot” and placed in plague-infected houses do not contract plague, but the control animals, not so protected, on several occasions (24 per cent.) developed the disease. Out of 247 fleas caught on the “tangle-foot,” 60 p.c. were human, 34 p.c. were rat and 6 p.c. were cat fleas. Plague-like bacilli were demonstrated in the stomach contents of one out of 85 human fleas dissected, and of 23 out of 77 rat fleas.
