Si è voluto studiare l'alimentazione dei gemelli nel primo semestre di vita e le cause che determinano la scelta di uno piuttosto che di un altro tipo di alimentazione. A tale scopo, si sono prese in considerazione 500 coppie di gemelli che frequentano l'ambulatorio pediatrico dell'Istituto Mendel di Roma.

Dall'esame dei dati (tabella), risulta evidente come vi sia una netta maggioranza (97.8%) di coppie con allattamento concordante qualitativamente nei due gemelli.

La causa prima dell'allattamento discordante materno-artifìciale è la scarsità della secrezione lattea nella madre che impedisce l'alimentazione di ambedue i figli, unita a qualche particolare condizione di uno dei due (minor peso, presenza di fattori morbosi intercorrenti, minor tolleranza verso l'allattamento artificiale, etc.) che fa optare per un allattamento discordante, piuttosto che per uno misto in ambedue.

A volte, anche se la madre ha una quantità di latte sufficiente, ed uno dei due bambini deve essere temporaneamente allontanato da lei per una malattia intercorrente o per ricovero in reparti speciali per immaturità, si incontra notevole difficoltà alla sua riassuefazione alla suzione al seno.

Dalla tabella risulta come, su 500 casi, solo il 18% ha un allattamento materno concordante e completo fino all'inizio del divezzamento (intorno al V-VI mese). Il 19.2% è allevato con allattamento misto materno-artificiale con poppate alterne e complementari; il 38.8% con allattamento artificiale completo fin dai primi giorni; il 10.2% con allattamento materno nei primi mesi, poi misto; il 14.2% materno nei primi mesi, poi artificiale; il 2.4% misto nei primi mesi, poi completamente artificiale.

Weights of developing incisors in 10 pairs of twin fetuses have previously been related to ages in order to compare dental growth status (Stack, 1963). Ages (T0) at which weights (W) of mineralised portions of teeth of the temporary dentition become significant have been computed (Stack, 1968), using a Fortran program giving maximum likelihood estimates of the required parameters, based on the statistical model of Angleton and Pettus (1966).

Availability of the additional parameter T0 allows a reexamination of the previous data, now fortified by observations on incisors from five further pairs of twin fetuses. Estimates of ages were made from the relationship (T—T0) = k·W½, where values for upper and lower central incisors were 2.0 and 2.8 for k, 19.5 and 18.5 for T0, respectively. Less reliable estimates were obtained from the lateral incisors. These values were computed from observations on 40 dentitions from normal singleton fetuses. Ages were also estimated from tabulated body weights (Documenta Geigy).

In his recent book Dr. Roy J. Schepherd states that:

“Further information of genetic effects is likely to be derived from comparisons of identical (monozygotic) and fraternal (dizygotic) twins. Gedda (1961) has already shown that identical twins are more likely to have a similar interest in sports than are fraternal twins. It would be interesting to carry out comparisons of maximum oxygen intake and other objective measurements of endurance fitness on the two classes of twins, but this crucial experiment has yet to be completed”.

We are very glad to be able to announce here that Dr. Schepherd's wish is being fulfilled within the scope of a wider research project undertaken jointly by the Gregor Mendel Institute for Medical Genetics and Twin Research and the Italian Olympic Committee's Institute of Sports Medicine under the auspices of the Italian Sports Medicine Federation.

The studies we have undertaken aim to assess the respective roles of genetic and environmental factors as related to selection and training in sports and physical education.

The twin method affords, through a comparison of MZ and DZ within-pair concordances, an assessment of the degree of genetic conditioning in any individual trait.

It is quite obvious that the possible influence of training on any given trait is inversely proportional to the degree of genetic conditioning of the same trait.

If a given trait, required for a certain type of activity, is completely or almost completely conditioned by the genotype, we should seek those who have inherited it from their parents without wasting time and effort in useless training sessions.

As we can see, the scientific information we are seeking has great practical importance in the selection and training of athletes in every single field of sports and physical education.

The purpose of this paper is to draw attention to the statistical implications of abortion risks being greater for twins than singletons, greater for MZ than DZ twins, and greater for one member of a twin pregnancy than both members. This last fact is important, because if one twin aborts the other is usually mistaken for a singleton at birth; thus allowing a given number of nonconcordant abortions to have a marginally greater effect on twin frequency than double the number of concordant abortions.

This feature of abortion hazards is shown in Tab. I, which is based on Sandon's calculations of concordance effects. The figures show how much greater is the effect of a high rate of nonconcordant abortions (Case B) than the effect of an equally high rate of 90% concordant abortions (Case A) or a lower rate of 60% concordant abortions (Case C). In other words, a low frequency of twins in general (and MZ twins in particular) should be typical of an abortion-ridden population and might be a means of recognizing diseases whose prevalence rates are misleading, because they are associated with an abortion hazard (i.e., inherited diseases and diseases acquired during embryogenesis).

Il problema dell'ereditarietà della gemellazione non può dirsi ancora convenientemente risolto (Gedda e Brenci, 1964), benché si possano formulare alcuni postulati:

1) La gemellazione è un fenomeno familiare di tipo ereditario;

2) L'influenza dell'eredità si manifesta in modo più sensibile nello stipite materno che in quello paterno;

3) L'espressione zigotica del fenomeno gemellare (cioè, la variabilità qualitativa e quantitativa della gemellazione) è da ricondurre alla variabilità qualitativa della medesima unità ereditaria;

4) La gemellazione è un carattere che riguarda la madre, ma che si evidenzia solo nella generazione filiale; probabilmente si tratta di un carattere recessivo limitato al sesso.

Le gonadotropine umane usate per la terapia della sterilità femminile hanno prodotto numerose gravidanze multiple non MZ, confermando l'ipotesi che la tendenza a generare gemelli DZ corrisponda a deficit o eccesso di un sistema ormonico controllato dal genotipo (Gedda, 1965). Sarebbe l'eccesso primario di FSH (o secondario, in rapporto a difetto di LH o di progesterone) a provocare la maturazione di più oociti e forse, qualora questa situazione si verifichi a fecondazione avvenuta, a trasformare lo zigote in un protozigote. Anche la gemellazione MZ sarebbe pertanto provocata dal medesimo sistema ormonico, controllato dal genotipo, precedentemente ipotizzato come fattore causale della gemellazione DZ.

Acid-base balance was analysed in cord blood after normal spontaneous delivery of twins. Whereas in the first twin the balance results entirely comparable with that of normal singletons, second twins exhibit most of the time some pathological degree of acidosis, the cause of which is intrauterine hypoxia.

The importance of this phenomenon is discussed in relation to the conduct of labour and the value of the twin method in genetical investigation.

Leukocytic infiltration of the umbilical cord, particularly of the umbilical vein, is common both after normal and abnormal delivery (Tab. I).

The significance and the cause(s) of this phenomenon, however, are still highly controversed subjects (Tab. II). According to some authors (Benirschke, i960; Benirschke and Clifford 1959; Blanc 1961; and Kelsall et al, 1967), cord infiltration may be an indication of intrauterine fetal infection. As a consequence, the terms vasculitis, umbilical phlebitis and funiculitis have been introduced. Other authors (Barter, 1962, and Dominguez et al, 1960), mostly impressed by the discrepancy between the incidence of clinically evident neonatal infection and funiculitis, as well as by the significant association of cord infiltration with prolonged labor and clinical signs of fetal distress, have come to the conclusion that leukocytic infiltration of the umbilical cord must predominantly be the effect of fetal hypoxia. Dominguez et al (i960) go even further and postulate “that the examination of the umbilical cord for leukocytic infiltration affords a means for a long term study of children to determine the effect of intrauterine hypoxia on their ultimate growth and development”.

One way to check the validity of the latter hypothesis, is to investigate the umbilical cords of twins. Since our group has proven that, compared to the first-born twin, the second-born infant is as a rule significantly more hypoxic at birth (Derom and Thiery, 1969), one may expect that intragroup comparison of the incidence of leukocytic infiltration of twin cords should at least permit to disprove the validity of Dominguez assumptions.

I feti gemellari possono avere, nell'utero, situazioni diverse, regolate, come nella comune gravidanza, dalla legge dell'adattamento. Le situazioni principali sono la longitudinale (a estremo cefalico o podalico) e la trasversale. Sono possibili le seguenti combinazioni: (i) ambedue i feti in presentazione occipitale: 31-47%; (2) ambedue in presentazione podalica: 8-12%; (3) uno in presentazione occipitale e l'altro in presentazione podalica: 34-38%; (4) ambedue in situazione trasversale: 0.5-0.1%. Possono aversi anche altre combinazioni, quali: la combinazione « T », quando i feti sono uno sopra l'altro — quello inferiore in situazione longitudinale e quello superiore in situazione trasversale; la combinazione del « T » rovesciato; oppure la combinazione con ambedue i feti in situazione trasversale. Nei casi in cui i feti sono uno davanti all'altro, si nota la combinazione a croce, e cioè, un feto in situazione longitudinale e il feto posteriore in situazione trasversale. Tali combinazioni, in cui ha importanza se vi siano membrane comuni o separate, specialmente per quanto riguarda quelle amniotiche, possono provocare complicazioni all'atto del parto.

The maternal response in terms of changes in weight, plasma volume, total body water, intravascular protein mass and urinary oestriol excretion to a singleton pregnancy has been shown to be related to the size of the baby produced. Twin pregnancies have been studied to assess the maternal response to the double load, to see whether a much greater response is required compared to a singleton pregnancy. It is hoped that this study will eventually show not only what can be considered to be a physiological response to a twin pregnancy, but will show how the maternal organism sometimes fails to respond normally to the double load.

Observations were made at 30 to 32 weeks and again between 36 and 39 weeks of pregnancy in 14 primigravid twin pregnancies and 17 multigravid twin pregnancies, as well as in singleton primigravidae. For the purposes of comparison the singleton pregnancies have been divided up by the amount of weight gain during pregnancy. High weight gain was defined as 600 g/week or more during the 20th to 30th weeks. Normal weight gain was 360-540 g/week and low weight gain 320 g/week or less. The observations made were plasma volume using dye T. 1842 (Evan's Blue); total body water using deuterium oxide and measuring in an infrared spectrophotometer; the intravascular protein mass; the red cell mass; and the urinary oestriol excretion.

Für die Diagnose von Eineiigkeit bei Zwillingen ist die Feststellung von Gefäss-Anastomosen in den Placenten bedeutungsvoll. Aber nur 50% der Placenten der Monochoriaten weisen Anastomosen auf; das Fehlen von Anastomosen kann deshalb diagnostisch nicht verwertet werden.

Die einzelnen Anastomosenarten (arterielle, venöse, kapillare) finden sich verschieden häufig und können auf die Entwicklung der Zwillinge eine Wirkung haben.

Grosse, arterielle Anastomosen sind häufig, aber fast immer findet man nur eine einzige grosse Verbindungsarterie. Diese ist für die Entwicklung der Zwillinge günstig und wirkt ausgleichend.

Grosse, venöse Anastomosen sind selten und sind für den schwächeren Zwilling gefährlich, da ihm immer mehr Blut entzogen wird. Bei lebenden Zwillingen findet man sie immer nur gleichzeitig mit einer ausgleichend wirkenden arteriellen Anastomose.

Kapillare Anastomosen sind häufig und finden sich in den verschiedensten Kombinationen. Eine Auswirkung auf die Entwicklung der Zwillinge liess sich nicht feststellen.

Bei fehlenden Gefässverbindungen kann die Eineiigkeit-diagnose bei gleichgeschlechtlichen Zwillingen aus dem Placentabefund nicht gestellt werden; dies ist erst durch Ähnlichkeits-diagnose anhand der Erbfaktoren in späteren Jahren möglich.

Circa un anno fa, uno di noi teneva una relazione sulla genetica della sterilità al Congresso Argentino di Ostetricia. In quell'occasione fu richiesta una valutazione scientifica della gemellazione provocata dalle cure ormonali della sterilità. Risultò così che una rassegna dell'argomento non esiste; nacque allora l'idea di una inchiesta che abbiamo realizzato nei mesi scorsi e di cui riferiamo i primi risultati.

La nostra inchiesta riguarda l'induzione del concepimento gemellare in seguito a trattamento antisterile: (i) mediante gonadotropine; (2) mediante clomifene; (3) mediante altre terapie.

Inoltre, essendo state segnalate gemellazioni secondarie a sospensione di trattamento antiovulatorio, abbiamo chiesto informazioni sul concepimento gemellare dopo trattamento con estroprogestinici.

Nella Tab. I vengono indicati gli studiosi che hanno finora risposto all'inchiesta, insieme alle rispettive località e nazioni. Le informazioni riguardano soprattutto Europa, America e Medio Oriente.

Per diverse frazioni proteiche plasmatiche, fra le quali alcune proteine di trasporto come le aptoglobine e le transferrine, è stato osservato con metodi biochimici e/o immunologici un polimorfismo fenotipico ed è stato dimostrato un corrispondente polimorfismo genetico, risultando così definiti i gruppi plasmatici. Per le ceruloplasmine, che sono, com'è noto, le proteine plasmatiche con specifica funzione di trasporto del rame, non è stato ancora dimostrato un sicuro polimorfismo negli individui sani: la presente ricerca è appunto intesa a dimostrare l'esistenza di una variabilità individuale per questa proteina, come primo passo per poterne studiare la eventuale correlazione genetica.

É stata studiata la velocità di migrazione delle ceruloplasmine mediante elettroforesi a disco su gel di poliacrilamide: in realtà non è stato rilevato direttamente questo parametro, ma il rapporto fra lo spostamento delle ceruloplasmine e quello del Blu Bromofenolo (BBF) sottoposti contemporaneamente all'elettroforesi. Il BBF ha, com'è noto, una migrazione prealbuminica, mentre le ceruloplasmine migrano con le α-globuline: dopo l'elettroforesi e la colorazione specifica delle ceruloplasmine restano evidenti nel cilindro di gel i soli due dischi del BBF e della ceruloplasmina. Il rapporto di migrazione della proteina ripetto al BBF è stato ricavato direttamente da un tracciato elettroforetico ottenuto dalla lettura del cilindro di gel con un elettrofotometro adatto allo scopo.

Dopo la scoperta che i soggetti con leucemia mieloide cronica presentano una delezione del braccio lungo di uno dei cromosomi della coppia 21 (cromosoma Filadelfia o Ph1), è stata avanzata l'ipotesi (Moloney, 1961) che in questo cromosoma siano localizzati il gene, o i geni, che controllano la fosfatasi alcalina leucocitaria (FAL). Sulla base di questa ipotesi fu poi postulato che la FAL dovesse essere aumentata nella sindrome di Down, dove esiste una trisomia 21. Numerose ricerche hanno infatti dimostrato che, in questa sindrome, la FAL è spesso elevata (Trubowitz et al, 1962; King et al, 1962a, 1962b; Lennox et al, 1962; O'Sullivan e Pryles, 1963; Alter et al, 1963; Cardinali et al, 1966; Medzewski e Loesch, 1966).

Indagini condotte nel nostro Istituto su un gruppo di 40 bambini affetti da sindrome di Down ci hanno permesso di dimostrare che l'aumento medio della FAL nei pazienti rispetto ai controlli era molto vicino al rapporto previsto di 3: 2 (Cardinali et al, 1966). Le nostre indagini hanno anche indicato che la FAL può essere aumentata, non solo nei casi di sindrome di Down in cui l'alterazione cromosomica è del tipo trisomia, ma anche nei casi in cui l'alterazione è del tipo traslocazione. Esiste, tuttavia, un certo numero di pazienti in cui i valori della FAL sono sovrapponibili a quelli dei soggetti normali. È noto, d'altro canto, che questa attività enzimatica può variare notevolmente sotto l'influenza di vari fattori fisiologici e patologici, quali l'età, la gravidanza, certi ormoni, le infezioni e altri stati morbosi. Resta quindi da chiarire quale sia l'importanza dei fattori genetici nel controllo della FAL.

Riferiamo qui solo alcuni dati preliminari di uno studio a lunga scadenza, intrapreso allo scopo di stabilire se la curva glicolitica spontanea del sangue incubato sterilmente a 37° presenti: (1) un profilo normale standard; (2) modificazioni del profilo geneticamente determinate; (3) alterazioni del profilo che possano servire come screening per svelare eventuali compromissioni degli enzimi glicolitici.

In questa comunicazione trattiamo unicamente della metodologia e di alcune osservazioni ad essa inerenti.

Siamo partiti dalla nozione che un sangue citratato prelevato e incubato sterilmente a 370 va incontro, entro un periodo massimo di 24 ore, ad una glicolisi spontanea pressoché completa; che nel sangue sono contenuti gran parte dei principali enzimi glicolitici e alcuni elementi, quali, ad esempio, le piastrine, dotate di intensa attività glicolitica.

Abbiamo iniziato l'indagine adoperando sangue di soggetti normali sia mononati che gemelli MZ e DZ, prelevato a digiuno e immesso in una provetta sterile contenente citrato di sodio al 3.8% nelle proporzioni di 1: 5 rispetto al sangue prelevato. La provetta è stata messa in termostato a 370 e, a intervalli regolari di tempo, sono stati prelevati campioni su cui si è eseguita la determinazione del glucosio. Inizialmente, i dosaggi venivano effettuati ogni 15' per un periodo di 6 ore; successivamente, abbiamo ritenuto opportuno prelevare ogni 30' per le prime 3 ore e ogni 60' nelle successive, protraendo però più a lungo l'osservazione.

Il glutatione è un tripeptide solforato che trova la sua specifica funzione quale sistema ossido-riduttivo a servizio della respirazione tissulare anaerobia, grazie appunto alla sua doppia presentazione nelle fasi ridotta (GSH) e ossidata (GSSG), che costituiscono, completandosi, il « sistema glutationemico ». Esso è presente nel protoplasma delle cellule viventi dei tessuti e del sangue.

Il fenomeno dell'isoglutationemia gemellare fu rilevato da uno di noi (Gedda, 1943) in una coppia di gemelli MZ, in cui il dosaggio del glutatione ematico dimostrò un'esatta corrispondenza nei due individui; i dosaggi eseguiti su di una coppia qualunque di individui non gemelli non dettero mai cifre identiche.

Altre ricerche gemellari (Gedda e Guarino, 1943, 1945; Gedda e Benigni, 1947), effettuate oltre che sull'uomo anche sul coniglio, dimostrarono che vi erano alcuni casi di discordanza glutationemica (anisoglutationemia), che però nei casi di coppie MZ era soltanto transitoria, e cioè si trasformava, in successive analisi a distanza di tempo, in una isoglutationemia. In un ristretto numero di coppie DZ Pisoglutationemia non poté mai essere dimostrata.

In a previous study (Hosenfeld and Schröter, 1970) we were able to demonstrate the activity of three serum enzymes, Cholinesterase (EC 3.1.1.8), ceruloplasmin (EC 1.10.3.2.) and alkaline phosphatase (EC 3.1.3.1) as individual characteristics which in healthy adults remain fairly constant for several months. For the purpose of investigating whether these quantitative features are genetically determined, we carried out similar studies on twins.

The twins were traced in the Birth Register of Kiel. A certain amount of selection could have been caused by the willingness to have blood samples taken and by the interest in obtaining Accident Cards. We examined 30 MZ and 30 same-sexed DZ twin pairs, aged 8-20 years. The average age of the two groups of twins was about 15 years. Among the MZ 15 pairs were female; among the DZ, 11 pairs.

Zygosity was established by polysymptomatic comparison of similarity. For documentation, color slides were made. ABO and Rh blood groups of all twins were determined, and in most cases further erythrocytic and serum properties. Blood samples were usually obtained in the afternoon about three to four hours after the last meal. The serum was centrifuged off and frozen. Enzyme determination was carried out during the next one to three weeks. Following substrates were used for estimation of the three enzymes: benzoylcholine, paraphenylendiamine and paranitrophenylphosphate. Acording to the methods of Richterich (1968), all measurements were carried out kinetically, employing a recording spectrophotometer (Beckman DB).

Stabenau and Pollin (1968) observed, in a study of 23 pairs of adult MZ twins, that a significant correlation existed between birth-weight (a measure of fetal maturity) and adult protein-bound iodine (PBI) (r = + 0.58, P < 0.001). In a sub-sample of 12 individuals, the correlation between birthweight and thyroxine-binding globulin (TBG) was + 0.94 (P < 0.001). These data suggested that differences in protein hormonal binding may be established as a result of intrauterine factors, the effect of which may persist into adulthood. The study of neonates has demonstrated that measures of PBI and butanol-extractable iodine (BEI) are higher at birth than in later adulthood, and that rapid changes in individual values occur during the early neonatal period. Marks and Man (1965), Perry et al (1965) and Man (1966) have demonstrated that BEI and PBI values are higher for infants who are heavier in weight at birth and lower for infants of low birthweight. The weight of a fetus at birth has been used as one single estimate of fetal maturity when measures such as fetal length and gestational age are unavailable.

A sample of children and adolescent MZ and DZ twins and singletons was studied to test the hypothesis that measures of thyroid hormone activity are related to maturity at birth. In addition, estimates of the relative genetic control for thyroid hormone levels were made.

Twenty-one pairs of healthy MZ twins, all males 18-21 years of age, have been studied for possible correlations between individual personality characteristics and relatively constant patterns of pituitary-adrenocortical functioning. Subjects were studied by means of psychiatric interviews, Rorschach and other psychological tests, and 35 24-hour urine collections for 17-hydroxycorticosteroid (17OHCS) and 17-ketosteroid (17KS) determinations.

Our previous observations have been reported, both on these healthy male volunteers (Fox et al, 1961 and 1965) and on various clinical syndromes (Rizzo et al, 1954; Fox et al, 1958; Gifford and Gunderson, 1970). Our cumulative clinical impressions suggested that high levels of 17OHCS excretion are found in individuals with an intense need for close personal relationships, or with an equally intense need to defend themselves against emotional involvement, by active forms of avoidance and denial. Low 17OHCS levels were found in individuals who were more effectively defended against emotional intimacy by well-organized neurotic defenses, with some isolation, constriction of affect or less conscious awareness of conflict.

In studying twins, however, an unusual pattern of high 17KS and low 17OHCS compelled us to pay more attention to the significance of 17KS levels. High 17KS excretion patterns were found in energetic, ambitious individuals, with strong aggressive drives or equally strong defenses against them. Individuals with low 17KS levels were overcon trolled, with limited drive-endowment and widespread inhibitions of impulse and action.

The present paper is aimed to celebrate Franz J. Kallmann's memory, by illustrating the twin-family method, which he developed. His phenomenal powers of organization are testified to by his extensive samples. Their representative nature has the built-in check of the ratio of MZ: DZ twins. He brought a refinement of his own to the existing methods of calculating expectancy from nett figures, that permits comparison of studies of varying age distributions relative to manifestation period. His investigative design allows not only for the standard global concordance comparisons between DZ and MZ twins as well as for those reared together and apart, but exploits the implications of the hitherto neglected comparison of the categories of sibs and DZ twins also. Further, his combined twin-family method comprehends in its extended range the powerful analytical tool of comparative figures for the series step-sibs, half-sibs, sibs, DZ twins and MZ twins (Kallmann and Sander, 1947; Kallmann, 1953). Kallmann (1954a, 1954b) has moreover given minute attention to controversies concerning legitimate inferences from twin studies, and has drawn attention to a point made by Darlington (1953), which is often overlooked by the best of geneticists. Discordance between MZ twins is not a measure merely of postnatal or even of prenatal environmental effects: it may also have a genetic component through the action of genes sensitive to cytoplasmic asymmetry.

The biometrical genetical method of analysis of continuously varying human traits is described and shown to unify a number of common approaches to the analysis of twin data. The method, which is based on the analysis of variance, goes beyond alternative approaches by providing tests of basic assumptions and by investigating the possibility of assortative mating and dominant gene action.

Twins form an indispensable group for the application of the biometrical genetical method but the value of including other groups, often more easily collected, is demonstrated.

The techniques are illustrated through the reanalysis of data on the behavioural measures, neuroticism and intelligence quotient (I.Q.). For both measures fundamental assumptions are met. The results suggest the absence of dominant gene action for neuroticism, indicating an evolutionary history of stabilising selection, and directional dominance for high I.Q., indicating an evolutionary history of strong directional selection. Both these findings seem reasonable in view of psychological knowledge concerning the two measures.
