The P3 components (P3a and P3b) of the event-related potentials have been analysed in sixteen in-patients with a major depressive episode (DSM III-R) according to their subtype - anxious-agitated and impulsive versus retarded and blunted-affect - assuming that these two opposite dimensions of the depressive mood would rely on different physiopathological mechanisms. The P3a component, which indexes automatic cognitive processes, was dramatically decreased in the retarded and blunted-affect group. This suggests that this special type of cognitive dysfunction in depression is rather related to negative symptoms than to depression itself.

De onderzoeksresultaten warden per methode samengevat. Onderzocht wordt of (a) bij depressieve patiënten een vertraging vastgesteld werd, (b) waaruil deze bestond, (c) welke de aard van de vertraging was, (d) of er correlaties gevonden werden met andere meetmethodes, en (e) welke effecten van behandeling er werden gemeten.

Diagenesis in the Cretaceous and Tertiary sediments of the Alava Block (Basque- Cantabrian basin) has been studied using the clay mineralogy (X-ray diffraction) of cuttings from three representative wells of a N—S cross-section. More than 5500 m of various lithologies (marls, mudstones and sandstones) have been drilled in the northern part of the domain, and 2100 m in the southern zone. The illitization of smectite and the disappearance of kaolinite, due to diagenesis, are the most characteristic features in the northern well. Evolution of smectite to illite has been differentiated into four zones, from top to bottom of the series, each showing specific I-S interstratified clay assemblages. The disappearance of smectite and the distribution of kaolinite in the other two wells are explained based on source-area considerations. Burial and thermal history have been reconstructed, revealing a northward increase in thermal flow until the Oligocene (Alpine orogeny paroxysm). In the northern well, the thermal model suggests temperatures of 160 and 270°C for the disappearance of smectite (R0) and illite-smectite (I-S) mixed-layer R1 clay minerals, respectively. The disappearance of kaolinite is related to a temperature of 230°C, a temperature never attained in the other two wells. Retardation of these processes, in relation to temperature values in the literature, is a consequence of the poor reactivity of marly lithologies, due to the low availability of cations. In this regard, the scarcity of reactants (K-bearing phases) and the absence of pathways (low permeability) for their access and circulation imply that illitization could have taken place in a closed system, by diffusion, on a very small scale, i.e. that of the original smectite grains.

Several studies have demonstrated that psychomotor retardation is an important sign of the major depressive episode, both from a diagnostic point of view and as a predictor of treatment outcome. However, it is uncertain to what extent psychomotor retardation is specific for the major depressive episode. A heterogeneous group of psychiatric patients (n=26) was studied using a rating scale developed specifically to assess psychomotor retardation (RRS). The 13 patients suffering from a major depressive episode had a significantly higher RRS score than patients with another diagnosis. Furthermore, in the depressive subjects the severity of the depression (measured by means of Hamilton's depression rating scale) appeared to correlate with the severity of the psychomotor retardation. Cognitive features of psychomotor retardation were mainly responsible for the more severe psychomotor retardation in the depressive patients. Surprisingly. Hamilton's depression scale which was used in this study does not pay much attention to these aspects.

This review (part I and II) contains an overview of the literature of the past fifteen years over psychomotor retardation in depressed patients, as measured by the following methods:

1. observation scales (part I);

2. observation, coding and analysis of specific nonverbal behaviour (part I);

3. speech research (part I);

4. (choice) reaction time tasks (part II);

5. analysis of gross motor activity (part II);

6. of fine motor behaviour (part II).

In each section the results of the different studies are summarized and discussed, in order to answer the following questions: (a) did the depressed patients show any retardation?, (b) how did this retardation manifest itself?, (c) what was the nature of the retardation?, (d) were there any correlations with the results of other methods? and (e) what were the effects of antidepressive treatment?

A l’examen des items de l’échelle de ralentissement dépressif (ERD) de Widlocher et de ceux de l’échelle d’évaluation de la symptomatologie négative d’Andreasen (SANS), il apparaît une communauté symptomatologique entre syndrome déficitaire et ralentissement dépressif, ce qui a amené les auteurs á faire l’hypothése suivante: la note globale de la SANS peut être une mesure du ralentissement dépressif. Effectuée sur une population de 33 patients diagnostiqués épisode dépressif majeur, DSM III, cette étude a permis de montrer que la note globale de la SANS était corrélée a l’intensité du syndrome dépressif et que cette corrélation se faisait préférentiellement avec la composante ralentissement du syndrome dépressif (mesuré par l’ERD et par les items de l’échelle de mélancolie de Bech et Rafaelsen). Par ailleurs, la note globale de l’ERD était plus fortement corrélée a la SANS qu’aux autres échelles de dépression utilisées dans cette étude (échelle de dépression d’Hamilton á 26 items, échelle de dépression de Montgomery-Asberg). La note globale de la SANS pourrait donc être considerée comme une mesure du ralentissement dépressif.

Les travaux sur la dépression de l’adolescent restent encore peu nombreux, malgré la fréquence et la gravité de ce trouble à cette période d’âge. Aux études classiques qui évaluaient, chez l’adulte, la dépression dans son ensemble se sont récemment substitués des travaux qui analysent la symptomatologie dépressive sur un mode dimensionnel.

Nous avons tenté dans ce travail de comparer le ralentissement dépressif et les différentes dimensions de l’humeur dépressive, entre une population d’adultes et une population d’adolescents déprimes.

Nous avons pour cela utilisé l’échelle de ralentissement dépressif et une échelle expérimentale de mesure de l’humeur dépressive (Jouvent et coll.) chez 24 adultes et 15 adolescents présentant les critères DSM III d’épisode dépressif majeur.

Les résultats montrent que les scores évalués globalement par ces deux échelles sont identiques dans les deux populations. Mais l’analyse dimensionnelle met en évidence des différences nettes (figures 1, 2 et 3) entre adultes et adolescents, avec une dimension de déficit émotionnel plus marquée chez les adultes, et une dimension de tristesse plus importante chez les adolescents.

Ces résultats nous amènent à penser que c’est peut-être par l’expression quantitativement différente de dimensions cliniques identiques que 1’adolescent trouve sa spécificité par rapport à l’adulte. D’autres études doivent être menées pour étayer cette hypothèse, et pour tenter de valider chez les adolescents des instruments d’évaluation clinique plus précis.
