The concept of specific receptors that bind drugs or transmitter substances onto the cell, thereby either initiating biological effects or inhibiting cellular functions, is today a cornerstone of pharmacological research and pharmaceutical development. Yet, while the basic ideas of this concept were first explicitly formulated in 1905 by the Cambridge physiologist John Newport Langley (1852–1925), drug receptors remained hypothetical entities at least until the end of the 1960s. Without doubt, the development of receptor-subtype specific pharmaceuticals—especially the beta-adrenergic receptor antagonist propranolol (introduced in 1965)—promoted the acceptance of the receptor concept in pharmacology. It was only in the 1970s, however, that receptors began to be isolated as specific proteins of the cell membrane and that their composition and conformation began to be explored. During the last twenty years the modern techniques of molecular biology have helped to determine the genetic basis of receptor proteins, to identify their amino acid sequences, and to further elucidate their remarkable structural diversity as well as their similarities and evolutionary relationships. Numerous receptor types and subtypes have since been characterized.

Robert Koch's identification of the tuberculosis pathogen in 1882 is held to be his greatest scientific achievement. In the eyes of his friend and colleague, Friedrich Löffler, the discovery was a “world-shaking event” which resulted in both instant and everlasting fame, turning Koch “overnight into the most successful and outstanding researcher of all times”. Paul Ehrlich, remembering Koch's presentation in Emil du Bois Reymond's Institute for Physiology in Berlin on 24 March 1882, called it “my single greatest scientific experience”. The sensational character of Koch's achievement, which is noted in Löffler's and Ehrlich's retrospective statements, seems to have been obvious to contemporaries of the event. Albert Johne, writing a history of tuberculosis in 1883, found that history had, in a way, come to an end: “resulting from the latest of Koch's publications, the pathogenic aspects of the tubercle question are settled at large”. Koch himself profited from the overwhelming reception by being promoted to the rank of a senior executive officer, Geheimer Regierungsrath, in June 1882. March 24, 1882 thus came to stand for two things: Koch's breakthrough to world fame and a sort of doomsday for tuberculosis. Not surprisingly, the event was held in similar esteem by later biographers: Bernhard Möllers in 1950 called it the “greatest and most important success of his life”, and Thomas Brock, Koch's most recent biographer, assessed the discovery of the tubercle bacillus as the first of two steps on Koch's road to fame in the early 1880s. In conjunction with the 1883–84 cholera expedition, which made Koch a hero for the public, the tubercle bacillus indicated his breakthrough in the scientific world.

Thecolica Pictonum or colic of Poitou, under these and many other names, was a frequent, widespread, and deadly disease from Roman times until the eighteenth century. Its unique pathognomonic, notably a severe colic succeeded by paralysis and other central nervous system dysfunction, makes it possible to identify the disease with certainty as chronic lead disease, usually caused by the ingestion of lead-adulterated wines. The custom of sweetening and preserving sour wines with lead-containing additives is traced to the Romans. They had made the empirical discovery that sapa, a syrup prepared by concentrating must in a lead vessel, kept wine from spoiling and that it had, moreover, an agreeable flavour.

Reports of outbreaks of the colica Pictonum appear in the medical literature from Roman times, but the correct aetiology of the disease was not discovered until the seventeenth century following a series of outbreaks in Southern Germany which were related to unfavourable climatic and political conditions. The connexion between the disease and prevailing methods for “correcting” wines was drawn in 1696 by Eberhard Gockel, then the city physician of Ulm. This achievement can be traced to his familiarity with Samuel Stockhausen's work on plumbism among miners and potters, as well as to the favourable epidemiological situation presented by Gockel's monastic patients.

From the literary evidence assembled here and from experimental determinations of the lead content of sapa and similar concentrates, it is possible to estimate the lead levels and toxicity of wines from various eras. The levels range up to 80 mg/l and make it apparent that many wines were sufficiently toxic to account for the incidence and severity of the colica Pictonum. Explanations for the disastrous persistence of the colic of Poitou are discussed, as are the similarities between Gockel's approach and the methods of modern environmental medicine.

The concept of drug receptors has played a significant role in the biomedical sciences and in pharmaceutical innovation in the second half of the twentieth century. Although the concept dates back to the work of the German bacteriologist and immunologist Paul Ehrlich and of the British physiologist John Newport Langley at the end of the nineteenth and the beginning of the twentieth century, its acceptance was delayed because of conflicting ideas about drug action, and because of uncertainties and hesitations about the concept itself.

In the course of the twentieth century road traffic injuries (RTIs) became a major public health burden. RTI deaths first increased in high-income countries and declined after the 1970s, and they soared in low- and middle-income countries from the 1980s onwards. As motorisation took off in North America and then spread to Europe and to the rest of the world discussions on RTIs have reflected and influenced international interpretations of the costs and benefits of ‘development’, as conventionally understood. Using discourse analysis, this paper explores how RTIs have been constructed in ways that have served regional and global development agendas and how ‘development’ has been (re-)negotiated through the discourse of RTIs and vice versa. For this purpose, this paper analyses a selection of key publications of organisations in charge of international health or transport and places them in the context of (a) the surrounding scientific discussion of the period and (b) of relevant data regarding RTI mortality, development funding, and road and other transport infrastructure. Findings suggest that constructions of RTIs have shifted from being a necessary price to be paid for development to being a sign of development at an early stage or of an insufficiently coordinated development. In recent years, RTI discussions have raised questions about development being misdirected and in need of fundamental rethinking. At present, discussions are believed to be at a crossroads between different evaluations of developmental conceptualisations for the future.
