The aim of this work is to report on the tumour control probability (TCP) of a UK cohort of lung stereotactic ablative radiotherapy patients (n = 198) for a range of dose and fractionations common in the UK.

TCP values for 3 (54 Gy), 5 (55 and 60 Gy) and 8 (50 Gy) fraction (#) schemes were calculated with the linear-quadratic Marsden TCP model using the Biosuite software.

TCP values of 100% were computed for the 3 # and for 5 # (α/β = 10 Gy) cohorts; reduced to 99% (range 97–100) for the 5 # cohort only when an α/β of 20 Gy was used. The average TCP value for the 50 Gy in 8 # regime was 97% (range 92–99, α/β = 10 Gy) and 64% (range 48–79, α/β = 20 Gy). Statistical significant differences were observed between the α/β of 10 Gy versus 20 Gy groups and between all data grouped by fraction.

TCPs achievable with current planning techniques in the UK have been presented. The ultra-conservative 50 Gy in 8 # scheme returns a significantly lower TCP than the other regimes.

The intent of the review was to identify different methodological approaches used to calculate the planning target volume (PTV) margin for head and neck patients treated with volumetric arc therapy (VMAT), and whether the necessary factors to calculate the margin size with the selected formula were used.

A comprehensive, systematic search of related studies was done using the Hydi search engine and different databases: MEDLINE, PubMed, CINAHL, ProQuest (Nursing and Allied Health), Scopus, ScienceDirect and tipsRO. The literature search included studies published between January 2007 and December 2020. Eligibility screening was performed by two reviewers.

A total of seven studies were found. All the reviewed studies used the Van Herk formula to measure the PTV margin. None of the studies incorporated the systematic errors of target volume delineation in the PTV equation. Inter-fraction translational errors were assessed in all the studies, whilst intra-fraction errors were only included in the margin equation for two studies. The studies showed great heterogeneity in the key characteristics, aims and methods.

Since systemic errors from target volume delineation were not considered and not all studies assess intra-fraction errors, PTV margins may be underestimated. The recommendations are that studies need to determine the effect of target volume variance on PTV margins. It is also recommended to compare PTV margin results using various formulas.

When patients receive radiation therapy for breast cancer, they need to take off their underwear to avoid build-up effects. However, it is a mental burden for female patients to take off their underwear at every fraction of radiation therapy. The purpose of this study was to investigate whether commercially available thin underwear can be worn during radiation therapy for breast cancer.

In this phantom study, we investigated whether commercially available underwear can be worn during radiation therapy for breast cancer using six thin non-disposable brassieres and one disposable paper brassiere. The dose increase rate (ΔD) was calculated by measuring skin doses with or without each brassiere.

The mean ΔD values of six non-disposable brassieres were 13.5% (9.0–21.8%), whereas that of disposable ones was 2.0%.

Due to the risk of excessive radiation to the skin, wearing commercially available underwear is not recommended during radiation therapy for breast cancer, but a thin disposable paper brassiere may be safe to be used.

Radiation therapy has historically used margins for target volume to ensure dosimetric planning criteria. The size of margin for a given treatment site is still uncertain particularly for moving targets along with set-up variations leading to a fuzziness of target volume. In this study, we have estimated the dosimetric benefit of normal structures using biological-based optimal margins. The treatment margins are derived by knowledge-based fuzzy logic technique which is considering the radiotherapy uncertainties in treatment planning.

All treatment plans were performed using stepped increments of asymmetric margins to estimate prostate radiobiological indices such as tumour control probability (TCP) and normal tissue complication probability (NTCP). An absolute NTCP of 5% was considered to be the maximum acceptable value while TCP of 85% was considered to be the minimal acceptable limit for each volumetric modulated arc therapy (VMAT) plan of localised prostate cancer radiotherapy. Results were used to formulate rules and membership functions for Mamdani-type fuzzy inference system (FIS). In implementing the rules for the fuzzy system for ΔNTCP values above 10%, the PTV margin was not permitted to exceed 5 mm to avoid rectal complications due to margin selection. The new margins were applied in VMAT planning of prostate cancer for standard displacement errors. The dosimetric results of normal tissue predictors were estimated such as organ mean doses, rectum V60 (volume receiving 60 Gy), bladder V65 (volume receiving 65 Gy) and other clinically significant dose–volume indicators and compared with VMAT plans using current margin formulations.

Dosimetric results compared well to the results obtained by current techniques. Good agreement was obtained between proposed fuzzy model margins and currently used margins in lower error magnitude, but significant results were observed at higher error magnitude when organ toxicity concerned without compromising the target volumes.

The new margins may be helpful to estimate possible outcomes of normal tissue complications and thus may improve complication free survival particularly when organ motion errors are inevitable, case by case.

This study dosimetrically compared volumetric-modulated arc therapy (VMAT) to intensity-modulated arc therapy (IMRT) for patients with liver carcinoma.

Ten patients with liver carcinoma previously treated with IMRT or VMAT were retrospectively selected for this study. Each patient received a total dose of 54 Gy in 1·8 Gy fractions. Dosimetric evaluations for each patient were performed using the dose–volume histograms (DVHs) for planning target volumes (PTVs) and organs at risk (OAR). All dosimetric parameters were statistically analysed using mean values, standard deviations and p-values for determining the significance. The conformality index (CI) and homogeneity index (HI) were calculated and compared. For efficiency evaluation, monitor units (MUs) and beam on times (BOT) were recorded.

Compared to IMRT, VMAT plans showed significant differences in the heterogeneity with p < 0·01 and insignificant differences in both conformality and normal tissue sparing. VMAT required marginally fewer mean MU and shorter BOT when compared to IMRT with insignificant differences.

For radiation therapy treatment of liver carcinoma, IMRT and VMAT can achieve similar PTV coverage and normal tissue sparing. Treatment time is only marginally shorter with VMAT versus IMRT with insignificant differences.

Coronavirus disease (COVID-19) has significantly challenged the access to cancer care and follow-up for a patient with cancer.

Based on published literature and our experiences, it is reasonable to presume that clinical examination and follow-up visits have been significantly curtailed worldwide in order to adhere to the new norms during the pandemic. Although telephonic and telemedicine consultations may help bridge a few gaps, completely dispensing with in-person consultation has its challenges, especially in low middle-income countries. Telephonic consultations could facilitate triaging of ambulatory cancer patients and allocation of face-to-face consultations for high priority patients.

We propose a telephonic consultation-based triaging approach for ambulatory cancer patients in order to identify those needing in-hospital consultations.

Total body irradiation (TBI) is an external radiotherapy technique in which the whole body including the superficial regions is required to receive the therapeutic dose. The purpose of this study is to evaluate the received surface dose during TBI technique.

The anterior/posterior (AP/PA) TBI was implemented with 18-MV photon beam at 312-cm treatment distance for human-like phantom. The GAFCHROMIC-EBT3 films were used for superficial dose measurements.

The percentage of surface-absorbed dose relative to the prescription point for 8 points of measurements was between 102·78–121·48% and 104·51–127·43% at 5 and 10 mm depth, respectively. In the chest wall region due to the presence of lung blocks, the absorbed dose was below the acceptable level, so an electron boost was required to increase the chest wall absorbed dose.

According to the results, the implemented technique was able to deliver sufficient dose to the shallow surface of phantom’s body.

The aim of this study was to evaluate the use of Eclipse’s beam angle optimiser (BAO) for three-dimensional conformal radiotherapy planning.

Eleven 3D conformal lung plans, with varied tumour volumes, were retrospectively studied. For each clinical plan, a BAO plan was produced and then optimised by an experienced planner. Plan quality was assessed using International Commission on Radiation Units and Measurements (ICRU)-83 and  Radiation Therapy Oncology Group (RTOG) recommended dose reporting metrics for dose volume prescribing and reporting.

Differences in dose volume histograms for both methods showed no clinical significance. Planning target volume Dmax for both plans was comparable and within ICRU guidelines. Reported spinal cord Dmax and the doses to 33% and 67% volume of the heart were within the RTOG recommended limits. Mean lung V20 values for BAO and non-BAO plans were 20 and 16%, respectively. The average monitor units for the BAO plans were about 11% lower. The conformity and homogeneity indices were within the acceptable range for both cases. On average, it took 23 minutes to plan using the BAO compared to 68 minutes for the non-BAO plans.

Eclipse BAO shows the potential to produce good quality conformal plans and reduce planning time. This process could be further refined with multi-leaf collimator and optimal collimator angle options.

A middle-aged gentleman presenting with dyspnoea was presumptively assumed to be a novel coronavirus suspect.

Nasopharyngeal and oropharyngeal swabs were reported negative, and clinico-radiological workup revealed a case of adenoid cystic carcinoma (ACC) of the lung with metastases to the contralateral lung and the lumbar vertebrae.

ACC is a rare malignancy of exocrine glands. Most commonly found in the minor salivary glands, they may rarely occur in other sites. Primary ACC of the lung is a rare histologic subtype that is encountered infrequently in clinical practice for which standard guidelines do not exist.

Bullous pemphigoid (BP) is the most common autoimmune bullous disease, which is usually seen in older ages. Rarely, radiotherapy (RT) has been reported as a localised trigger factor for BP.

We present a case of a 64-year-old woman who underwent radical mastectomy and radiation therapy for breast cancer. Two months after the completion of RT with adjuvant Trastuzumab, a BP eruption developed in the irradiated area, spreading thereafter to other skin areas. The patient was treated with oral corticosteroids and the lesions evolved favourably.

This case demonstrates a relatively benign skin reaction that could develop during the course of cancer treatment, which required co-management by oncologists, pathologists and dermatologists.

To investigate the use of co-registration of the computerised tomography (CT) planning scan with transperineal ultrasound (TPUS) as an aid to the delineation of the clinical target volume (CTV), and the use of TPUS as a tool for inter- and intra-fractional monitoring in men with bilateral hip prostheses (b-P) undergoing prostate radiotherapy.

We marked the CTV of three patients with and without the co-registered TPUS images. A metal artefact reduction algorithm was utilised. Two patients were treated with intensity-modulated radiotherapy (IMRT) and one with volumetric-modulated arc therapy (VMAT). The inter- and intra-fractional monitoring details were reviewed retrospectively.

Clinician marking with TPUS/CT fusion improved the confidence of prostate CTV delineation leading to a consistent change in volumes across two observers. Inter- and intra-fractional monitoring was possible using TPUS as image guidance, as it is for those patients with non-prosthetic hips.

Using TPUS in the radiotherapy workflow has enabled us to more confidently plan, treat and monitor patients with b-HP. Due to transperineal image acquisition, the ultrasound images are not affected by the presence of hip prostheses, which are outside the field of view.
