Due to extreme shortages of personal protective equipment caused by the COVID-19 pandemic, many healthcare workers will be forced to recycle protective masks intended for disposal after a single use. We propose investigating the use of ultraviolet germicidal irradiation to sterilize masks of SARS-CoV-2 for safer reuse.

Decontamination of N95 respirators is being used by clinicians in the face of a global shortage of these devices. Some treatments for decontamination, such as some vaporized hydrogen peroxide methods or ultraviolet methods, had no impact on respiratory performance, while other treatments resulted in substantial damage to masks.

A total of 38 long-term care facilities within a region participated in a 3-month quality improvement initiative focused on environmental cleaning and disinfection. Significant improvements in daily and discharge cleaning were observed during the project period. Further study of the sustainability and clinical impact of this type of initiative is warranted.

Pediatric long-term care facilities were surveyed to assess infection control and antimicrobial stewardship practices. Policies mandated by the Centers of Medicare and Medicaid Services (CMS) were included. Only 40% of sites reported implementing >90% of surveyed CMS policies. The survey also identified several gaps in non–CMS-mandated policies.

A retrospective time series analysis was conducted to compare inpatient fluoroquinolone use when susceptibilities were masked and after susceptibilities were unmasked. Although inappropriate culture-directed prescriptions increased, overall fluoroquinolone usage decreased. Culture-directed therapy was a small part of fluoroquinolone usage; hence, efforts should target empiric use to reduce overall consumption.

Environmental surface testing was performed to search for evidence of severe acute respiratory coronavirus virus 2 (SARS-CoV-2) environmental contamination by an asymptomatic SARS-CoV-2 carrier with persistently high viral loads under isolation. No evidence of environmental contamination was found. Further studies are needed to measure environmental contamination by SARS-CoV-2 carriers and to determine reasonable isolation periods.

Nonventilator hospital-acquired pneumonia (NV-HAP) is one of the most common healthcare-associated infections, but most hospitals do not track it. We created a pilot electronic definition for NV-HAP and compared its accuracy to Centers for Disease Control and Prevention (CDC) criteria. Kappa values for the electronic definition and CDC criteria versus “true” pneumonia were similar: 0.40 and 0.47, respectively.

We investigated potential transmissions of a symptomatic SARS-CoV-2–positive physician in a tertiary-care hospital who worked for 15 cumulative hours without wearing a face mask. No in-hospital transmissions occurred, despite 254 contacts among patients and healthcare workers. In conclusion, exposed hospital staff continued work, accompanied by close clinical and virologic monitoring.

Background:Clostridioides difficile infections (CDIs) present and are transmitted in both community and healthcare settings. Patients who become colonized or infected during hospitalization may be discharged into the community. Asymptomatic spread and/or community-based transmission have also been posited as alternative sources for healthcare-onset CDI cases. The objective of our study was to determine whether individuals are at greater risk for developing a CDI if they have a family member that spent time hospitalized in the prior 90 days, even if the hospitalized family member had no prior diagnosis of CDI. Methods: We conducted a retrospective cohort study using the Truven Marketscan database from 2001 through 2017; both commercial claims and Medicare supplemental data were included. We categorized enrollees by age, sex, month, year, exposure to a family member with CDI, hospitalization, or high- or low-risk antibiotic use in the prior 90 days. We then subdivided these groups based on the total amount of time that other family members spent hospitalized in the prior 90 days: ≤4 days, 5–10, 11–20, 21–30, 41–50 or >50 days. Within each subgroup, we computed the incidence of CDI. We then used a stratified regression model (log-linear quasi-Poisson) to estimate the incidence of CDI in each enrollment bin. Finally, we repeated our analysis using all CDI cases, CDI cases with no prior CDI in the family, and cases without prior hospitalization. Results: Over the 17-year study period, >5.1 billion enrollment months were represented in our dataset. We identified 224,818 cases of CDI, 223,744 cases without prior CDI in a family member and 164,650 CDI cases where the case patient had no prior hospitalization. Table 1 depicts the estimated risk (incident rate ratios) associated with the amount of time that other family members spent hospitalized in the prior 90 days. There is a very clear dose–response curve, and the relative risk for CDI increase as the amount of time other family members spent hospitalized increased. Other risk factors included prior hospitalization, low- and high-risk antibiotics, age, female sex and exposure to a family member with CDI. Conclusions: Having a family member who has been hospitalized in the prior 90 days significantly increases the risk for CDI, even if the family member did not have CDI. The total amount of time other family members spent in the hospital is positively associated with the level of risk.

Ultraviolet C (UV-C) light reduces contamination on high-touch clinical surfaces. We assessed the efficacy of 2 UV-C devices at eradicating important clinical pathogens in hyperbaric chambers. Both devices were similarly efficacious against MRSA but differed significantly against C. difficile. Additionally, direct UV-C exposure was more efficacious against both species than indirect exposure.

The early phase of the coronavirus disease 2019 (COVID-19) pandemic and ongoing efforts for mitigation underscore the importance of universal travel and symptom screening. We analyzed adherence to documentation of travel and symptom screening through a travel navigator tool with clinical decision support to identify patients at risk for Middle East Respiratory Syndrome.

We retrospectively evaluated antimicrobial stewardship program (ASP) interventions over a 63-month period. We compared acceptance rates for those interventions communicated telephonically versus those communicated with a temporary note left in the electronic medical record. Telephonic communication produced superior acceptance rates overall and when analyzed by intervention type and provider.

To determine barriers and facilitators to standardization of ultrasound probe disinfection at ambulatory sites, we conducted observations and interviews of staff. Variability was noted in disinfection practices and in the use of protective equipment even for procedures with the potential for the probe to contact sterile tissues. Standardization is needed.

Previously, we showed that disinfection of sink drains is effective at decreasing bacterial loads. Here, we report our evaluation of the ideal frequency of sink-drain disinfection and our comparison of 2 different hydrogen peroxide disinfectants.

We compared methicillin-resistant Staphylococcus aureus (MRSA) bloodstream infections (BSIs) captured by culture-based surveillance and MRSA septicemia hospitalizations captured by administrative coding using statewide hospital discharge data in Connecticut from 2010 to 2018. Observed discrepancies between identification methods suggest administrative coding is inappropriate for assessing trends in MRSA BSIs.

We surveyed Canadian Nosocomial Infection Surveillance Program hospitals to evaluate infection prevention and microbiology laboratory preparedness for Candida auris. We identified significant gaps: most hospitals were not prepared to screen patients for colonization, and only one-half of laboratories reported identifying all clinically significant Candida isolates to the species level.

We describe a widespread laboratory surveillance program for severe acute respiratory coronavirus virus 2 (SARS-CoV-2) at an integrated medical campus that includes a tertiary-care center, a skilled nursing facility, a rehabilitation treatment center, and temporary shelter units. We identified 22 asymptomatic cases of SARS-CoV-2 and implemented infection control measures to prevent SARS-CoV-2 transmission in congregate settings.

In a crossover study, 30 stethoscopes were assessed and disinfected using 3 protocols: isopropyl alcohol, a quaternary ammonia or biguanide disinfectant, and ultraviolet germicidal irradiation (UVGI). All protocols effectively reduced bacterial loads, but UVGI was less effective at higher contamination levels (P = .0004). The effectiveness of each intervention was short in duration.

We assessed Clostridioides difficile toxin testing and positivity for all patients in Manitoba hospitals during June 2016–November 2018. The testing rate was 30 per 10,000 patient bed days (95% confidence interval [CI], 30–31) and the incidence rate was 3.5 per 10,000 patient bed days (95% CI, 3.3–3.7). The context of testing is essential to the interpretation of data among jurisdictions.
