Many older adults experience what is clinically recognised as frailty but little is known about the perceptions of, and attitudes regarding, being frail. This qualitative study explored adults' perceptions of frailty and their beliefs concerning its progression and consequences. Twenty-nine participants aged 66–98 with varying degrees of frailty, residing either in their homes or institutional settings, participated in semi-structured interviews. Verbatim transcripts were analysed using a Grounded Theory approach. Self-identifying as ‘frail’ was perceived by participants to be strongly related to their own levels of health and engagement in social and physical activity. Being labelled by others as ‘old and frail’ contributed to the development of a frailty identity by encouraging attitudinal and behavioural confirmation of it, including a loss of interest in participating in social and physical activities, poor physical health and increased stigmatisation. Using both individual and social context, different strategies were used to resist self-identification. The study provides insights into older adults' perceptions and attitudes regarding frailty, including the development of a frailty identity and its relationship with activity levels and health. The implications of these findings for future research and practice are discussed.

This paper describes how space and place have been understood in gerontology as phenomenon that are both physical and social in character, yet are relatively bounded and static. The argument is posed as to how, following recent developments in human geography, a relational approach might be adopted. Involving a twist in current thinking, this would instead understand space and place each as highly permeable, fluid and networked at multiple scales. Moreover, it is proposed that the concept of ‘affect’ might also be insightful, recognising space and place as being relationally configured and performed, possessing a somatically registered energy, intensity and momentum that precedes deep cognition. Three vignettes illustrate the relationalities and affects in the lives and circumstances of older people, and how focusing more explicitly on them would allow for a richer understanding of where and how they live their lives. The paper closes with some thoughts on future theoretical, methodological and disciplinary considerations.

Data from six European regions participating in the Eleven Country Study on Health Care of the Elderly suggested that feelings of loneliness were more prevalent in areas where living alone was rarest and where community bonds were strongest. Individual variables describing life-situation did not explain the differences. The article examines loneliness as an historical and cultural phenomenon. It is argued that loneliness reflects, through complex mediations, the mutual relationship between the individual and the community and the extent to which the ideology of individualism prevails in society. In attempts to understand the differences between the study areas, the article looks more closely into the role of the community and the family in two selected areas: the industrial town Tampere in Finland and rural Greece.

This paper reports a study of the complex associations between older people's participation in activities and their wellbeing in later life using data from a national sample of 1,334 Jewish-Israeli retirees. Confirmatory factor analysis substantiated a division of the activities into solitary, formal and informal categories, as postulated by activity theory. The outcome measure, the latent construct wellbeing, was compiled from scores on the 12-item General Health Questionnaire (GHQ-12), a global measure of life satisfaction, and a measure of satisfaction with the use of time. The analysis also examined the influence of socio-economic status, health status and the quality of inter-personal relationships. ‘Social relationship quality’, also a latent construct, was a composite of measures of satisfaction with children, friends and neighbours and a self-rated loneliness scale. Path analysis using structural equation modelling was employed. The results showed that when the quality of social relationships was taken into account, the amount of activity had no independent effect on the respondents' wellbeing. Moreover, it was social relationship quality, a facet of informal activity that has generally been neglected in activity research, that emerged as the most influential variable in the association between activity and wellbeing. Thus, the findings provide empirical backing for the assertion that the quality of social ties matters more than activity participation per se as predictors of a good old age.

Since 1990, Albania has witnessed rural out-migration on a massive scale, both to other countries, chiefly Italy and Greece, and internally to Tirana and other major towns. The scale of this migration has disrupted the multi-generational rural social and kinship systems that, before 1990, displayed strong and coherent family bonds, and simultaneously accommodated paternalistic state directives and were supported by welfare provision for all members of the population. The sudden political, social and rural dislocations that followed the end of the communist regime have made older people particularly vulnerable: many have been left behind by their emigrant children, creating the phenomenon of socially-isolated ‘elderly orphans’. While the migrants' remittances cushion this social isolation, the loss of children and grandchildren through emigration has undermined older people's self-respect and raison d'être in Albanian family life. This paper, based on fieldwork and interviews in regions of heavy out-migration in northern and southern Albania, examines the human impact of emigration on the older people who have been left behind as well as their coping mechanisms, one being to follow their children abroad to care for the grandchildren, enabling the ‘middle generation’, working-age parents both to engage in paid work.

It was evident from interviews with 15 men and women over 70 years of age that the experiences derived from being physically active and playing sport in later life are expansive, dynamic and completely imbued with multiple interpretations. Furthermore, their beliefs support the notion that the physically active body should be considered more than a physical object, as it represents a conscious, feeling, thinking and reflective self. The findings signify much is to be gained by regularly partaking in deliberate physical activity during later life although this is not always as easy as it sounds. At a personal level, changes in functional capacity and dealing with the sometimes ailing body can serve as a deterrent. From a broader perspective, the stigma associated with being older also poses a significant physical, sociological and psychological challenge, particularly during the initial phases. In conclusion, the study discusses why change must occur at a personal and societal level before a greater proportion of the older population become more physically active.

‘Active ageing’ is a key concept in current policy and research on ageing and yet is under-analysed or interpreted largely within an economic framework. This paper explores active ageing in the broader context of older people's lives. Drawing on a series of biographical interviews with 23 people aged 60–96 years, the discussion focuses on the theme of future hopes and concerns. Exhortations for ‘active ageing’ may be challenged by a lifelong unwillingness to look forward or plan ahead. Nevertheless, the findings show that fears for a future of limited resources, decline and dependency can exist alongside not only the desire to live longer but also the positive anticipation of forthcoming events and strong inter-generational relations. ‘Living for now’ and ‘taking a day at a time’ – and by extension the accomplishment of everyday activities rather than the activity-driven goals of earlier years – are common strategies for dealing with the unpredictability of later life. In this respect, even stopping paid work and entering residential care may be actively chosen and empowering even though they are steps towards disengagement and dependency. Similarly, planning for death, such as writing (living) wills and making funeral arrangements, may be positive and proactive responses to beliefs and concerns about dying. ‘Active ageing’ therefore needs to offer choices for life to be lived at all stages whilst recognising that much of the focus for older people is on ordinary needs, deeds and relationships.

Loneliness is often associated with old age, but many studies have shown that the relationship is not straightforward. This paper seeks a better understanding of the impact of social isolation on feelings of loneliness among older people, by building on the theoretical and actual distinction between social and emotional loneliness. Social loneliness refers to a lack of feelings of social integration; emotional loneliness emerges in the absence of an attachment figure. This paper focuses on social loneliness and has two aims, first to disentangle the direct and intermediate effects of both the number and the quality of social relationships on social loneliness in old age, and second to detect the groups at risk of social loneliness by identifying which personal features correspond with which relational deficits and therefore indirectly increase the risk on social loneliness. Data are analysed for a sample of 1,414 respondents aged 55 or more years drawn from the Panel Study of Belgian Households conducted in 2000. The results confirm that improved understanding is gained by decomposing the interrelation between age and other background features, on the one hand, and the social relational features, on the other, as indirect and direct predictors of social loneliness. Generally, this approach promotes a correct identification of the groups at risk of social loneliness in old age.

In spite of the many studies conducted over the past decades, the field of inquiry known as cross-cultural gerontology remains atheoretical. This is because of its shortcomings in generating culturally-relevant theoretical frameworks of its own. In this article, I address this failure and adapt Kluckhohn's model of value orientations for use in the study of ageing-related concepts. I discuss possible applications of the adapted model and, in particular, its application to one of the most frequently debated concepts in gerontology, successful ageing. In the light of this discussion, I conclude that the culturally-relevant theoretical framework hereby proposed could lead to the rectification of the current atheoretical predicament of cross-cultural gerontology.

This paper reports findings from a European Commission funded study of future long-term care expenditure in Germany, Italy, Spain and the United Kingdom, and presents projections of future long-term care expenditure in the four countries under a number of assumptions about the future. Macro-simulation (or cell-based) models were used to make comparable projections based on a set of common assumptions. A central base-case served as a point of comparison by which to explore the sensitivity of the models to alternative scenarios for the key determinants. The sensitivity of the models to variant assumptions about the future numbers of older people, the prevalence of functional dependency and informal care, patterns of long-term provision, and macroeconomic conditions are examined. It was found that, under the base-case, the proportion of gross domestic product spent on long-term care is projected to more than double between 2000 and 2050 in each country. The projected future demand for long-term care services for older people is sensitive to assumptions about the future number of older people, the prevalence of dependency and the availability of informal care, and projected expenditure is sensitive to assumptions about rises in the real unit-costs of services and the structure of the models. It is important, for planning purposes, to recognise the considerable uncertainty about future levels of long-term care expenditure.

The fourth age remains a poorly understood phenomenon and there is a lack of understanding of the perceptions of those who might be considered to be living in it. This article draws on findings from a study of dignity in later life which examined the day-to-day experiences of participants who were in need of support and care because of failing health. It discusses their accounts of the changes to their sense of self and their identity that came about as a result of their ageing and health problems and looks also at the ways in which the support and care they received helped to shape their adjustment to those changes. The accounts given by participants reveal a great deal about the physical, mental and emotional effort entailed in maintaining a sense of self and highlight the essential role played by social relationships in the maintenance of identity. These findings are analysed by reference to emerging theories of the fourth age.

Ageing in place has been promoted by policy makers as the optimal residential solution for later life, premised on older people's reluctance to contemplate relocation, their declining residential mobility and high levels of residential satisfaction. This paper takes a critical perspective to the notion of ageing in place by examining older people's dislikes about, rather than levels of satisfaction with their home and neighbourhood environments, and establishing whether such dislikes influence a desire to move. Analysis of the 2004 Living in Wales Survey shows that despite high levels of residential satisfaction, a significant proportion of older people do wish to move. Logistic regression results indicate this desire is strongly associated with dislikes about their immediate home environment, more than neighbourhood factors. Contemplating a move in later life may be shaped more by a desire to ‘attach’ to people, than to remain in situ to preserve an attachment to place.

The current study describes from an attachment-theoretical viewpoint how intergenerational support in adult child-parent relationships is associated with wellbeing in both generations. The attachment perspective and its focus on affective relationship characteristics is considered as an important theoretical framework for the investigation of special relationships across the life span. Data from the Netherlands Kinship Panel Study (N=1,456 dyads) were analysed to investigate if relationship quality moderated the association between providing intergenerational support to parents and wellbeing in adult children, on the one hand, and receiving intergenerational support from children and wellbeing of older parents on the other hand. The perspectives of both relationship partners were taken into account to allow for dependence within dyads. Intergenerational support, in terms of instrumental help provision, was negatively associated with the child's and parent's wellbeing. Being the stronger and wiser partner in adult-child parent relationships, as reflected by giving advice and being the initiator within the relationship, was beneficial for the wellbeing of both generations. Additionally, relationship quality was the strongest predictor of wellbeing in both generations. Parental wellbeing was benefited by filial support in high quality relationships. If an intergenerational relationships was of high quality, the challenges of intergenerational support provision and receipt were easier to deal with for both generations, parents and children.

This paper is a contribution to the developing understanding of social relationships in institutional care settings. It focuses on two areas that have been neglected in research: the reasons for and types of social interaction in institutional settings, and the ways in which the context of people's lives shapes social interaction. The paper draws on ethnographic observations conducted in four care settings in Scotland using a symbolic interactionist perspective. It finds that residents communicate and interact, and that the personal, cultural and structural contexts frame social interaction and influence the ways that residents use humour, express sexuality, and show hostility. The paper concludes that residents create social interactions in which action is embedded, but do so within specific structural and cultural contexts. These contexts ‘control’ resident action by establishing frameworks for the interpretation of meaning. At the same time, each facet of context is ‘controlled’ by the ways in which residents actively take on the ‘role’ of others, and project ‘self’ and a ‘label’.

This investigation examines how support from adult children is affected by their parents' involvement in grandchild care. Instead of focusing on dyadic interactions, we adopt a gendered extended family perspective to examine how financial and emotional support from children was influenced when their siblings received help with child care from their elder parents. The data were from a two-wave (2001, 2003) longitudinal study of 4,791 parent–child dyads with 1,162 parents, aged 60 and older, living in rural areas of Anhui Province, China. Random effects regression showed that emotional support from both sons and daughters was strengthened when parents provided more child care for their other adult children; in addition, daughters were more emotionally responsive than sons under this situation. Concerning dyadic parent–child relationships, daughter and sons increased their financial support, and sons increased their emotional support when they themselves received help with child care from parents. We suggest taking a gendered extended family perspective when studying intergenerational relationships in rural China.

This study examines associations between quality of life and multiple indicators of socio-economic position among people aged 50 or more years in 10 European countries, and analyses whether the relative importance of the socio-economic measures vary by age. The data are from the Survey of Health, Ageing and Retirement in Europe (SHARE) in 2004. 15,080 cases were analysed. Quality of life was measured by a short version of the CASP-19 questionnaire, which represents quality of life as comprising four conceptual domains of individual needs that are particularly relevant in later life: control (C), autonomy (A), self-realisation (S) and pleasure (P). The short version has 12 items (three for each domain). Five indicators of socio-economic position were used: income, education, home ownership, net worth, and car ownership. A multiple logistic regression showed that quality of life was associated with socio-economic position, but that the associations varied by country. Relatively small socio-economic differences in quality of life were observed for Switzerland, but comparatively large differences in Germany. Education, income, net worth, and car ownership consistently related to quality of life, but the association of home ownership was less consistent. There was no indication that the socio-economic differences in quality of life diminished after retirement (i.e. from 65+ years). Conventional measures of socio-economic position (education and income), as well as alternative indicators (car ownership and household net worth), usefully identified the differential risks of poor quality of life among older people before and after the conventional retirement age.

This qualitative study explores the conditions and experiences of older adults' ‘formal’ volunteering through non-profit organisations (NPOs) in Toronto from both organisational and individual perspectives. In spite of the ageing population and the need for NPOs to expand their services, the participation of Canadian seniors in community volunteering has been stagnant for 15 years. What organisational and structural supports might encourage the expansion of volunteering among this group? How do current administrative conditions impact upon senior volunteers? What do older adults expect to gain from community volunteering? The qualitative data collected through interviews, documents and participant observation are analysed using an inter-disciplinary framework that combines theories of the moral economy of ageing, adult development and transformative learning. The results include a socio-demographic profile of senior volunteers in 12 Toronto NPOs, and the administrative characteristics of the six organisations that engage the majority. It is argued that the self-help and transformative mechanisms embedded in community volunteering provide opportunities for retirees to sustain their self-esteem and sense of wellbeing, while cultivating ‘generativity’ in late adulthood. Promoting transformative learning enables community volunteering to provide meaningful roles for seniors, and promotes citizenship participation and the social economy in an ageing society.

Loneliness has been consistently identified as one of the specific ‘social problems’ which accompanies old age and growing older: 90 per cent of the general population of Britain feel that loneliness is a problem associated with old age. There is a widespread presumption that loneliness and isolation have become more prevalent in Britain in the period since the Second World War as a result of the decline in multi-generation households and changes in family structure. This paper examines the accuracy of this stereotype and considers if current cohorts of older people are more likely to report experiencing loneliness than previous generations of elders, through a comparative analysis of historical and contemporary data. Historical data are provided by three ‘classic’ social surveys undertaken in England between 1945 and 1960. Contemporary data are from a postal survey of 245 people aged 65–74 living in South London in 1999. The questions used in all four surveys were comparable, in that respondents self-rated their degree of loneliness on scales ranging from never to always. The overall prevalence of reports of loneliness ranged from five to nine per cent and showed no increase. Loneliness rates for specific age or gender sub-groups were also stable. Reported loneliness amongst those living alone decreased from 32 per cent in 1945 to 14 per cent in 1999, while the percentages decreased for both those reporting that they were never lonely and that they were ‘sometimes’ lonely.

Elderly people with functional limitations are predominantly cared for by family members. Women – spouses and daughters – provide most of this care work. In principle, gender inequality in intergenerational care may have three causes: first, daughters and sons have different resources to provide care; second, daughters and sons respond differently to the same resources; third, welfare state programmes and cultural norms affect daughters and sons differently. In this paper, we address the empirical question whether these three assumed causes are in fact responsible for gender differences in intergenerational care. The empirical analyses, based on the Survey of Health, Ageing and Retirement in Europe (SHARE), reveal that parents in need are in fact more likely to receive care from daughters than from sons. Daughters are more responsive to the needs of their parents than sons and respond differently to the same resources. Gender inequality is highest in countries with a high level of intergenerational care, high public spending on old-age cash-benefits, a low provision of professional care services, high family obligation norms and a high level of gendered division of labour. Welfare state programmes reduce or increase gender inequality in intergenerational care by reducing or increasing the engagement of daughters in intergenerational care. In general, care-giving by sons is hardly influenced by social care policies.

The research reported here is concerned with the future of informal care over the next thirty years and the effect of changes in informal care on demand for formal services. The research draws on a PSSRU computer simulation model which has produced projections to 2031 for long-term care for England. The latest Government Actuary's Department (GAD) 1996-based marital status projections are used here. These projections yield unexpected results in that they indicate that more elderly people are likely to receive informal care than previously projected. The underlying reason is that the GAD figures project a fall in the number of widows and rise in the number of elderly women with partners. What this implies is that ‘spouse carers’ are likely to become increasingly important. This raises issues about the need for support by carers since spouse carers tend to be themselves elderly and are often in poor health. The article explores a number of ‘scenarios’ around informal care, including scenarios in which the supply of informal care is severely restricted and a scenario in which more support is given to carers by developing ‘carer-blind’ services. This last scenario has had particular relevance for the Royal Commission on Long Term Care.
