Urban–rural harmonisation has risen to prominence in recent social security reform in China. This article offers an account of the changing welfare institutions and social citizenship configurations unfolded by this particular policy approach. As social activism gained substantial weight as part of the regional developmental strategies of local governments, harmonisation efforts have led to a boundary shift of social citizenship largely defined by the within–without criterion rather than the urban–rural divide. In places where urban–rural harmonisation takes hold, the pivotal criterion for claiming social benefits is the possession of local resident status, regardless of whether this status is urban or rural. The heterogeneity of regional social security developments resulting from social decentralisation also calls attention to the ‘variable geometry’ of institutional change, i.e. various social policy domains manifest diverse degrees of institutional dynamics towards harmonisation. In this light, urban–rural harmonisation is likely to trigger competitive solidarity in terms of regional competition and emulation in economic development and social provision, leading to regional disparities that will shape the future contours of social policy and social citizenship in China.

Many governments world-wide are promoting longer working life due to the social and economic repercussions of demographic change. However, not all workers are equally able to extend their employment careers. Thus, while national policies raise the overall level of labour market participation, they might create new social and labour market inequalities. This paper explores how institutional differences in the United Kingdom, Germany and Japan affect individual retirement decisions on the aggregate level, and variations in individuals’ degree of choice within and across countries. We investigate which groups of workers are disproportionately at risk of being ‘pushed’ out of employment, and how such inequalities have changed over time. We use comparable national longitudinal survey datasets focusing on the older population in England, Germany and Japan. Results point to cross-national differences in retirement transitions. Retirement transitions in Germany have occurred at an earlier age than in England and Japan. In Japan, the incidence of involuntary retirement is the lowest, reflecting an institutional context prescribing that employers provide employment until pension age, while Germany and England display substantial proportions of involuntary exits triggered by organisational-level redundancies, persistent early retirement plans or individual ill-health.

Forecasts suggest that unemployment will remain above 2 million in Britain for the best part of the 1980s. The social consequences of protracted high levels of unemployment are explored. The evidence comes from four types of source: cross-sectional surveys of (unemployed) individuals; longitudinal and cohort studies; aggregrate time series; and area-based studies. The advantages and limitations of each are identified. The available evidence on the consequences of unemployment for poverty; health and mortality; mental health; crime and delinquency; and the social fabric is reviewed. The implications for policy and public expenditure are explored.

This article examines the relationship between disability, generation and social policy. The moral and legislative framework for the post-war welfare settlement was grounded in a long-standing cultural construction of ‘normal’ life course progression. Disability and age (along with gender) were the key components in this construction, defining broad categories of welfare dependency and labour force exemption. However, social changes and the emergence of new policy discourses have brought into question the way in which we think about dependency and welfare at the end of the twentieth century. The article suggests that, as policy-makers pursue their millennial settlement with mothers, children and older people, they also may be forced to reconstruct the relationship between disabled people and the welfare state.

While poverty is widely accepted to be an inherently multi-dimensional concept, it has proved very difficult to develop measures that both capture this multi-dimensionality and facilitate comparison of trends over time. Structural equation modelling appears to offer a solution to this conundrum and is used to exploit the British Household Panel Study to create a multi-dimensional measure of poverty. The analysis reveals that the decline in poverty in Britain between 1991 and 2003 was driven by falls in material deprivation, but more especially by reduced financial stress, particularly during the early 1990s. The limitations and potential of the new approach are critically discussed.

This article presents an historical overview of the changing meaning of the patient-consumer, and specifically the role played by patient groups in constructing the patient as consumer. It is argued that patient groups were central to the formation of the patient-consumer, but as health consumerism was taken on by the state, they lost control of this figure. Competing understandings of what it meant to be a patient-consumer developed, a shift that raises further questions about the unity of claims made in the name of the patient-consumer.

Contemporary policies to reduce emissions of greenhouse gases (GHGs) will have distributive consequences and thus implications for the scope and remit of social policy. This paper studies current carbon mitigation policies and their distributive impacts. It considers a range of current and proposed social programmes to ameliorate these impacts, before proposing alternatives. This argument is pursued in two parts according to whether emissions are conceived and accounted within a production or a consumption framework. The first part works within the Kyoto policy framework, critiques the present suite of policies and suggests alternative policy scenarios that may better marry together the goals of carbon reduction and social equity. The second half justifies and operationalises a broader focus on all GHGs emitted by British consumers, whether directly or embodied in goods and services. It argues that to target these will require going beyond the current policy paradigm to develop more radical policies to modify preferences and behaviour, and to constrain total consumption demand. It then speculates on ways that new social policy programmes might combine the pursuit of these goals together with social equity.

Recent feminist literature on care has suggested that, conceptually, it is better to dissolve the boundaries between ‘formal’ and ‘informal’ care when analysing care. This article suggests that there are policy developments taking place, both in Britain and in some of the countries of Europe, which are dissolving the boundaries between formal and informal care, particularly as far as payment for care is concerned. It is argued that, in this case, far from benefit systems being a form of ‘decommodification’ they are actually a form of ‘commodification’ of the caring relationship. The article then explores these empirical developments, considers their gendered nature and genderedimpact, and their possible consequences for the relationship between care-giver and care-recipient.

Evidence on social determinants of health and health equity (SDH/HE) is abundant but often not translated into effective policy action by governments. Governments’ health policies have continued to privilege medical care and individualised behaviour-change strategies. In the light of these limitations, the 2008 Commission on the Social Determinants of Health called on health agencies to adopt a stewardship role; to take action themselves and engage other government sectors in addressing SDH/HE. This article reports on research using analysis of health policy documents – published by nine Australian national or regional governments – to examine the extent to which the Australian health sector has taken up such a role.

We found policies across all jurisdictions commonly recognised evidence on SDH/HE and expressed goals to improve health equity. However, these goals were predominantly operationalised in health care and other individualised strategies. Relatively few strategies addressed SDH/HE outside of access to health care, and often they were limited in scope. National policies on Aboriginal health did most to systemically address SDH/HE.

We used Kingdon's (2011) multiple streams theory to examine how problems, policies and politics combine to enable, partially allow, or prevent action on SDH/HE in Australian health policy.

Questions concerning what it means to be a human agent and the capacities of those who receive welfare services to reflect upon and shape their lives, and the kinds of social conditions which create opportunities for such ‘reflexivity’, have begun to move to the centre of social policy and social work analysis. Using empirical evidence drawn from a study of child and woman protection, this paper argues that, contrary to claims that the concept of self-reflexivity as developed in the work of Beck and Giddens is of little relevance to marginalised citizens, in late-modernity the socially excluded are using social work and welfare services in creative ways to critically engage in life-planning, to find safety and healing. However, the data suggest that much greater specificity is needed in relation to the areas in which it is possible to act to change and develop the self and the social world in late-modernity. The paper argues for a complex theory of agency and reflexivity in welfare discourse which takes account of the intersection of structural disadvantage, intervention practices and personal biography and how people adjust to adversity and cope with toxic experiences and relationships in their lives. This helps to account for the limits to the capacities of agents to reflect and know why they act as they do and their capacities to act destructively, as well as providing for an appreciation of the creative, reflexive welfare subject.

This paper reports the extent and nature of adult protection cases dealt with by two Social Services Departments during a twelve month period in 1995–6, within the framework of newly revised generic policies on abuse of vulnerable adults. The two counties varied in the level of reporting documented and in the number of cases logged for individuals who fall within the main four groups of ‘vulnerable adults’ covered by the policies. Arguments for consistent reporting are set out and the tension which exists between formal and informal approaches to the subsequent investigation of abuse discussed with reference to American vulnerable adult statutes. Adult protection work inevitably overlaps with existing planning and regulation systems such as social care assessment, care planning, the care programme approach and inspection and registration of residential homes. The point at which these systems need to be augmented by specific adult protection procedures is a matter of professional judgement and negotiation within and between agencies. Continued monitoring of cases reported under the procedures will allow these authorities to reflect on progress to date and may provide one set of norms against which other authorities can evaluate their own practice.

In the last two decades there have been a number of social, medical and legal initiatives in the UK and elsewhere to provide assistance to women who suffer violence from their partner. The most recent innovations focus on responding to the men who perpetrate this violence. In this article we present the initial results of the first British study of programmes for violent men. The three-year study used a longitudinal method to compare the effects of two court mandated programmes with other, more orthodox, forms of criminal justice intervention (fines, admonishment, traditional probation, prison). Here we describe the men's programmes, locate the current study in the context of existing evaluations of similar programmes operating in North America, outline the methods employed, present the results of the post-hoc matching used to assess the probable effects of selection bias and using subsequent prosecutions and the accounts of women, compare the impact of different criminal justice interventions. The results indicate that twelve months after the criminal justice intervention a significant proportion of the Programme men had not subsequently been violent to their partner. This was in contrast to men sanctioned in other ways (the Other CJ group) who were much less likely to have changed their violent behaviour.

A recent edition of this Journal (Volume 16, Part 2, April 1987) was devoted to a number of articles on the definition and measurement of poverty. Surprisingly, perhaps, this did not include any specific discussion of gender differences in the causes, extent and experience of poverty. But such gender differences do exist, though they are often obscured by much research on poverty. Our initial response to the special edition on poverty was to write a reply discussing how the various contributors had ignored the issue of gender. But women cannot simply be ‘added in’ to existing analyses; instead a different analytic framework is required. This article therefore begins by looking at some of the widespread evidence of the economic disadvantage of women compared with men. We then go on to discuss why it is necessary to focus on the gender dimensions of poverty. We argue that this involves far more than simply disaggregating data to produce statistics about the situation of women. Rather, this focus leads us to explore the structural causes of women's poverty and the gendered processes in the labour market, welfare systems and domestic household which interact to create and maintain that disadvantage. In the final section we consider some of the important conceptual and methodological issues which must be tackled if we are to find ways to investigate and measure poverty which are not gender-blind.

The ways in which government supports families and protects children are always a fine balance. In recent years, we suggest that this balance can be characterised increasingly as ‘child-centric’, less concerned with families and more focused on individual children and their needs. This article charts the changes in families and government responses over the last 40 years, and the way this is reflected in organisational and administrative arrangements. It notes in particular the impact on everyday practice of the introduction of information and communication technologies. Findings are reported from recent research which shows the struggles faced by practitioners who try to manage systems which separate children from their familial, social and relational contexts. As a consequence, we suggest, the work has become increasingly fragmented and less mindful of children's life within families. While the data and analysis draw on research carried out in England, we suggest that similar changes may be going on in other Western liberal democracies.

The employment of migrant workers in long-term care is increasingly evident across western welfare states. This article examines the ways in which immigration controls shape the exercising of choice and control by migrant care workers over their labour. It draws on the findings of in-depth interviews with migrant care workers employed by residential and home care providers and by older people and their families in the UK. It is argued that the differential rights accorded to migrants on the basis of citizenship and immigration status shape, first, entry into particular types of care work, second, powers of ‘exit’ within work, and, third, ‘voice’ regarding the conditions under which care labour is provided.

This article is concerned in part to inform the quest for an understanding of the perceptions which Muslim minorities have of Western welfare state provision, but its wider purpose is to explore the essence and the potential of the Islamic welfare state. Heuristic models constructed by social policy academics have provided insights into the influences of religion upon different kinds of welfare state, but no model exists by which specifically to understand Islamic welfare traditions. The article explains the Islamic tradition of Zakat; its significance as one of the central pillars of Muslim faith, and the principles through which it addresses the relief of poverty and the redistribution of wealth. Islamic conceptions of state and community are then explored and the ways in which Muslim faith and culture are adapting at both the global and community level. The article concludes with some speculative remarks about the scope for rapprochement between Western debates about the moral basis for welfare and Muslim perspectives on social justice.

This article argues that the social construction of user participation policies includes both differences and similarities regarding three user groups: older people, disabled people and people with mental health problems. The article is based on a historical discourse analysis of national documents in Norway. It points at a democracy/social rights discourse, based on the idea of social citizenship, as a common and historically stable discourse for all three user groups and relates this to the specific characteristics of Norwegian welfare policies. A contrasting consumer discourse, stressing users’ consumer role and related to the impact of New Public Management reforms, is only evident in the case of older people and from the 1990s. A co-production/co-partnering discourse, stressing user/professional-partnership, is evident in the current policies directed at older people and those with mental health problems. Both the consumer and co-production discourse remain marginal in the case of disabled people.

There is a long tradition in social policy of discussing and critiquing the notion of ‘deservingness’ in relation to ‘the poor’. This paper will apply such debates to ‘the rich’ to consider the grounds on which this group might be considered ‘deserving’. The paper identifies three sets of arguments. The first set of arguments concerns the appropriateness of rewarding merit/hard work/effort/risk-taking etc. The second concerns more consequentialist/economic arguments about providing incentives for wealth creation. And the third considers the character and behaviour of the rich. As well as discussing the potential criteria for deservingness, the paper will also debate whether the degree of income and wealth gained by the rich is deserved. Finally, the paper will discuss the social policy implications, including taxation policies, which emerge from this debate.

International debates about the comparative institutional structures of welfare states have focused on social expenditure and the inclusiveness of social policy. However, these debates have not accounted for the significant rise of fiscal welfare and, in particular, social tax expenditures (STEs) in our understanding of welfare regimes. The growth of STEs has been particularly significant in Australia. While there has been recognition that STEs contribute to a second tier of welfare provision in some policy domains, there has been no systematic attempt to account for them within the institutional structure of the Australian welfare state. In this article, we chart the rise of STEs, the reasons for their growth in the Australian political economy and conceive of them as forming a second institutional layer of a dual welfare state. We conclude by suggesting that this analysis has broader implications for other, particularly liberal, welfare regimes.

In common with most countries in the world, China is facing an increasing demand for welfare. The free-market economic reforms of the last decade have dramatically transformed social policy in China, which is gradually retreating from an egalitarian and collective approach and moving towards a pluralistic and ‘residual’ orientation. Now, widening income inequalities are accepted as both necessary and inevitable.
