Contrast media are frequently used during radiation therapy simulation. However, there are concerns about dosimetric variations when dose calculation is done on contrast-enhanced computed tomography (CT). This study evaluates the dosimetric effect of oral contrast during three-dimensional conformal radiotherapy (3D-CRT) and volumetric modulated arc radiotherapy (VMAT) planning.

Rectal cancer patients were consecutively enrolled. For each patient, one unenhanced CT and one contrast-enhanced CT were taken using oral and intravenous contrast. Then, a 3D-CRT plan and an Intensity-modulated radiation therapy (IMRT)/VMAT plan were generated in the enhanced CT, and the dose distribution was recalculated in the respective unenhanced CT. The beam intensities were kept the same as for the enhanced CT plans. Finally, the unenhanced and enhanced plans were compared by calculating the gamma index.

For 3D-CRT plans, there were statistically significant differences in second phase planning target volume (PTV) D2% (Mean difference (MD) between unenhanced and enhanced CT 0·01 Gy, 95% CI [0·003 to 0·02 Gy]) and in maximum doses to the bladder (MD 0·26 Gy, 95% CI [0·05 to 0·47 Gy]). For IMRT/VMAT plans, there were statistically significant differences in small intestine V45 Gy (MD 3·1 cc, 95% CI [0·81 to 5·4 cc]), bladder V45 Gy (MD 2·9%, 95% CI [1·4 to 4·3%]) and maximum dose to the bladder (MD 0·65 Gy, 95% CI [0·46 Gy to 0·85 Gy]). In addition, for PTV D98% the MD between unenhanced and enhanced CT was 0·22 Gy 95% CI [0·05 to 0·39].

For most of the dose metrics, the differences were not clinically meaningful. The greatest differences were found in VMAT plans, especially in V45 Gy of the small intestine. This difference could lead to an underestimation of dose–volume metrics when the plan is based on an enhanced CT. The use of small bowel oral contrast does not significantly influence dose calculations and may not affect the acceptability of plans when adhering to constraints.

This study aims to evaluate dosimetric parameters like percentage depth dose, dosimetric field size, depth of maximum dose surface dose, penumbra and output factors measured using IBA CC01 pinpoint chamber, IBA stereotactic field diode (SFD), PTW microDiamond against Monte Carlo (MC) simulation for 6 MV flattening filter-free small fields.

The linear accelerator used in the study was a Varian TrueBeam® STx. All field sizes were defined by jaws. The required shift to effective point of measurement was given for CC01, SFD and microdiamond for depth dose measurements. The output factor of a given field size was taken as the ratio of meter readings normalised to 10 × 10 cm2 reference field size without applying any correction to account for changes in detector response. MC simulation was performed using PRIMO (PENELOPE-based program). The phase space files for MC simulation were adopted from the MyVarian Website.

Variations were seen between the detectors and MC, especially for fields smaller than 2 × 2 cm2 where the lateral charge particle equilibrium was not satisfied. Diamond detector was seen as most suitable for all measurements above 1 × 1 cm2. SFD was seen very close to MC results except for under-response in output factor measurements. CC01 was observed to be suitable for field sizes above 2 × 2 cm2. Volume averaging effect for penumbra measurements in CC01 was observed. No detector was found suitable for surface dose measurement as surface ionisation was different from surface dose due to the effect of perturbation of fluence. Some discrepancies in measurements and MC values were observed which may suggest effects of source occlusion, shift in focal point or mismatch between real accelerator geometry and simulation geometry.

For output factor measurement, TRS483 suggested correction factor needs to be applied to account for the difference in detector response. CC01 can be used for field sizes above 2 × 2 cm2 and microdiamond detector is suitable for above 1 × 1 cm2. Below these field sizes, perturbation corrections and volume averaging corrections need to be applied.

Comparison of the integral dose (ID) delivered to organs at risk (OAR), non-target body and target body by using different techniques of craniospinal irradiation (CSI).

Ten CSI patients (medulloblastoma) already planned and treated either with linear accelerator three-dimensional conformal radiation therapy (Linac-3DCRT) technique or with linear accelerator RapidArc (Linac-RapidArc) technique by Novalis-Tx Linac machine have been analysed. Retrospectively, these patients are again planned on Radixact-X9 Linac with Helical, Direct-3DCRT and Direct-intensity-modulated radiation therapy (Direct-IMRT) techniques. The dose prescription to planning target volume brain (PTV-Brain) and PTV-Spine is 36 Gy in 20 fractions and is kept the same for all techniques. The target body, non-target body, OARs and total body dose are compared.

ID is lowest in the RapidArc plan for every patient in comparison to Helical and Direct-IMRT. The ID for Body-PTV was found slightly higher in the RapidArc plan in comparison to 3DCRT plans. But there is better normal tissue sparing for most of the OARs in RapidArc plans if it compares with 3DCRT plans.

RapidArc is a better alternative for the treatment of CSI. It provides better target coverage and better OARs sparing from any other treatment techniques.

Monte Carlo (MC) simulations have been used extensively for benchmarking photon dose calculations in modern radiotherapy using linear accelerators (linacs). Moreover, a major barrier to widespread clinical implementation of MC dose calculation is the difficulty in characterising the radiation source using data reported from manufacturers.

This work aims to develop a generalised full MC histogram source model of an Elekta Precise SL-25 linac (electron exit window, target, flattening filter, monitor chambers and collimators) for 6 MV photon beams used in standard therapies. The inclusion of many different probability processes such as scatter, nuclear reactions, decay, capture cross-sections and more led to more realistic dose calculations in treatment planning and quality assurance.

Two different codes, MCNPX 2·6 and EGSr-BEAM, were used for the calculation of particle transport, first in the geometry of the internal/external accelerator source, and then followed by tracking the transport and energy deposition in phantom-equivalent tissues. A full phase space file was scored directly above the upper multilayer collimator’s jaws to derive the beam characteristics such as planar fluence, angular distribution and energy spectrum. To check the quality of the generated photon beam, its depth dose curves and cross-beam profiles were calculated and compared with measured data.

In-field dose distributions calculated using the accelerator models were tuned to match measurement data with preliminary calculations performed using the accelerator information provided by the manufacturer. Field sizes of 3×3, 5×5, 10×10, 15×15 and 20×20 cm2 were analysed. Local differences between calculated and measured curve doses beneath 2% were obtained for all the studied field sizes. Higher discrepancies were obtained in the air–water interface, where measurements of dose distributions with the ionisation chamber need to be shifted for the effective point of measurement.

The agreements between MC-calculated and measured dose distributions were excellent for both codes, showing the strength and stability of the proposed model. Beam reconstruction methods as direct input to dose-calculation codes using the recorded histograms can be implemented for more accurate patient dose estimation.

This is the second of two papers giving an overview of the use of Monte-Carlo techniques for radiotherapy applications.

The first paper gave an introduction and introduced some of the codes that are available to the user wishing to model the different aspects of radiotherapy treatment. It also aims to serve as a useful companion to a curated collection of papers on Monte-Carlo that have been published in this journal.

This paper focuses on the application of Monte-Carlo to specific problems in radiotherapy. These include radiotherapy and imaging beam production, brachytherapy, phantom and patient dosimetry, detector modelling and track structure calculations for micro-dosimetry, nano-dosimetry and radiobiology.

The impact of COVID-19 social restrictions on mental wellbeing of health professional students during placement is largely unknown. Conventional survey methods do not capture emotional fluctuations. Increasing use of smartphones suggests short message service (SMS) functionality could provide easy, rapid data. This project tested the feasibility and validity of gathering data on Therapeutic Radiography student mental wellbeing during clinical placement via emoji and SMS.

Participants provided anonymous daily emoji responses via WhatsApp to a dedicated mobile phone. Additional weekly prompts sought textual responses indicating factors impacting on wellbeing. A short anonymous online survey validated responses and provided feedback on the method.

Participants (n = 15) provided 254 daily responses using 108 different emoji; these triangulated with weekly textual responses. Feedback concerning the method was positive. ‘Happy’ emoji were used most frequently; social interaction and fatigue were important wellbeing factors. Anonymity and opportunity to feedback via SMS were received positively; ease and rapidity of response engendered engagement throughout the 3-week study.

The use of emoji for rapid assessment of cohort mental wellbeing is valid and potentially useful alongside more formal evaluation and support strategies. Capturing simple wellbeing responses from a cohort may facilitate the organisation of timely support interventions.

It is unclear whether body mass index (BMI) is a useful measurement for examining prostate motion. Patient’s subcutaneous adipose tissue thickness (SAT) and weight has been shown to correlate with prostate shifts in the left/right direction. We sought to analyse the relationship between BMI and interfraction prostate movement in order to determine planning target volume (PTV) margins based on patient BMI.

In all, 38 prostate cancer patients with three implanted gold fiducial markers in their prostate were recruited. Height, mass and SAT were measured, and the extent of interfraction prostate movement in the left/right, superior/inferior and anterior/posterior directions was recorded during each daily fiducial marker-based image-guided radiotherapy treatment. Mean corrective shift in each direction for each patient, along with BMI values, were calculated.

The median BMI value was 28·4 kg/m2 (range 21·4–44·7). Pearson’s product-moment correlation analysis showed no significant relationship between BMI, mass or SAT and the extent of prostate movement in any direction. Linear regression analysis also showed no relationship between any of the patient variables and the extent of prostate movement in any direction (BMI: R2=0·006 (ρ=0·65), 0·002 (ρ=0·80) and 0·001 (ρ=0·86); mass: R2=0·001 (ρ=0·87), 0·010 (ρ=0·54) and 0·000 (ρ=0·99); SAT: R2=0·012 (ρ=0·51), 0·013 (ρ=0·50) and 0·047 (ρ=0·19) for shifts in the X, Y and Z axis, respectively). Patients were grouped according to BMI, as BMI<30 (n=25, 65·8%) and BMI≥30 (n=13, 34·2%). A two-tailed t-test showed no significant difference between the mean prostate shifts for the two groups in any direction (ρ=0·320, 0·839 and 0·325 for shifts in the X, Y and Z axis, respectively).

BMI is not a useful parameter for determining individualised PTV margins. Gold fiducial marker insertion should be used as standard to improve treatment accuracy.

Therapeutic radiographers routinely offer advice to patients regarding treatment-related side effects. Cranberry has long been used as a natural remedy for several health complaints and has more recently been suggested as having a role in the treatment and prophylaxis of urinary tract infection (UTI) and symptoms.

The aim of this review was to investigate whether there is a place for cranberry as part of the management of radiation cystitis caused by radiotherapy treatment of pelvic cancers, in order to aid therapeutic radiographers in tailoring their advice regarding pelvic side effects.

A structured search was carried out using PubMed, CINAHL, Scopus and Cochrane Library databases. A total of 25 articles were selected for review.

Themes of mechanism of action of cranberry, composition of cranberry products, cranberry and UTI, use of cranberry in radiotherapy and further issues to consider were identified and explored.

A lack of high-quality data was identified in the literature reviewed and no firm evidence was found to support the continued recommendation of cranberry as part of management of radiation induced urinary tract side effects. Well-designed randomised controlled trials are required before further recommendations regarding the use of cranberry in radiotherapy are made.

Meta-analyses demonstrate single-fraction radiotherapy to be as effective as multi-fraction treatment in palliating painful bone metastases, although surveys suggest reluctance in prescribing single fractions.

Assess the factors influencing the choice of dose-fractionation regimen in an unselected population; examine retreatment rates and subsequent skeletal events.

Data were extracted from case notes for 120 patients treated in 2000 and 2006 in a single centre serving a defined population; analysis used χ2 and Fisher's exact statistical tests.

An 8 Gy fraction was the commonest regimen prescribed (single-fraction delivery rate 53·6%). Tumour site was a significant factor in choice of dose-fractionation schedule. Patients with metastatic breast carcinoma were significantly less likely to receive single-fraction treatment compared with those with metastatic lung carcinoma (year 2000: p = 0·038, 2006: p = 0·001). There was a significantly higher retreatment rate following single-fraction compared with multi-fraction treatment (11% versus 3%). There were two subsequent neural axis compressions and four pathological fractures.

Single-fraction treatment is the commonest regimen but multiple fractions are still frequently delivered. Better prognosis groups appear more likely to receive multi-fraction treatment, possibly to avoid the need for retreatment. Subsequent skeletal events are rare but carry high morbidity when they occur.

This work reports our study to commission a radiochromic film dosimetry system using the timely EBT3 film. We carried out dosimetric evaluations on different characteristics of photon beams (e.g., flatness, symmetry and penumbra) in radiation dose delivery.

A Varian linear accelerator producing 6 and 15 MV photon beams with 120 multi-leaf collimator was used in this study. PTW ionisation chamber was used to measure the beam characteristics such as symmetry, flatness and penumbra and these measurements were used to commission the radiochormic EBT3 film dosimetry system. The results of irradiated films were analysed using the radiochromic film QA Pro software 2016.

The measured film doses were analysed at two different colour channels (green and red) using two scanning geometries (i.e., upper or lower side of film facing the scanner light source) at two dose levels (10 and 40 Gy). The difference between the ionisation chamber and film results was found insignificant and within the acceptable range as per the World Health Organisation standard.

Results of the comparison between the ionisation chamber and film measurements show that our radiochormic EBT3 film dosimetry system is reliable and cost-effective in the output measurement of a linear accelerator. Our measurements confirm that our EBT3 film dosimetry agreed well with the ionisation chamber, and can be used as a re-validation tool for linear accelerator quality control.

To establish whether the use of a passive or active technique of planning target volume (PTV) definition and treatment methods for non-small cell lung cancer (NSCLC) deliver the most effective results. This literature review assesses the advantages and disadvantages in recent studies of each, while assessing the validity of the two approaches for planning and treatment.

A systematic review of literature focusing on the planning and treatment of radiation therapy to NSCLC tumours. Different approaches which have been published in recent articles are subjected to critical appraisal in order to determine their relative efficacy.

Free-breathing (FB) is the optimal method to perform planning scans for patients and departments, as it involves no significant increase in cost, workload or education. Maximum intensity projection (MIP) is the fastest form of delineation, however it is noted to be less accurate than the ten-phase overlap approach for computed tomography (CT). Although gating has proven to reduce margins and facilitate sparing of organs at risk, treatment times can be longer and planning time can be as much as 15 times higher for intensity modulated radiation therapy (IMRT). This raises issues with patient comfort and stabilisation, impacting on the chance of geometric miss. Stereotactic treatments can take up to 3 hours to treat, along with increases in planning and treatment, as well as the additional hardware, software and training required.

Four-dimensional computed tomography (4DCT) is superior to 3DCT, with the passive FB approach for PTV delineation and treatment optimal. Departments should use a combination of MIP with visual confirmation ensuring coverage for stage 1 disease. Stages 2–3 should be delineated using ten-phases overlaid. Stereotactic and gated treatments for early stage disease should be used accordingly; FB-IMRT is optimal for latter stage disease.

The purpose of this study was to develop the patient-specific quality control (QC) process by most commonly used dosimeters in Bangladesh and recommend a suitable passing rate for QC, irrespective of the dosimetric tools used.

Intensity-modulated radiation therapy (IMRT) and volumetric-modulated arc therapy (VMAT) plans of five head-and-neck (HN) and five prostate patients were selected for the patient-specific QC. These plans were generated using the Eclipse TPS v11·0 (Varian Medical Systems, Inc., Palo Alto, CA, USA) 6 MV X-ray from a Varian TrueBeam linear accelerator (Varian Medical Systems, Inc.) for each case. Each IMRT and VMAT plans were measured by two-dimensional (2D) ion chamber arrays (I’matriXX) and electronic portal imaging devices (EPID), respectively. The passing rates of the dosimetric tools were calculated using criteria of 3%/3 mm.

The average passing rates (±SD) of I’matriXX for prostate and HN were 97·9±0·76 and 98·88±0·24, respectively. For VMAT verification, the average passing rates of EPID for prostate for arc1 and arc2 were 96·15±0·49 and 97·8±0·70, respectively; similarly, for HN the rates were 97·85±0·63 and 97·2±0·56, respectively.

The results showed that both the dosimeters can be used in patient-specific QC, although the EPID-based IMRT and VMAT QC is more advantageous in terms of time-saving and ease of use. Hence, for patient-specific QC, one can use the ion chamber arrays (I’matriXX) or EPID in hospital, but the systems need to be cross-checked.

The incidence of breast cancer has surpassed cervical cancer in India and it has now become the most common cancer in women. Multiple randomised studies have reported low α/β value in the range of 3–4 for breast cancer, which predict a potential radiobiological advantage for hypofractionated radiotherapy resulting in such schedules becoming standard in many centers with reduction in overall treatment time. Volumetric modulated arc therapy (VMAT) is a novel technique of delivering radiotherapy that reduces treatment delivery time, requires less monitor units (MU) and offers good conformity. The mean dose to normal tissue may be minimised using this technique although there will be inferior sparing if we consider the low-dose volume such as V5, the effect of which is not quantifiable yet.

Reporting acute toxicity, cosmetic effects, and quality of life in patients of early breast cancer treated with adjuvant hypofractionated VMAT with SIB.

The records of 44 patients registered at the hospital between August 2014 and December 2015 were included in this analysis. Acute toxicities were analysed using CTCAE v4.03. Cosmetic outcomes were assessed using Harvard scale, while quality of life outcomes were assessed using EORTC scales and Health Related quality of life (HRQOL) questionnaires (QLQ-C30 and QLQ-BR23).

No grade ≥2 skin toxicities were recorded. Breast pain was recorded as Grade 1 in 13·8% patients and Grade 1 fatigue in 18·2%. The maximum haematological abnormality grade recorded was Grade 1. Cosmesis was assessed at the baseline, 6 months, 1 year and 2 years. A total of 88·6% of the patients had an Excellent or good cosmesis at the baseline, which was similar even at 6 months, at 88·7%, improved further at 1 year to 90·9%. At 6 months post radiotherapy, high functional scale QOL scores were noted.

The technique is associated with minimum acute toxicity, good to excellent cosmesis and acceptable quality of life.
