Men's supportive stance is an essential component for making women's world better. There are growing debates among policymakers and researchers on the role of males in maternal health programmes, which is a big challenge in India where society is male driven. This study aims to look into the variations and determinants of maternal health care utilization in India and in three demographically and socioeconomically disparate states, namely Uttar Pradesh, West Bengal and Maharashtra, by husband's knowledge, attitude, behaviour towards maternal health care and gender violence, using data from the National Family Health Survey III 2005–06 (equivalent to the Demographic and Health Survey in India). Women's antenatal care visits, institutional delivery and freedom in health care decisions are looked into, by applying descriptive statistics and multivariate models. Men's knowledge about pregnancy-related care and a positive gender attitude enhances maternal health care utilization and women's decision-making about their health care, while their presence during antenatal care visits markedly increases the chances of women's delivery in institutions. From a policy perspective, proper dissemination of knowledge about maternal health care among husbands and making the husband's presence obligatory during antenatal care visits will help primary health care units secure better male involvement in maternal health care.

This paper describes the biological fitness of an urban population of Havana city, Plaza de la Revolución, which has the lowest fertility and the highest demographic ageing in Cuba. The aim is to assess the biological fitness of this community through the indexes of action opportunity of natural selection, to determine its evolutionary pattern and the influence of its socio-cultural peculiarity. Demographic data were obtained from the reproductive histories of 1200 women between the ages of 55 and 64. Data concerning mortality and surviving offspring from the first embryonic stages until age of reproduction were also collected. In order to measure the level of biological fitness two indexes were used: the Crow index of action opportunity of natural selection and the corrected index proposed by Johnston and Kensinger, which takes into account prenatal mortality. This corrected index was calculated including and excluding induced abortions in order to evaluate the contribution of these to biological fitness. When only postnatal mortality was considered, the results showed an evolutionary pattern similar to that of developed countries, based on low mortality and fertility. However, when prenatal mortality was taken into account, biological fitness decreased and the corrected index of natural selection was 4.5 times higher than when miscarriages and fetal deaths were not considered. Moreover, this corrected index was 2.65 times higher when induced miscarriages were considered, indicating the large decrease in biological fitness as a result of the current reproductive behaviour of frequent induced abortion.

Obesity has been linked to several physiological and psycho-social diseases, decreases in cognitive function, poor levels of scholastic achievement, low socioeconomic status and delayed onset of maturity. This study investigates the association between obesity and both academic performance and reasoning ability in 394 male and 398 females students between the ages of 6 and 12 from Lisbon, Portugal. It also assesses how this relationship may be influenced by chronological age, maturity and socioeconomic status. The results suggest that: 1) reasoning ability is independent of socioeconomic status and level of maturity; 2) no differences in reasoning ability exist between groups of different BMI; 3) academic performance is moderated by chronological age in boys, and by maturity in both genders; 4) obesity is not associated with academic performance. It is concluded that reasoning ability and academic performance are not associated with obesity, and that inter-individual differences in academic performance in boys may be explained by differences in their level of maturity.

This study examines the hypothesis that the stopping rule – a traditional postnatal sex selection method where couples decide to cease childbearing once they bear a son – plays a role in high sex ratio of last births (SRLB). The study develops a theoretical framework to demonstrate the operation of the stopping rule in a context of son preference. This framework was used to demonstrate the impact of the stopping rule on the SRLB in Vietnam, using data from the Population Change Survey 2006. The SRLB of Vietnam was high at the level of 130 in the period 1970–2006, and particularly in the period 1986–1995, when sex-selective abortion was not available. Women were 21% more likely to stop childbearing after a male birth compared with a female birth. The SRLB was highest at parity 2 (138.7), particularly in rural areas (153.5), and extremely high (181.9) when the previous birth was female. Given the declining fertility, the stopping rule has a potential synergistic effect with sex-selective abortion to accentuate a trend of one-son families in the population.

Around 25% of abortions in South Africa are performed in the second trimester. This study aimed to better understand what doctors, nurses and hospital managers involved in second trimester abortion care thought about these services and how they could be improved. Nineteen in-depth interviews with abortion-related service providers and managers in the Western Cape Province, South Africa, were undertaken. Data were analysed using a thematic analysis approach. Participants expressed resistance to the dilation and evacuation (D&E) procedure, as this required more active provider involvement. Medical abortion was preferred as it required less provider involvement in the abortion process. A shortage of providers willing to perform D&E resulted in most public sector services being outsourced to private sector doctors. Respondents noted an increased demand for services and a concomitant lack of infrastructure, physical space and personnel to respond to these demands, sometimes resulting in fragmented or poor quality care. At medical induction sites, most thought introducing the combined mifepristone–misoprostol regimen would improve service capacity, although they were concerned about cost. Improving contraceptive services was also seen as a much-needed intervention to improve care and prevent abortion. Ongoing training, including values clarification, as well as emotional support and team-building for providers are needed to ensure sustainable, high-quality second trimester abortion services.

Women carry the primary responsibility for family planning in most parts of the world, and should be afforded the power of decision-making and control over their fertility. This study seeks to gain insight into Iranian women's perception of the meaning of empowerment in family planning. Using a qualitative study, seven focus group discussions and five individual interviews were conducted with 35 married Iranian women of reproductive age. The data were analysed using a conventional content analysis approach, in which themes and categories were explored to reveal women's experiences of empowerment in family planning. The results demonstrated four main categories: control over fertility plan, participative family planning, maintaining health and access to optimal family planning services. They viewed knowledge of family planning and autonomy of decision-making in fertility issues as essential elements for control of their fertility plan. Participants felt more empowered when joint family planning decisions were made with their partners in an atmosphere of agreement. Therefore, family planning policymakers should plan services with new approaches that focus on women's health and empowerment.

Using a sample of 2090 British father and son pairs the relationships between social and geographical intra- and inter-generational mobility were examined in relation to height, weight and body mass index (BMI). There was much more social mobility than geographical (regional) migration. Social mobility and geographical migration were not independent: socially non-mobile fathers and sons were more likely to be geographical non-migrants, and upwardly socially mobile fathers and sons were more likely to be regional migrants. Upwardly socially mobile fathers and sons were, on average, taller and had a lower BMI than non-mobile and downwardly mobile fathers and sons. In general, no significant associations were found between geographical migration and height or weight. Migrating fathers had a lower BMI than sedentes, as did their sons who migrated between 1965 and 1991. There was no significant interaction that indicated that social mobility and geographical migration were acting in a simple additive way on height, weight and BMI.

This text comments on the article by Lundborg et al. ‘Getting ready for the marriage market? The association between divorce risks and investments in attractive body mass among married Europeans’ published in July 2007 in this journal. This commentary presents differing results from the original data using multilevel analysis for calculation. The results presented here suggest there is no significant relation between divorce risk and body mass index (BMI) among married individuals in European countries. Therefore, the primary finding of Lundborg et al. (2007) is questioned.

Overweight and obesity constitute a major and increasing health and welfare problem throughout the world. Assessing the multifaceted mechanisms – biological, environmental and behavioural – behind this development is a crucial task in medical, social and economic sciences. We are, therefore, grateful to have been given the opportunity to, once again, discuss whether the risk of divorce may be one of the factors influencing the incentives of becoming overweight or obese and, hence, ultimately the physical appearance among the married. In this Debate, colleagues Schneider and Grimps present the results of a multilevel analysis, in which they could not identify any statistically significant association between body mass index (BMI) and divorce risk among married people. Thus, they question the findings, previously published in this Journal (Lundborg et al., 2007). The Schneider and Grimps arguments are not convincing, however. So, we still claim that the statistical material at hand does, indeed, imply that divorce risk at the national level may well influence the weight of the married.

There is an obvious need for better understanding of people's motives for body weight change, due to the importance of the health-related, social and economic consequences of obesity. In particular, exploration of little-discussed social aspects could provide further insights, but requires, however, close examination. This also applies to the study of the effect of marriage-market-related incentives and divorce risks on individual body mass index (BMI). Nevertheless, the ongoing debate about whether and to what extent the risk of divorce influences the body constitution of middle-aged individuals has as yet failed to mature into a common agreement. This paper will therefore re-examine theoretical assumptions and statistical calculations to clarify still contentious points. Finally, the results of this study suggest once more that there is no significant relationship between country-specific divorce risk and BMI among married individuals in Europe.
