Yersinia enterocolitica biotype 1, serotype O.10K was isolated from 19 patients in the paediatric wards of a district general hospital over a period of 3 months. Fifteen cases were patients on the medical ward. Shortly afterwards. Y. enterocolitica biotype 1, serotype 0.6,30 was isolated from a further 17 patients on this ward in 1 month. The same serotypes of Y. enterocolitica were isolated from the pasteurized milk supplied to the ward. Epidemiological evidence indicated that contaminated pasteurized milk was the source of the yersinia organisms exereted by the patients.

In 1988, there were two outbreaks of legionellosis in Bolton Health District. Altogether 37 cases of Legionnaires' disease and 23 cases of non-pneumonic legionellosis were identified. Twenty-five patients with Legionnaires' disease were associated with an engineering plant, 4 with Bolton town centre, and 8 with both the plant and town centre. Twenty-two people with non-pneumonic legionellosis were linked with the engineering plant and one with the plant and the town centre. A case-control study carried out among 37 employees with legionellosis and 109 control subjects at the plant showed that infection was associated with one of the 15 cooling towers on the site. Legionella pneumophila indistinguishable by serological and genetic typing methods was isolated from this cooling tower and from sputum samples from two patients. In the town centre, no one tower was linked with infection and L. pneumophila was not cultured from any of the nine towers identified. Control measures were implemented and to date there have been no further cases of legionellosis associated with Bolton Health District.

The coccidian protozoan parasite Cryptosporidium has been described in many host species since its discovery in the early part of the century, but it remained obscure until the recognition by veterinary workers in the 1970s of its importance as a cause of scours in young livestock animals [1–4].

One hundred and seventy-nine isolates of Legionella pneumophila serogroup 1, obtained from a site associated with an outbreak of Legionnaires' disease, were examined by monoclonal antibody subgrouping, restriction fragment length polymorphism typing, restriction endonuclease analysis and plasmid content. Nine distinct phenotypes were detected but at the genotypic level all strains were closely related. The data presented indicate that phenotypic variation of a single parent strain can occur within an environmental site. The implications of these findings are discussed in relation to the investigation of outbreaks of Legionnaires' disease.

Pasteurized bottled milk supplied by a single dairy was frequently found to be contaminated with Yersinia spp. Investigations were carried out at the dairy in an effort to pinpoint the source of these organisms. Viable counts obtained from milk bottle rinses indicated that bottle washing was often unsatisfactory, and on one occasion Y. frederiksenii was isolated from the pooled rinse water of six bottles. Samples of milk were taken on arrival at the dairy and at various stages following pasteurization. Heat resistance tests carried out on strains of yersinia isolated from pasteurized milk indicated that they would not survive the pasteurization process. However two strains of yersinia were isolated from a sample of milk taken immediately after pasteurization but before bottling. The thermograph indicated that the time/temperature conditions applied during pasteurization were adequate. The presence of yersinia strains in the milk at this stage therefore suggests that undetectable levels of raw milk were being allowed to contaminate the pasteurized milk. The absence of yersinia in cartoned samples produced on the same day as contaminated bottled samples indicated that environmental contamination of the bottle filler valve also may have occurred at times. Results of this investigation indicate that increased vigilance is required to ensure proper operation of pasteurizers and bottle washers.

This survey assessed the maintenance of evaporative cooling towers in Glasgow, following two Legionnaires' disease outbreaks. Information was obtained from 76 of 81 premises and a maintenance score was calculated for each of 174 towers. The quality of maintenance was extremely varied (range of maintenance scores, 8–30 mean, 22 (S. D., 5.0); median, 23; maximum possible, 33) and some towers were neglected. Breaches of maintenance principles were mainly structural and organizational, e.g. inadequate drift control, rather than failure to use chemicals. Low maintenance scores were associated with no log book, no guidelines, no change in procedures in last 5 years, solitary cooling towers, and towers on industrial premises. Despite intense publicity the standard of cooling tower maintenance in Glasgow remained a concern. Information campaigns directed at those responsible for cooling–tower maintenance are neccessary.

Sera from 252 patients with community-acquired pneumonia were examined for the presence of antibodies to 15 antigens of 7 Legionella spp. by indirect immunofluorescent antibody testing. The sera had been collected as part of the British Thoracic Society/Public Health Laboratory Service study of community-acquired pneumonia in adults. We also examined sera from 20 patients with gram-negative sepsis.

A large outbreak of Legionnaires’ disease was associated with Stafford District General Hospital. A total of 68 confirmed cases was treated in hospital and 22 of these patients died. A further 35 patients, 14 of whom were treated at home, were suspected cases of Legionnaires’ disease. All these patients had visited the hospital during April 1985. Epidemiological investigations demonstrated that there had been a high risk of acquiring the disease in the out patient department (OPD), but no risk in other parts of the hospital. The epidemic strain of Legionella pneumophila, serogroup 1, subgroup Pontiac la was isolated from the cooling water system of one of the air conditioning plants. This plant served several departments of the hospital including the OPD. The water in the cooling tower and a chiller unit which cooled the air entering the OPD were contaminated with legionellae. Bacteriological and engineering investigations showed how the chiller unit could have been contaminated and how an aerosol containing legionellae could have been generated in the U–trap below the chiller unit. These results, together with the epidemiological evidence, suggest that the chiller unit was most likely to have been the major source of the outbreak.

Nearly one third of hospital staff had legionella antibodies. These staff were likely to have worked in areas of the hospital ventilated by the contaminated air conditioning plant, but not necessarily the OPD. There was evidence that a small proportion of these staff had a mild legionellosis and that these ‘influenza–like’ illnesses had been spread over a 5–month period. A possible explanation of this finding is that small amounts of aerosol from cooling tower sources could have entered the air–intake and been distributed throughout the areas of the hospital served by this ventilation system. Legionellae, subsequently found to be of the epidemic strain, had been found in the cooling tower pond in November 1984 and thus it is possible that staff were exposed to low doses of contaminated aerosol over several months.

Control measures are described, but it was later apparent that the outbreak had ended before these interventions were introduced. The investigations revealed faults in the design of the ventilation system.

Clinical information was collected on 722 cases of Listeria monocytogenes infections in humans occurring in Britain between 1967 and 1985: 248 cases (34%) were associated with pregnancy (maternal, foetal, and neonatal), and comprised 9 cases (4%) of maternal bacteraemia without infection of the foetus; 42 cases (19%) of intra-uterine deaths; 118 cases (54%) of neonatal infection diagnosed within 2 days post-partum; and 50 cases (23%) of neonatal infection diagnosed as ill after 2 days post-partum. An overall mortality of 50% was recorded.

The cases unassociated with pregnancy are described elsewhere (see accompanying paper).

The colonization, survival and control of Legionella pneumophila in a hospital hot–water system was examined. The organism was consistently isolated from calorifier drain–water samples at temperatures of 50°C or below, despite previous chlorination of the system. When the temperature of one of two linked calorifiers was raised to 60°C, by closing off the cold–water feed, the legionella count decreased from c. 104 c.f.u./l to an undetectable level. However, 10 min after turning on the cold–water feed which produced a fall in calorifier temperature, the count in the calorifier drain water returned to its original level. Investigations revealed that the cold–water supply was continually feeding the calorifiers with L. pneumophila. Simple modifications in the design of the system were made so that the cold–water feed no longer exceeds 20°C; these measures have considerably reduced the number of L. pneumophila reaching the calorifiers.

Clinical information was collected on 722 cases of Listeria monocytogenes infections in humans occurring in Britain between 1967 and 1985. This series comprised 34% (248 cases) associated with pregnancy and 66% (474 cases) in non-pregnant adults and juveniles. The cases not associated with pregnancy comprised: 76% in patients with severe underlying illness (of which 34% had central nervous system infections, and 42 % bacteraemia without involvement of the central nervous system); 21% in previously healthy individuals (of whom 18% had meningitis); and 3% in patients without bacteraemia or involvement of the central nervous system. Cases occurred most often in male patients over 40 years of age. The overall mortality was 44%.

Overall, the pattern of infection has altered to a disease of higher incidence, affecting most often susceptible non-pregnant individuals and the unborn.

An annual increase in incidence of listeriosis occurred in the autumn in all categories of patients.

A prospective study of genital infection was conducted in four inner-city family–planning clinics. Fifteen per cent of routine attenders had symptoms and signs of vaginal infection and many more women attended primarily because of symptoms. Among the women with both signs and symptoms, 70% had positive laboratory findings, Trichomonas vaginalis, Candida albicans and bacterial vaginosis being equally prevalent. Measurement of vaginal pH in the clinic was the single most useful clinical finding for directing empirical therapy. Among patients with a discharge confirmed on examination and an abnormally high pH, 72% had either T. vaginalis or bacterial vaginosis. Neisseria gonorrhoeae was isolated from 4% of women with, and 1% of those without, symptoms. We believe that it is worthwhile to investigate patients presenting to family-planning clinics with vaginal symptoms. No single specimen was found ideal for all pathogens, a cervical swab is better for gonococci and also for T. vaginalis but a vaginal swab is needed for Candida and bacterial vaginosis.

During a study of genital infection in inner-city family-planning patients we examined 452 women for Chlamydia trachomatis. The prevalence of infection was 7·3%. There was no significant difference between patients attending because of genital symptoms and those who were attending for routine family-planning advice. Infection was found to be correlated with five main demographic parameters; age less than 25, no stable partnership, hormonal contraception, nulliparity and West Indian Ethnic origin. Using these parameters a simple scoring system was devised which allowed a high-risk population to be defined in whom screening would be economically justified.

The largest recorded outbreak of foodborne botulism in the United Kingdom occurred in June 1989. A total of 27 patients was affected; one patient died. Twenty-five of the patients had eaten one brand of hazelnut yoghurt in the week before the onset of symptoms. This yoghurt contained hazelnut conserve sweetened with aspartame rather than sugar. Clostridium botulinum type B toxin was detected in a blown can of hazelnut conserve, opened and unopened cartons of hazelnut yoghurt, and one faecal specimen. Cl. botulinum type B was subsequently cultured from both opened and unopened cartons of the hazelnut yoghurt and from one faecal specimen. Investigations indicated that the processing of the conserve was inadequate to destroy Cl. botulinum spores. Control measures included the cessation of all yoghurt production by the implicated producer, the withdrawal of the firm's yoghurts from sale, the recall of cans of the hazelnut conserve, and advice to the general public to avoid the consumption of all hazelnut yoghurts.

A survey of ABO blood groups, secretor status and smoking habits among 389 students and staff of a school in which there was an outbreak of meningococcal disease found no difference in the distribution of the ABO blood groups but a significantly higher proportion of non-secretors (37·6%) in the population examined compared with that reported for previous surveys of the neighbouring population in Glasgow (26·2%) (P < 0·0005). There was also a significantly higher proportion of non-secretors among carriers of meningococci (47%) compared with non-carriers (32%). Increased carriage of meningococci among non-secretors might contribute to the increased susceptibility of individuals with this genetic characteristic to meningococcal disease observed in previous studies. Although passive exposure to cigarette smoke has been associated with meningococcal disease, there was no association between passive smoking and carriage. There was, however, a significant association between active smoking and carriage.

Fifty-eight sera, from 29 chickens originating from two layer flocks known to be naturally infected with Salmonella enteritidis phage type (PT) 4, were examined for antibodies to S. enteritidis. Using the techniques of immunoblotting and ELISA, antibodies to the lipopolysaccharide (LPS) of S. enteritidis were detected in 43 of 58 sera. Antibodies were of the IgG class and bound to the S. enteritidis LPS antigen 0=12. Bacterial agglutination reactions using whole-cell preparations of S. enteritidis and S. pullorum, correlated with anti-LPS antibody reactions as detected by immunoblotting and ELISA. A rapid means of screening chicken sera for antibodies to the LPS of S. enteritidis as an indicator of infection is discussed.

Infant mice were used for the evaluation of the efficacy of bismuth subaslicylate (BSS) in the prevention the growth of Campylobacter jejuni in the intestine. The MIC90 of ten C. jejuni strains was 900 μg/ml. Of three dosage regimens tested, continuous treatment before and after the bacterial challenge, mimicking the way BSS is used in the prevention of traveller's diarrhoea, was the most effective. Growth inhibition was dose dependent; the high dose of 2000 μg per day was more effective than 300 μg per day. After cessation of treatment, campylobacter counts increased to the same level as in the control animals.

Of 28 multi-resistant isolates of Staphylococcus aureus collected during 1986 from hospitals in major cities around Australia, 27 were found to contain the same prophage (denoted phage 604). Hospital isolates carrying three or fewer resistance markers, and community isolates carrying one or no resistance markers, did not carry this prophage. Phage 604 does not confer antibiotic resistance on its lysogens, nor does it increase virulence in chick embryo assays. Phage 604 appears to be a correlate of antibiotic multi-resistance in S. aureus in Australia, and may provide a molecular marker for incipiently epidemic strains of this bacterium in Australian hospitals.

Trimethoprim-resistance genes of Shigella dysenteriae 1 strains, isolated from a different location of six different countries of Asia over a 5-year period were characterized by using three different dihydrofolate reductase (DHFR) gene probes. The trimethoprim-resistant (TMPR) strains hybridized only with the type I DHFR gene probe by colony hybridization. None of the strains hybridized with types II and III DHFR gene probes. Southern blot experiments using plasmid DNA extracted from these resistant strains indicated that the type I DHFR genes were either on a 20 MDa plasmid or might be located on the chromosome. None of the other plasmids present in S. dyysenteriae 1 strains hybridized with the probe. This indicates that the TMP resistance in these S. dysenteriae 1 strains are mediated by type I DHFR enzyme, and there may be transposition of this type I DHFR gene occurs between the 20 MDa plasmid and the chromosome in this serotype of shigella.

Earlier studies showed that the minimum infective dose (>106 organisms) of a virulent strain of Fusobacterium necrophorum could be greatly reduced by suspending the fusobacteria in sub-lethal doses of cultures of other bacteria such as Escherichia coli before inoculating mice subcutaneously.

In the present study the infective dose of the same strain of F. necrophorum was reduced by a factor of >103 by suspending the fusobacteria in sub-lethal doses of 5% homogenate of gaur or wallaby faeces. Sterile faecal filtrate had no such effect. The sites of low grade infection produced by the prior subcutaneous injection of E. coli culture or gaur faecal suspension were susceptible to superinfection by doses of F. necrophorum far below those required to infect normal tissue.

This work helps to explain the production of necrobacillosis by the faecal contamination of small wounds. It proved impossible, however, to produce necrobacillosis in mice by the subcutaneous injection of faecal suspensions from 33 farm cattle. This suggests that the proportion of cattle with virulent F. necrophorum in their faeces is low.
