A 3-year-old boy with known significant cardiomyopathy presented to his local A&E with a 2-day history of cough and coryza. He had been brought in due to parental concern regarding a reduction in oral intake and lethargy. The referring team described him as being irritable and inconsolable, disorientated but talking. His chest was clear but he had mildly increased work of breathing. Abdominal examination revealed a large liver. The CXR unsurprisingly, showed significant cardiomegaly.
