  • Tinnitus and its relationship with muscle tenderness in patients with headache and facial pain

  • Objective:

    This study aimed to examine tinnitus prevalence in patients with different types of headache and the relationship between tinnitus and the pericranial muscle tenderness and cervical muscle tenderness scores.

    Methods:

    A cross-sectional study was conducted of 1251 patients with migraine and/or myogenous pain, arthrogenous temporomandibular joint disorders and tension-type headache. Standardised palpation of the pericranial and cervical muscles was carried out and univariable and multivariable analysis was used to measure the odds ratio of suffering tinnitus by the different diagnoses and muscular tenderness grade.

    Results:

    A univariable analysis showed that myogenous pain, pericranial muscle tenderness and cervical muscle tenderness scores, sex, and age were associated with tinnitus. When a multivariable model including only age, sex and a headache diagnosis was used, myogenous pain, migraine and age were found to be associated with tinnitus. When muscle tenderness scores were also included, only the cervical muscle tenderness and pericranial muscle tenderness scores were found to be significantly associated with tinnitus.

    Conclusion:

    In a population of patients with headache and craniofacial pain, tinnitus was related to increased cervical muscle tenderness and pericranial muscle tenderness scores, rather than to any particular form of headache.

  • Current evidence based data found in the scientific literature recommend that the therapeutic management of patients suffering from temporomandibular disorders (TMD) should be based initially on simple, conservative and reversible procedures including exercise therapy. The integration of this message into oral health instructions for patients makes it easy for them to keep informed about TMD and aware of the steps they can take to deal with their symptoms. The integration of exercise therapy into structured patient education programs provides patients with information about TMD and suggests methods to deal with it. This medical approach is similar to those used to treat other joints in the musculoskeletal system.

    Easy to set up, this exercise therapy helps patients suffering from temporomandibular disorders to become partners in the conduct of their treatment plan of reducing pain and restoring functional comfort.

  • Summary

    The evolution of thinking regarding temporomandibular disorders (TMDs) has included many theories of etiology and pathogenesis, ranging from anatomical features of the temporomandibular joint (TMJ), to occlusal abnormalities, to behavioral origins such as bruxism. Many people with TMDs use over-the-counter medications to reduce their pain. The case studies in this chapter illustrate some of the variation in symptoms, duration of pain and treatment history typically seen in patients at a TMD clinic. Epidemiological research in the area of TMDs has focused primarily on estimates of prevalence rates for signs and symptoms, and descriptions of characteristics of TMD cases versus controls. Rational treatment of TMDs includes pharmacological adjuncts to optimize other treatment modalities such as splint therapy, physical therapy or biofeedback. Chronic, debilitating TMD pain that persists despite comprehensive multidisiplinary treatment may necessitate the long-term use of opiates.
