In 2010, Ireland found itself at the eye of an international storm as a network of head shops emerged selling new psychoactive substances (NPS) and Irish youth rapidly became the heaviest users of NPS in Europe. Within months, the Irish government enacted novel legislation, which has since been copied by other countries, which effectively stopped the head shops selling NPS. Critics of this policy argued that it could cause harms to escalate. A number of separate studies indicate that a range of drug-related harms increased amongst Irish youth during the period of head shop expansion. Within months of their closure, health harms began to decline. NPS-related addiction treatment episodes reduced and admissions to both psychiatric and general hospitals related to any drug problem began to fall. Population use underwent sustained decline. Consequently, the closure of head shops can be viewed as a success in terms of public health.

Background: Most admissions to hospitals occur through the emergency department (ED). The impact of emergency physicians’ decisions to admit a patient to hospital can have wide ranging effects on health care spending, hospital congestion and patient outcomes. A growing body of evidence shows that outpatient management of conditions such as diverticulitis, heart failure and pulmonary embolism is both safe, effective and can reduce costs. Aim Statement: To support emergency staff in making safe, informed decisions to appropriately reduce admission rates without increasing the rate of patients returning and being admitted. Measures & Design: Significant variability in admission rates between emergency physicians exists and no correlation between actual and self-reported admission rates is observed. One means to change behavior is through audit and feedback, however a Cochrane review on this topic concluded that it was only effective if specific conditions were met; findings which were incorporated into this project. An audit tool was created comparing individual physicians’ admission and “bounce back” rates to their peers. The tools contained averages for the individual and site for admission and bounce back rates and were shared with physicians every 2 months. Physicians were divided into three equal groups, low, medium and high admitters and targets established. Department heads met with high admitters. Evaluation/Results: The project was started in September 2016. Admission rates in the three physician groups were compared in the ten months before September 2016 (prior) and after January 2017 (post). September to December 2016 was considered the “rollout” period and not included in the analysis. Significance was tested using a Permutation test and a p-value cut off level of 5%. Nine emergency departments took part. Seven sites experienced a significant decrease in the admission rate of top admitters, three showed a significant increase in the rate of low admitters and two showed a significant increase in the rate of medium admitters. Pooled results showed a decrease in the admission rates of the top admitters and no significant change to the medium or low admitters. Discussion/Impact: Comparing the pre- and post-periods yielded a decrease in admissions of 773 patients on an annualized basis. The impact of the change in the top five highest admitters at the biggest three hospitals estimated an annualized beds savings of 25.3 beds.

Streptococcus pneumoniae is an important cause of community-acquired pneumonia and pneumococcal conjugate vaccines (PCVs) may reduce this burden. This study's goal was to analyse trends in lower respiratory tract infections (LRTI) hospitalisations before and during a routine vaccination programme targeting all newborns with PCV was started in the province of Quebec, Canada in December 2004. The study population included hospital admissions with a main diagnosis of LRTI among 6–59 month-old Quebec residents from April 2000 to December 2014. Trends in proportions and rates were analysed using Cochran-Armitage tests and Poisson regression models. We observed a general downward trend in all LTRI hospitalisations rate: from 11·55/1000 person-years in 2000–2001 to 9·59/1000 in 2013–2014, a 17·0% reduction, which started before the introduction of PCV vaccination. Downward trends in hospitalisation rates were more pronounced for all-cause of pneumonia (minus 17·8%) than for bronchiolitis (minus 15·4%). There was also a decrease in the mean duration of hospital stay. There was little evidence that all-cause pneumonia decreased over the study period due mainly to the introduction of PCVs. Trends may be related to changes in clinical practice. This study casts doubt on the interpretation of ecological analyses of the implementation of PCV vaccination programmes.

Physical health outcomes for patients with long-term conditions can be significantly affected by their psychological wellbeing; those experiencing psychological difficulties are less able to manage symptoms, have a poorer quality of life, and more frequent hospital admissions. National guidance recommends the assessment and treatment of psychological difficulties secondary to respiratory disease, but implementation of this across services is inconsistent. Here, we describe the process and findings of a 9-month study integrating psychological assessment and intervention into an acute respiratory department. The aim of this provision was to provide appropriate interventions for both in-patients and out-patients with identified psychological needs, and to evaluate the impact of these across a range of outcome domains. Psychological assessment and intervention was flexibly implemented as clinically appropriate within the context of the wider multidisciplinary team. Hospital admissions data were collated, and feedback obtained from both patients and staff. Results highlighted that psychological provision was well received by both patients and staff, and was associated with improved patient experiences and a greater focus on holistic care. Of the 73 patients receiving psychology input with at least 1 month of follow-up data, 75% showed a reduction in their admission frequency following intervention. The estimated savings to the wider NHS exceeded the costs of providing psychology input. In light of existing literature, national guidance, and the present findings, we highlight the need for those commissioning and managing respiratory services to consider the various potential benefits of integrating psychological provision for a patient group with high levels of psychological need.

Young age, adverse environmental conditions and infectious agents are established risk factors of lower respiratory tract infection (LRTI), whereas pneumococcal conjugate vaccines may be protective. To explore their relative role as predictors of hospitalizations under the continental climate prevailing in the province of Quebec, Canada, an ecological study was performed. Records with a main diagnosis of LRTI in children born during 2007–2010 and observed up to their second-year anniversary were extracted from the provincial hospital administrative database. Respiratory virus surveillance data and statistics on ambient air temperature were obtained. Vaccine use in different birth cohorts was derived from the Quebec City Immunization Registry. Additive and multiplicative Poisson regression models were applied to estimate attributable fractions. Age, month of birth, ambient temperature, and respiratory syncytial virus (RSV), human metapneumovirus (hMPV) and influenza-positive test proportions were significant predictors of LRTI hospitalizations. No substantial differences were observed in cohorts exposed to the 7-valent or 10-valent pneumococcal conjugate vaccines. In the additive model, the fraction of hospitalizations explained by temperature variation was 37%, whereas RSV circulation explained 28%, hMPV 4% and influenza 1%. Complex interplay between biological, environmental and social mechanisms may explain the important role of ambient air temperature in predicting LRTI hospitalization risk in young children.

There is increasing worldwide recognition of the social and economic burden of psychotic illness. Substance use disorders frequently complicate the treatment of psychosis and are associated with increased readmission rates, decreased compliance, and increased violence and suicide risk. The Northern Territory of Australia has a scattered remote population – 25% of whom are Indigenous people. There are increasing rates of drug use across the Territory, and hospital admission rates have been gradually increasing. Psychotic disorders are the most frequent cause of hospital admissions, and substance abuse complicates the majority of these admissions. It is imperative that comprehensive treatment programs are established, integrating services, and utilising culturally appropriate cost-effective strategies.

La prévalence élevée des états dépressifs dans la clientèle du généraliste (environ 10% des consultants) et chez les patients de médecine interne (15 à 25% des hospitalisés) est maintenant bien établie. Ce fait a été longtemps négligé par les praticiens et les psychiatres; même actuellement son importance reste sous-estimée.

La proportion d’états dépressifs non-identifiés par les praticiens varie de 1/3 à 2/3 selon les études. Les dépressions méconnues sont source de surconsommation médicale (consultations répétées, examens paracliniques, hospitalisations pour bilans) et ont une évolution moins favorable, plus chronique, que les dépressions identifiées et correctement traitées. Des cas sévères de dépression échappent souvent au dépistage, ce qui n’est pas sans conséquences graves: on sait en effet que la majorité des gens qui se suicident ont consulté un médecin dans le mois qui a précédé leur décès.

Parmi les facteurs qui rendent compte de la méconnaissance des états dépressifs par les médecins non-spécialistes, les uns tiennent aux patients qui, dans leur dialogue avec le généraliste, utilisent davantage le langage de la somatisation qu’ils ne verbalisent explicitement un vécu dépressif. D’autres tiennent aux praticiens, à leur intérêt prévalent et parlois exclusif pour les problèmes somatiques, à leur souci légitime de ne pas passer à côté du diagnostic d’une affection médicale, à l’idée qu’ils se font de la dépression et des déprimés, à leur propension à considérer toutes les manifestations dysphoriques comme conséquences légitimes de la maladie physique. Il faut aussi compter avec les conditions d’exercice du généraliste, le temps très court de sa consultation où il n’est pas facile de différencier une réelle pathologie dépressive, généralement liée à des troubles anxieux et moins nette que dans les consultations Psychiatriques, de troubles émotionnels mineurs, situationnels et transitoires.

Les instruments habituellement utilisés pour le dépistage et le diagnostic de la dépression (échelles d’autoévaluation, critères de diagnostic type RDC ou critères du DSM III) n’ont pas été validés pour les populations de patients somatiques. Récemment deux nouveaux instruments viennent d’être mis au point pour la détection de la symptomatologie dépressive chez les patients somatiques: I'échelle HAD (Hospital Anxiety and Depression Scale) de Zigmond et Snaith (1983) et le Questionnaire Abrégé d’Auto-Évaluation de la Symptomatologie Dépressive (QD2A) de Pichot et al. (1984). Ces échelles, de passation rapide et bien acceptées par les patients, peuvent servir d’aide au diagnostic pour le praticien dans la perspective d’un meilleur dépistage de la dépression et d’une prise en charge plus efficace des déprimés.
