By the use of computerized tomography (CT) the authors have examined and precisely defined the radiological anatomical characteristics of the attic and its relation to other structures of the middle and inner ear. The identification of individual morphological structures has been made on axial, semi-axial and frontal CT sections. Each type of original CT section has been compared with the corresponding original anatomical section.

This study compared nasopharyngeal sepsis with under-aeration ear disease. Nasopharyngeal sepsis was assessed by culture taken directly from the surface of removed adenoid tissue. 156 cases were studied; 100 showed varying degrees of middle-ear under-aeration and 56 had no active ear disease or history of it. In addition, an assessment of post-nasal obstruction was made and this too was compared with middle-ear status.

The conclusions suggested that, whilst normal ears can exist in the presence of accepted nasal pathogens, diseased ears are rarely seen in the absence of nasal sepsis. The degree of post-nasal obstruction seemed irrelevant.

The commonest nasopharyngeal pathogen was Haemophilus influenzae and when found alone this had a particularly detrimental affect on middle-ear aeration, causing a significant incidence of mucoid middle-ear effusion in this group.

Over the 27-year period from 1954–1981, 51 patients with non-Hodgkin's lymphoma arising in the tonsil were treated at the Regional Radiotherapy Centre, Mount Vernon Hospital. The original biopsy material was reviewed in each case and reclassified using the British National Lymphoma Investigation criteria. Grade 2 non-Hodgkin's lymphoma was reported in 80 per cent of cases. Ninety per cent of patients presented in stages IE or HE and were treated with radical local radiotherapy. A 5-year survival of 51 per cent was achieved and a 5-year relapse-free survival of 35 per cent. Stage IE patients had a statistically significant survival advantage compared to stage HE. Sixty-one per cent of patients relapsed, of whom 74 per cent died from active lymphoma. The abdomen was the most frequent site for relapse with involvement of both nodal and extranodal sites.

The frequency histograms of six patients' laryngographs were studied pre- and post-endotracheal intubation. The changes were correlated with the length of intubation. The post-operative hoarse voice was seen as an increase in the spread of the lower frequencies, and an increase in the fundamental frequency. The latter change was thought to be a subconscious compensation for the hoarseness. The increased lower frequency spread as measured by standard deviation was directly proportional to the length of intubation.

The ultrastructural, X-ray microanalytical, histochemical and immunocytochemi-cal features of intraluminal crystalloids found in adenocarcinomas of the parotid gland have been studied. The crystalloids, putatively derived from an abnormal crystalization of salivary duct proteins, are considerably different from the crystalloids found in normal parotid glands, pleomorphic adenomas, and sialocysts.

Otorhinolaryngological operations would be facilitated by the use of a safe, cheap, surgical adhesive. Though commercial fibrin glue is effective, it is both relatively expensive and, because it is prepared from pooled human blood, there has been concern that it carries the risk of transmitting viral disease, in particular hepatitis and acquired immune deficiency syndrome. We describe a rapid, cheap method of preparing sufficient fibrin glue, of adequate bonding power, from a small quantity of the patient's own blood, and therefore with no risk of contracting viral disease. Our method of preparation of fibrin glue promises to provide a valuable adjunct to otorhinolaryngological surgical practice.

Seventeen unrelated Danish patients with Pendred's syndrome, whose case stories have not been published previously, are presented. Acoustic and vestibular functions were examined and endocrinological screening was performed. There was a great variation in hearing ability as well as in thyroid function. Furthermore, in contrast to previous investigations, normal caloric function was demonstrated in the majority. In all patients a Mondini malformation was demonstrated. On the basis of this investigation it is concluded that: (1) the Mondini defect is part of Pendred's syndrome; (2) the inherited Mondini malformation is the underlying cause of the sensorineural hearing impairment; and (3) the hearing sensitivity varies greatly in these patients.

Chemical burns of the oesophagus caused by ingestion of corrosives present a difficult and potentially dangerous problem of management. Initial failure to recognize the seriousness of the injury and inexperience in the handling of the severer burn may result in consequences which are both life-threatening and life-long.

Analysis of 95 patients who had ingested caustic substances is presented. Ninty-one sustained burns of the mouth, 39 of the hypopharynx or oesophagus, and 31 had laryngeal burns. Sixty-three were children of 15 years and under. Caustic soda, especially in children, was the most common substance ingested. Severe oesophageal strictures developed early in 17 patients and in a further 21 oesophageal stricturing occurred late.

A protocol for the management of these patients is discussed.
