P-998 - Suicidal Attempt With Swallowing Foreign Body in Postpartum Catatonia

Abstract
Post partum psychosis is a condition that presents a group of mental illness with the sudden onset of psychotic symptoms following childbirth.
Catatonia has been reported in post partum psychosis but it is very rare.
Suicide is rare in postpartum psychosis, and homicide and infanticide are extremely rare.
The patient is a 25 - year - old woman without any history of psychiatric illness. During the two first weeks following the delivery she did not have any mental and physical illness symptom. She accidentally witnessed a quarrel between two stranger men and then suddenly developed agitation and crying. Shortly after the onset of illness, the patient experienced extreme fear, persecurity delusion, auditory command hallucination and visual hallucination. Her mental health began to deteriorate gradually and she fell into catatonia condition.
The patient was obliged to perform their command to relieve of their threats. One time after hearing a voice command (eat some metal objects to kill yourself), the patient performed the order promptly and swallowed a safety pin and one nut and bolt.
There are some reports of suicidal attempt by inserting needles into body or swallowing them in psychotic patients. As far as we know, there is no report about foreign body swallowing in catatonia.
This report is a rare case with postpartum catatonia who swallowed some foreign bodies as a suicidal attempt in order to get ride of her hallucinations.
