Resumen

Propósito:

Investigaciones anteriores sobre la actividad del eje hipotálamo-hipófiso-suprarrenal (HPA) en personas que han intentado suicidarse han mostrado resultados contrapuestos. El propósito del presente estudio era examinar la influencia de los trastornos de personalidad y los diagnósticos concomitantes del eje I en la prueba diagnóstica de supresión con dexametasona utilizando análisis de regresión múltiple. La muestra constaba de 184 pacientes con un intento de suicidio reciente y 42 controles sanos. Como se esperaba, las concentraciones de cortisol pre- y posdexametaxona más bajas se encontraron en los pacientes con trastornos de la personalidad del eje II, grupo B, comparado con los demás pacientes. Los resultados fueron significativos cuando se analizaron para la covarianza con los diagnósticos del eje I del DSM-III-R, la edad o el sexo. Queda por esclarecer si estas concentraciones bajas de cortisol se deben a la experiencia anterior de acontecimientos estresantes extremos o una carga duradera, o si pueden ser consecuencia de una predisposición biogenética o psicológica de interés. Es preciso examinar con más detenimiento la comorbilidad del eje I.
