This study aimed to examine the associations among functional impairments, strain and depressive symptoms for caregivers of individuals with brain tumour and other cancers. Sixty-three caregivers (71% female) of individuals with brain tumour (n = 27) and other cancers (n = 36) were recruited from community services. Participants rated their level of depressive symptoms and strain and the functional impairments of the individual with cancer. Overall, approximately 25% of the total caregiver sample was in the clinical range for depressive symptoms. For caregivers of individuals with brain tumour, strain was significantly related to depressive symptoms (r = .57) and the individual's functional impairments (r = –.58); however, level of depressive symptoms was not significantly related to functional impairments (p > .05). For caregivers of individuals with other cancer, level of depressive symptoms was significantly correlated with strain (r = .50) and their relatives' degree of cognitive, interpersonal and emotional difficulties (r = –.36–.46), but not with activities of daily living (p > .05). In a multivariate analysis involving all the caregivers, strain predicted depressive symptoms (R2 = .32, p < .001) after controlling for gender and cancer type. In addition, strain was found to significantly mediate the effects of the interpersonal and emotional difficulties of individuals with cancer on caregiver depressive symptoms. Overall, these findings highlight the need to develop and evaluate strategies for alleviating the role strain associated with supporting individuals with cancer.

Driving cessation can result in a range of negative consequences, including impacts on identity, mood, and life satisfaction. The aims of this study were to explore the needs and experiences of people who cease driving following a stroke, with the aim of informing clinical practice. Method: Using a qualitative phenomenological approach, semi-structured, audiotaped interviews were conducted to gain an understanding of the needs and experiences of 24 participants (17 male, 7 female, mean of 5.5 years post driving cessation) who had ceased driving following a stroke. Results: After constant comparative analysis, four themes emerged from the data. These were labelled ‘life without driving’, ‘key times of need’, ‘alternatives and other ways’, and ‘carer support and assistance’. Most participants found stopping driving after their stroke to be a sudden, unexpected and intense experience and raised issues, including the loss of numerous life-roles, challenges associated with arranging alternative transport, and reliance on carers and the need for more information throughout the process. Conclusion: Individuals who cease driving following a stroke have unmet needs and difficult experiences throughout the process. Key times of particular need have been identified in this study. These findings have important implications for health professionals that include providing more information and support with driving cessation and alternatives to driving.

Virtual reality (VR) is a term used to describe computer-generated virtual environments that the patient can explore and interact with in real time. In this review, the use of VR to construct ecologically valid tasks for use in neuropsychological assessment and rehabilitation is discussed with particular reference to the measurement of prospective memory (PM) abilities. PM, the ability to act upon delayed intentions, is an example of an everyday ability requiring the coordination of a different cognitive processes that is ideally suited to research and rehabilitation using VR. Studies investigating PM and related abilities in patients with brain impairments are surveyed, and it is established that there is evidence for the utility of applying VR procedures to measure prospective remembering. Future research could usefully focus on the construction of a standardised testing procedure with credible psychometric characteristics. In particular, there needs to be evidence for the criterion validity of such tests. Another potential research avenue is the evaluation of VR platforms as tools to teach memory skills and awareness of impairments. It is concluded that difficulties in adapting programs to specific clinical needs, technical issues, and the reluctance of many clinicians to use computer-based technology need to be overcome before VR is likely to be widely used to measure PM.

There is mixed evidence linking adverse outcomes after traumatic brain injury to the presence of the ε4 allele of the apolipoprotein gene (APOE). Further, there has been limited investigation of the role of APOE in populations who have sustained severe brain injuries. In this study, 100 individuals aged 16 to 65 years with a severe to extremely severe traumatic brain injury were recruited prospectively from an inpatient rehabilitation unit. APOE genotypes were determined, and demographic and clinical data were collected by blind assessors at 6 months postinjury. Sixty-nine participants who were divided into an acute (less than 12 months postinjury) and chronic (greater than 12 months) groups also completed neuropsychological assessments testing various domains of memory, attention and problem-solving at follow-up. No significant differences in injury severity, cognitive or functional outcome were found between individuals with the ε4 allele and those without at either time postinjury. This finding is consistent with other recent data that has questioned the role of APOE status as a factor in recovery from TBI.

Objective: To investigate the participation outcomes of individuals with severe acquired brain injury (ABI) who were provided with up to 12 months of intervention based on the Community Approach to Participation (CAP). Method: A case series study design was undertaken with a total of 85 participants from two private practices specialising in community-based, CAP rehabilitation for people with severe ABI. Inclusion criteria were the person had sustained an acquired brain injury, was aged over 18 years and had received up to 12 consecutive months of CAP input from one of the two practices within a three-year period between January 2004 and January 2007. Twenty-six of the 85 participants were an average of 343 days post injury (Early group) and 59 of the participants were an average of 10.2 years post injury (Late group). They were living in a range of community environments and residential facilities. Initial participants were identified for the study in January 2005. Data were gathered using the Functional Independence Measure (FIM™), Community Integration Questionnaire (CIQ), and Role Checklist (RC Part 1) at two time points: prior to intervention (baseline), and after a 12-month period or at discharge (TI). Within the intervention period, all participants received a customised community occupational therapy (OT) rehabilitation program based on the key principles of the CAP. Intervention focused on participation goals of the individual, and development of the associated skills that underpinned valued role performance. Results: Participants each received an average of 51.01 hours of CAP OT during the 12-month period of intervention. Considerable improvement was seen in functional independence, community integration, and role participation after intervention in both the Early and Late groups. A statistically significant increase in FIM™ and CIQ total scores was found for all participants from Baseline to T1. The number of roles in which participants engaged increased on average by almost one role per participant following intervention, from a mean of 3.06 to 3.99. Increased participation in volunteer, home maintainer, participant in organisations and hobbyist roles were most common. Conclusion: Increased functional independence, community integration, and participation in both the Early and Late groups over a 12 month period demonstrates the potential for improved participation outcomes for people with complex needs following ABI, even many years post injury.

It is now recognised that prospective memory (PM) failures are commonly observed in clinical groups, and have important implications for functional independence. However, well-validated measures of PM have traditionally been lacking, making this construct difficult to assess in clinical practice. Rendell and Craik's (2000) Virtual Week measure was developed as an indicator of PM, and has been shown to have good psychometric properties and be sensitive to the effects of normal and abnormal adult ageing, as well as various forms of pathology. In this article, an overview of Virtual Week's characteristics is presented, and the literature relating to use of this measure in various populations reviewed. The potential implications of the development of a new computerised version of Virtual Week for clinical practice are discussed.

Objective: To examine the care and support and participation outcomes for individuals with severe Acquired Brain Injury (ABI) provided with three consecutive years of Community Approach to Participation (CAP) intervention. Method: A case series study design was undertaken with a total of 43 participants from two private occupational therapy practices specialising in community-based, CAP rehabilitation for people with severe ABI. Data were collected at four time points: January 2004 (Baseline), January 2005 (T1), January 2006 (T2), and January 2007 (T3) and included rating on the Care and Needs Scale (CANS); number of hours of paid and gratuitous weekly support; FIM™ and Community Integration Questionnaire scores; and number of current life roles (as recorded on Part 1 of the Role Checklist). Participants were an average of 6.73 years post injury and were living in a range of community environments and residential facilities. Results: There was considerable change in participants' CANS rating across the intervention period, with statistically significant decreases occurring from Baseline to T1 and T1 to T2. Ninety-five per cent of participants received some level of weekly paid support throughout the intervention period, with the amount varying considerably. Paid care decreased by an average of 10.78 hours per week from 85.85 hours per week at Baseline to 75.06 at T3. Although not statistically significant, this equated to an average cost saving of AU$324.84 per participant per week by the end of CAP intervention. There was an overall mean reduction of 4.96 hours of gratuitous care per week per participant over the intervention period, which was statistically significant. Increases in functional independence, community integration, and role participation were also found. On the FIM™, statistically significant improvement occurred between Baseline and T1 and on the CIQ between Baseline and T1 and T1 and T2. Significant increase on the Role Checklist occurred between Baseline and T1 and between T2 and T3. The number of life roles in which participants engaged increased by an average of 1.65 roles by the end of the intervention period. A reduction in care and support needs (measured by the CANS) was significantly associated with: (1) a reduction in total care hours, (2) and an increase in functional independence on the FIM™, and (3) an increase in community integration on the CIQ. Conclusion: With a long term approach and contextualised intervention targeted at skill development in the areas that underpin personally valued participation, increased role performance and community integration can be achieved by people with severe ABI, even many years post injury. This increased capacity has been found to be associated with a reduction in care and support needs, including paid and gratuitous care hours, over time.

Hypoxic brain injury is an under-recognised consequence of heroin overdose. This article documents the disability experienced by 10 people with hypoxic brain injuries following heroin overdose who presented in the 2-year period July 1997 to June 1999 at the Royal Talbot Rehabilitation Centre, a brain injury rehabilitation facility in Melbourne, Australia. Medical histories of these clients were reviewed and follow-up interviews were conducted between 2 and 4 years postinjury. Measures included the Functional Independence Measure (FIM) and the Community Integration Questionnaire (CIQ). The results showed that there were significant and enduring consequences for most of the 10 participants. At follow-up two participants had died from subsequent overdose, three required support to live in the community and one lived in an aged care facility. Only one participant was able to return to work. Cognitive problems were present in most of the group and problem solving and social interaction difficulties were the most commonly recorded problems. Three cases are described in detail to highlight some of the significant issues identified in this sample. Implications for the rehabilitation of people with brain injury following heroin overdose are discussed.

As the field of prospective memory (ProM) research has expanded, recognition of the need to make a clinical diagnosis of ProM difficulties has also increased. In addition, there is increasing interest in understanding in more detail the ways in which ProM deficits differ in populations with different underlying neurological dysfunctions. While questionnaires, naturalistic studies and laboratory studies have all been invaluable in furthering our understanding of ProM, until recently there has not been a standardised clinical measure. The Memory for Intentions Screening Test (MIST) was designed to be a relatively brief clinical measure of ProM in clinical populations. The MIST allows for comparisons of performance with event-based and time-based cues. The MIST also has some items with a short (2-minute) and some with a long (15-minute) delay period. Finally, the MIST contains items that require a verbal response as well as items that require an action response. The MIST has now been used in studies of normal ageing as well as a range of clinical populations, including individuals with traumatic brain injury, individuals with HIV infection and individuals with schizophrenia. The psychometric properties of the MIST have been found to be acceptable. Data from each of the clinical populations that have been administered the MIST demonstrate good specificity and sensitivity of the measure as well as the ability to begin to make comparisons about different patterns of performance between disease types.

Three single participant studies examined effects of in-home parent-implemented Positive Behaviour Support (PBS) interventions on behaviour of individuals with brain injuries. Parental and independent observations of morning routine behaviour or problem behaviour (viz., noncompliance during meals or morning routines, yelling, kicking, swearing, and so on) were taken during baselines and PBS interventions, which consisted of person-centred functional assessments of behaviour, emphasis on changing behavioural antecedents, parent participation in assessing, treating, and evaluating behaviour change, and in-home parent coaching and support. Results show decreased problem behaviour or improved morning routine behaviour, which corroborate and add to the few studies that applied similar parent-implemented interventions. Future comparative studies between PBS and out-patient interventions are indicated.

Many citizens misunderstand the actions of persons with brain injury, and these misunderstandings hamper rehabilitation. A specific misunderstanding is where people misattribute behaviours resulting from brain injury to the injured person's personality or life stage (e.g., adolescence). The present study examined if this pattern is explained by the Abnormal Conditions Focus model of attribution, which claims that attributions for behaviours reflect whether the behaviours are normal for the person (Consistency) and the culture (Consensus). Scenarios described an adolescent with brain injury performing four undesirable behaviours and then indicated whether the behaviour was normal or not normal for the person prior to injury and normal or not normal for that person's culture. For each of the four behaviours, students (n = 136) rated three attributions: brain injury, personality and adolescent norms. Participants attributed the behaviours more to brain injury and less to personality when the behaviours were not normal for that person than when they were normal for the person. They attributed the behaviours more to adolescence when the behaviours were normal for the person's culture than when they were not normal for the culture. Rehabilitation implications include assessing individuals' premorbid behaviour to enable citizens and less experienced professionals to make more accurate attributions for behaviours and target treatment more effectively.

This study aimed to assess planning ability in adults with traumatic brain injury (TBI) using a 4-disk version of the Tower of London (TOL). Thirty three individuals with TBI were compared with equivalent numbers of matched controls. Overall, the TOL4 was shown to be sensitive to the effects of brain injury, with the TBI group performing significantly more poorly on this version of the planning test than the matched controls. More specifically, group differences were found to be related to the complexity of the planning problems, particularly among a TBI subgroup with localised prefrontal damage. Results of the study provide support for the adverse effects of TBI on planning ability, and the important role of the prefrontal cortex in planning.

This article reports on the design of a three-arm, nonrandomised controlled trial of interventions targeting social communication skills following traumatic brain injury (TBI) in adult participants. People with severe TBI were allocated to one of the three groups: the TBI group, where only the person with TBI was trained, the JOINT group where both the everyday communication partner (ECP) and the person with TBI were trained together, and a delayed treatment control condition. The trial is comparing whether including everyday communication partners in the training process provide additional benefit when compared to training the person with TBI alone; and additionally, whether training the person with TBI alone is more effective than no training. A range of primary and secondary outcome measures will be used to evaluate outcomes. Publishing the protocol prior to the results of the trial being available has several important benefits (Godlee, 2001). The original hypotheses and intentions of the research are made explicit to ensure that the process of conducting this clinical trial is transparent to readers, and so that comments may be made before results are finalised. It provides the opportunity to outline a detailed description of this intervention and methodology, or to acknowledge changes to methodology, which may assist with eventual clinical application of the intervention. This article also informs the research community of the work that is underway to promote opportunities for collaboration and reduce unnecessary duplication of research. The protocol for this trial has previously been registered on Current Controlled Trials (http://www.controlled-trials.com/ISRCTN57815281).

Objective: To expand upon the existing psychometric properties of the Comprehensive Assessment of Prospective Memory (CAPM) for use with adults with traumatic brain injury by examining concurrent and criterion validity. Method: Participants were 45 adults with a traumatic brain injury. Participants and their relatives completed Section A of the CAPM and a measure of psychosocial integration. Participants were also administered two neuropsychological tests of prospective memory, the Cambridge Prospective Memory Test (CAM-PROMPT) and the Memory Intentions Screening Test (MIST). Concurrent validity was measured by comparing scores on the CAPM with scores on the CAM-PROMPT and MIST. Criterion validity was examined by correlating CAPM scores with level of psychosocial integration. Results: Participant self-reports on the CAPM were not significantly correlated with the CAM-PROMPT or MIST, but were significantly correlated with level of psychosocial integration. Relative reports on the CAPM were correlated significantly with total score on the MIST and CAM-PROMPT and level of psychological integration. Conclusions: The findings indicate that the concurrent validity of the self-report version of CAPM is low suggesting that self-reports alone do not provide an objective measure for assessing prospective memory function. The relative report version however, demonstrated reasonable concurrent and criterion validity, suggesting that the relative report version of the Section A of the CAPM is a useful means of evaluating frequency of prospective memory failure in adults with traumatic brain injury.

The current study aimed to investigate prospective memory in traumatic brain injury by using a laboratory-based but naturalistic style assessment task. The objective was to determine if performance differed when the prospective memory targets were self-generated or experimenter-generated (a generation effect). Sixteen people who had sustained a traumatic brain injury and had received rehabilitation were compared to 16 healthy age-matched control participants on a naturalistic virtual measure of shopping and prospective memory. Results demonstrated that the traumatic brain group was less accurate in overall prospective remembering performance than the control group but there was a lack of difference between the self- and experimenter-generated prospective memory targets for both participant groups. Both retrospective memory and executive attention of working memory were associated with prospective memory performance in the naturalistic task. The findings of the study suggest that the generation effect commonly observed in retrospective memory performance may not impact performance in prospective memory.

Objective: To explore the availability and perceived adequacy of service support for younger people with ABI and their families in Queensland during the transition phase from the perspective of service providers. Research Design: A qualitative research design utilising survey-based open-ended questioning. Methods and Procedures: Participants were service provider organisations in Queensland for younger people with aquired brain injury (ABI). A two-part survey was developed for the purpose of the study. The first part of the survey obtained general information regarding client base and service characteristics for each organisation, while the second part was designed to identify gaps in, and barriers and facilitators to service provision. Snowballing recruitment was used with the initial seed point an existing Brain Injury Network. Results: A response rate of 20% was obtained with a total of 12 surveys returned representing seven service providers. Services currently available included rehabilitation, consultancy, education and training, and practical and emotional supports. Existing gaps in, and barriers to service provision were clustered under three key themes: service infrastructure, transition support, and transition context. Some of the facilitators to service provision identified included interagency linkages and financial compensation for injury. Conclusions: Gaps in, and barriers to service provision during the transition phase were identified that could be utilised to develop models of service delivery to improve short- and long-term outcomes for younger people with ABI.

In this study, for the first time, prospective memory was investigated in 11 school-aged children with autism spectrum disorders and 11 matched neurotypical controls. A computerised time-based prospective memory task was embedded in a visuospatial working memory test and required participants to remember to respond to certain target times. Controls had significantly more correct prospective memory responses than the autism spectrum group. Moreover, controls checked the time more often and increased time-monitoring more steeply as the target times approached. These differences in time-checking may suggest that prospective memory in autism spectrum disorders is affected by reduced self-initiated processing as indicated by reduced task monitoring.
