Summary

This chapter reviews the evidence base for combined medication and psychotherapy treatment of depression. As a starting treatment for a patient who is not currently receiving treatment for depression, combined treatment may be best used for patients with chronic depressive episodes complicated by co-morbid personality disorder, noncompliance, or psychosocial problems. A patient may also begin a combined treatment when medication is prescribed for the onset of a significant depressive episode in the context of an ongoing psychotherapy. Alternatively, a patient who has not fully responded to one or more medication trials may begin psychotherapy, thereby beginning a combined treatment by a different route. Irrespective of the path by which the patient is prescribed combined treatment, the prescribing clinician must recognize that the execution of this treatment plan presents unique and complex challenges, which are discussed in the chapter.
