Case 34 - Clicking all night

Summary

This chapter describes the case of a 45-year-old female homemaker who was admitted at the sleep clinic with symptoms of daytime fatigue, sleepiness and chronic headaches. It presents the clinical history, examination, follow-up, treatment, diagnosis, and the results of the procedures performed on the patient. A facial and oral examination revealed significant tenderness to palpation of the jaw muscles and the temporalis muscles. Nocturnal polysomnography (PSG), a basic metabolic work-up and a thyroid function test were carried out. Polysomnographic recording showed frequent episodes of sleep bruxism in the EMG channel that occurred during NREM light sleep. The principal diagnosis was bruxism. This case illustrates the complexity of symptoms in patients with bruxism such that they can be multiple and inter-related and therefore must be addressed with a comprehensive and multidisciplinary approach.
