As Canadian health policy-makers struggle to contain costs while maintaining quality, patient education is promoted as making patients more effective producers of health and consumers of health care. Assessment of existing psychoeducational interventions has concentrated on knowledge enhancement, behaviour modification, and health status while economic outcomes have remained unexplored. Given that such programs are likely to be effective, but costly, economic appraisals are necessary to convince cost-conscious policy-makers. This manuscript describes a cost-effectiveness evaluation of the Arthritis Self-Management Program (ASMP) that I and my collaborators are conducting by randomizing participants to the ASMP as an adjunct to usual medical care versus usual medical care. Direct and indirect costs are evaluated through self-reporting of health services utilization and diminished productivity and effectiveness through a visual analogue scale and the SF-36. To influence health policy, the results must be disseminated to public health authorities, private insurers, and patient and health professional organizations, thereby encouraging funding, increasing awareness, and promoting participation.

This mixed-methods complementarity study explored family members’, friends’, and health care providers’ perspectives of acceptability of group leisure activities as an intervention for loneliness experienced by older adults living with dementia. A sample of 25 family members, friends, and health care providers of people living with dementia in ON rated the acceptability of group leisure activities (adapted Treatment Perception and Preference questionnaire) and discussed their ratings in an interview. Quantitative (descriptive statistics) and qualitative (conventional content analysis) results were integrated to understand acceptability. Participants viewed group leisure activities as effective, logical, and suitable for use with people living with dementia. Participants described the need for flexible programs, careful facilitation, and attention to activity selection. Group leisure activities were seen as low risk, but stigmas related to dementia could prevent participation. The findings inform the design of acceptable group leisure activities, promoting their use to address loneliness in people living with dementia.

This study aims to assess changes in long-term care (LTC) residents’ quality of life (QoL) before and during the COVID-19 pandemic. A pre-test post-test study of 49 QoL measures, across four dimensions from the interRAI self-reported QoL survey, was conducted. Secondary data from 2019 (n = 116) and 2020 (n = 128) were analysed to assess the change in QoL. A significant decline in 12 measures was observed, indicating a change in QoL of LTC residents during the pandemic. Social life was the dimension mostly affected with residents reporting less opportunities to spend time with like-minded residents, explore new skills and interests, participate in meaningful religious activities, and have enjoyable things to do in the evenings. Several measures of personal control, staff responsiveness and care, and safety also demonstrated a significant change. The results can inform future strategies for pandemic and outbreak preparedness. Balancing the safety of residents with attention to their QoL should be a priority moving forward.

Dementia is a growing concern in Canada, affecting peoples’ health and raising the cost of care. Between June and October 2019, we conducted an environmental scan to identify primary care models, strategies, and resources for dementia care from 11 pre-selected countries and assess their impact on quality-of-life measures. Search strategies included a rapid scoping review, grey literature search, and discussions with stakeholders. Eighteen primary care-based models of dementia care were identified. Common factors include team-based care, centralized care/case coordination, individual treatment plans, a stepped-care approach, and support for care partners. Five provinces had released a dementia strategy. Evidence of positive outcomes supported primary care-based models for dementia care, although only one model demonstrated evidence of impact on quality of life. Although these findings are encouraging, further research is needed to identify primary care-based models of dementia care that demonstrably improve quality of life for people living with dementia and their care partners.

Research has shown that long-term care (LTC) volunteers play important roles in enhancing the quality of life (QoL) of older LTC residents, often through providing unique forms of relational care. Guided by Kane’s QoL domains, we used a modified objective hermeneutics method to analyze how unique volunteer roles are represented and supported in provincial policies in Alberta, British Columbia, Ontario, and Nova Scotia. We found that policies define volunteer roles narrowly, which may limit residents’ QoL. This happens through (1) omitting volunteers from most regulatory policy, (2) likening volunteers to supplementary staff rather than to caregivers with unique roles, and (3) overemphasizing residents’ safety, security, and order. We offer insights into promising provincial policy directions for LTC volunteers, yet we argue that further regulating volunteers may be an inadequate or ill-suited approach to addressing the cultural, social, and structural changes required for volunteers to enhance LTC residents’ QoL effectively.

A cross-sectional analysis was conducted using data from a prospective cohort study to investigate whether frailty is associated with pain intensity, disability caused by low back pain (LBP), and quality of life in an older population with acute non-specific LBP. Six hundred and two individuals with a mean age of 67.6 (standard deviation [SD] 7.0) years were included in the analysis. In relation to frailty status, 21.3 per cent of the sample were classified as robust, 59.2 per cent were classified as pre-frail, and 19.5 per cent were classified as frail. In the unadjusted analysis, pre-frail and frail groups showed significantly higher pain and disability scores than the robust group. Moreover, the same two groups exhibited lower scores in both physical and mental domains of quality of life than the robust group. After adjusting for socio-demographic and clinical variables, disability scores and the physical component of quality of life were significantly associated with frailty. In older adults with acute LBP, frailty is associated with more disability and worse scores in the physical component of quality of life.

La qualité de vie des patients diabétiques est déterminée par un grand nombre de facteurs de type biologique, social et psychologique, qui interagissent ensemble. Le modèle biopsychosocial (BPS), élaboré par Engel dans les années 1980, est basé sur cette notion holistique et interactive. Plus récemment, cette conception a été opérationnalisée sous la forme d’un questionnaire hétéro-évaluatif qui comporte 20 items (outil INTERMED). Même si la notion de conception holistique du bien-être de l’individu diabétique est reconnue dans la pratique clinique, notamment hospitalière, peu de travaux s’intéressant, de manière expérimentale, au bénéfice de ce concept sont disponibles dans la littérature. L’étude présentée a inclus 28 patients diabétiques, hospitalisés en service d’endocrinologie. Chaque participant a donné son consentement écrit, s’est entretenu avec une investigatrice, et a rempli une échelle mesurant la qualité de vie (SF-36). 43 % d’entre eux présentent un haut niveau de complexité BPS, ce qui indique un besoin de prise en charge multidisciplinaire. À travers des analyses de corrélation, il a été démontré que chaque composante de la complexité est associée de manière négative, et statistiquement significative, à la qualité de vie perçue. De plus, la complexité globale permet de mieux expliquer la qualité de vie que la simple prise en compte de n’importe quel facteur local. En pratique, ces résultats soulignent la nécessité de prendre en charge les éléments psychosociaux auprès des patients afin d’assurer au mieux le vécu subjectif de leur état de santé. Des services, tels que la psychiatrie de liaison, deviennent ainsi essentiels dans l’offre de soin holistique et multidisciplinaire.

Les antipsychotiques de première et deuxième génération apportent un confort important de par la régularisation et la stabilisation des symptômes cliniques chez les personnes atteintes de schizophrénie et autres psychoses. Ils s’accompagnent d’effets secondaires nombreux parmi lesquels l’hyperprolactinémie qui n’est pas responsable d’une morbidité importante mais peut induire un inconfort pour le patient et responsable d’une demande d’interruption du traitement antipsychotique. Certains antipsychotiques induisent plus d’hyperprolactinémie (amisulpride, risperidone et paliperidone) que d’autres (aripiprazole et quetiapine) [1]. Cet effet secondaire de classe est corrélé à la diffusion et à l’affinité du médicament pour les récepteurs dopaminergiques D2 pituitaires.

L’objectif de cette étude est d’évaluer le niveau d’hyperprolactinémie induit par les antipsychotiques de première et deuxième génération et son éventuel retentissement sur le confort de vie des patients. La population étudiée comprend des patients hospitalisés et suivis en CMP, stabilisés. Ces patients ont le diagnostic de schizophrénie ou de troubles schizo-affectifs.

Nous allons effectuer le dosage de prolactinémie chez 50 patients (25 hommes, 25 femmes) traités par antipsychotiques de première et deuxième génération, associé à un questionnaire pour évaluer leur qualité de vie. Les résultats seront analysés en fonction des données de la littérature. Il s’agit d’une étude observationnelle puisque le dosage de la prolactinémie est recommandé dans la surveillance biologique et clinique de la prescription d’antipsychotique.

Dans le cadre du financement d’un Programme hospitalier de recherche infirmière et paramédicale, une recherche infirmière multicentrique (CH Saint-Jean de Dieu, CH Le Vinatier) est engagée en conjuguant psychoéducation et remédiation cognitive. L’utilisation de la remédiation cognitive chez des sujets souffrant de schizophrénie a fait l’objet de nombreuses études contrôlées attestant de son efficacité dans le domaine cognitif. La psychoéducation du patient commence à être appliquée en pratique chez les patients souffrant de troubles mentaux avec des résultats probants. L’étude se déroule sur trois ans, avec un recrutement de 80 patients répondant au diagnostic de schizophrénie randomisés en deux groupes appariés. Un premier groupe de 40 patients bénéficiera d’une prise en charge groupale de psychoéducation (TIPP conçu par le Dr Philippe Conus), puis d’une prise en charge individuelle de remédiation cognitive (RECOS conçu par Pascal Vianin). Le groupe contrôle de 40 patients bénéficiera d’une prise en charge individuelle de remédiation cognitive. Ce groupe bénéficiera, hors étude, d’une prise en charge groupale de psychoéducation. Les deux groupes seront comparés sur l’évolution de leurs performances pré- et post-prise en charge à l’aide d’une batterie neuropsychologique, d’une échelle d’insight et d’une échelle mesurant la qualité de vie. Cette évaluation sera renouvelée 6 mois après la fin du programme. Nous faisons l’hypothèse d’une potentialisation de l’efficacité de la remédiation cognitive par la psychoéducation placée en amont. Les bénéfices attendus seront une amélioration des fonctions neurocognitives, de l’insight et de la qualité de vie.

Orienter les soins et les services vers le rétablissement réalise un nouvel enjeu qui bouleverse les schémas de soins habituellement centrés sur l’institution et la maladie. Le mouvement de la réhabilitation a fait connaître le concept de rétablissement qui traduit l’expression des choix des usagers. Ce concept de rétablissement illustre des données connues mais peu valorisées : le pronostic des maladies mentales est favorable pour la moitié des personnes atteintes et dépend aussi de facteurs subjectifs et sociaux. Les objectifs jugés primordiaux par les soignants doivent alors être confrontés aux objectifs personnels des patients et usagers dans une nouvelle alliance thérapeutique, dans une éthique tenant compte de la vulnérabilité et de l’autonomie des personnes handicapées psychiques. Les soins orientés vers le rétablissement développeront des stratégies privilégiant les soins ambulatoires et les interventions écologiques, la qualité de vie immédiate, la préservation du pronostic fonctionnel et la déstigmatisation.

L’étude BDmiE a consisté en une enquête transversale non interventionnelle, en ligne, auprès d’étudiants français en études supérieures. Elle comportait trois objectifs :

– étudier le comportement de binge drinking et son impact sur la qualité de vie ;

– valider et adapter un outil de mesure de la qualité de vie dans les trouble liés à l’usage d’alcool, AQoLS, à la population des binge drinkers ;

– étudier la relation entre identité de « fêtard », binge drinking et impact sur la qualité de vie.

L’apathie est définie comme un manque de motivation qui impacte les dimensions cognitives, émotionnelles et comportementales du quotidien. Les comportements orientés vers un but (c.-à-d., l’obtention de toute forme de satisfaction élémentaire (nourriture par exemple) et la persévération à l’effort), les cognitions (prise d’initiative, stratégies d’adaptation telles que la flexibilité mentale et l’inhibition des conduites inadéquates) et l’état émotionnel associé (activation du système d’éveil et d’excitation) sont réduits voire même absents dans les cas extrêmes. La sensation de vacuité ressentie par l’entourage peut prendre le masque du rejet de la part du proche aidant/compagnon, d’un désinvestissement de la part du médecin ou bien même de l’oubli dans les institutions. L’apathie étant associée à l’assombrissement du pronostic et à une qualité de vie altérée dans différentes maladies neuropsychiatriques, une approche transnosographique apporterait-elle des évidences permettant de suspecter un déficit commun ? Ce symposium se déroulera en trois parties : au cours de la première, le Pr Dujardin présentera les données générales autour du concept d’apathie (notamment les définitions et les données épidémiologiques) puis seront traités de façon plus spécifiques (physiopathologies, outils d’évaluations et stratégies thérapeutiques) l’apathie dans les maladies psychiatriques dont la dépression et la schizophrénie et dans les maladies dégénératives dont les maladies d’Alzheimer et de Parkinson. L’objectif est de pouvoir soumettre aux auditeurs un modèle intégratif correspondant aux troubles évoqués.

Plus de la moitié des jeunes de 17 ans en France déclaraient avoir bu au moins 5 verres en une occasion au cours du mois écoulé en 2011. Les étudiants s’engagent plus facilement dans des modes de consommation d’alcool à risque comme le binge drinking, que les jeunes du même âge non étudiants. Ce mode de consommation s’installe dans les habitudes des nouvelles générations d’étudiants (Spilka, 2012). Si de nombreuses données sont disponibles sur les conséquences aiguës de ces alcoolisations (hépatites aiguës, grossesses non désirées, violences, accidents de la route…), un intérêt croissant se porte sur les conséquences à moyen et long terme sur la qualité de vie, comme sur le fonctionnement neural et neurocognitif de ces pratiques. Ainsi, l’exploration de la qualité de vie liée à l’alcool permet de mieux comprendre le basculement d’une pratique sociétalement valorisée à un état pathologique. En complément, alors qu’il a été montré que le cerveau adolescent paraît particulièrement vulnérable à la toxicité de l’alcool , la pratique du binge drinking a été impliquée dans des altérations cognitives, notamment au niveau du contrôle inhibiteur préfrontal. La mise en évidence de ces altérations pourraient ouvrir une nouvelle voie thérapeutique. Des perspectives récentes proposent de confronter les altérations neurobiologiques aux difficultés émotionnelles retrouvées elles aussi dans le trouble d’usage d’alcool . Ces lectures complémentaires du phénomène de binge drinking permettent ainsi d’appréhender de façon innovante la transition d’un comportement socialement intégré vers le trouble d’usage d’alcool, en identifiant des mécanismes physiopathologiques communs et des sous-groupes plus à risque.

This investigation derives its impetus from public health concerns around detecting, mitigating, and preventing the deleterious effects that alcohol use can cause particularly in advanced age. We aim to complement gerontological research by exploring the interactive effects of quality of life and related factors on alcohol use outcomes assessed by the Drinking Problem Index. The study is based on cross-sectional data collected from questionnaires mailed to a randomly drawn sample of 6,000 Norwegian adults aged 62 and older (participation rate: 32%). According to the Chi-square Automatic Interaction Detection (CHAID) analysis, constellations of interactive factors emerged differently for women and men, and, between non-problem and problem drinkers. For women, drinking outcomes were related to intra-psychic functioning, and for men, to physical health and social situation. An ongoing quality-of-life assessment may be very important in the comprehensive assistance provided to those older people who are vulnerable to undergoing alcohol-related harms.

The global population including Canada’s is aging, which demands planning for housing that will support older adults’ quality of life. This mixed-method study is the first Canadian study to examine the impact of cohousing on older adults’ quality of life and involved 23 participants. The older adults rated their quality of life very high, especially in the environmental, physical, and psychological domains of the World Health Organization Quality of Life (WHOQOL_BREF) survey; quality of life in the social domain was rated low, which was surprising in light of the focus group data findings. Four themes of “belonging in a community”, “life in the community”, “changes associated with aging,” and “aging in place” emerged from the qualitative data to explain factors that influence older adults’ quality of life. This research provides foundational, strong evidence that seniors’ cohousing is an innovative housing solution that can support older adults’ quality of life.

A community-based program is increasingly recognized as promoting health and active social participation in one’s life, yet information is lacking about the use and impact of such programs among immigrant visible minority seniors. This mixed-method research evaluated the impact of a cultural community program for Korean immigrant seniors by examining participants’ health-related quality of life (HR-QOL) benefits and overall well-being. In this study, 79 participants completed the SF-36v2 questionnaire twice to assess the impact of Canada Enoch Senior’s College (CESC) program on their HR-QOL and well-being. Statistically significant improvement in physical and mental health domains was observed: bodily pain and role limitations due to emotional problems. Qualitative data from participants’ interviews supported the survey findings with positive contributions in health and social arenas of seniors’ lives. These results suggest that the CESC program contributes to quality of life and well-being of Korean senior participants and supports similar community-based cultural programs.

We used a web-based mixed methods survey (HowsYourHealth – Frail) to explore the health of frail older (78% age 80 or older) adults enrolled in a home-based primary care program in Vancouver, Canada. Sixty per cent of eligible respondents participated, representing over one quarter (92/350, 26.2%) of all individuals receiving the service. Despite high levels of co-morbidity and functional dependence, 50 per cent rated their health as good, very good, or excellent. Adjusted odds ratios for positive self-rated health were 7.50, 95 per cent CI [1.09, 51.81] and 4.85, 95 per cent CI [1.02, 22.95] for absence of bothersome symptoms and being able to talk to family or friends respectively. Narrative responses to questions about end of life and living with illness are also described. Results suggest that greater focus on symptom management, and supporting social contact, may improve frail seniors’ health.

This study explored qualitatively how residents of long-term care (LTC) facilities feel about and adapt to the care they receive. We interviewed and observed a purposeful selection of elderly residents in seven facilities purporting to provide person-centred care. Interpretative descriptions from 43 personal interviews with 23 participants answered the question: How do residents perceive the care rendered in LTC facilities purporting to offer person-centred care? Three themes emerged: (1) the caring environment; (2) preservation of dignity; and (3) maintenance of personal autonomy. Participants were sympathetic to the nursing staff’s workload, but felt distant from the staff. Participants gave examples of poor care and lack of empathy, human indignities, and violations of personal autonomy caused by institutional policies they felt inhibited their ability to receive care based on their preferences. Overall, they challenged the claims of person-centred care, but adapted to cope with an environment that threatened their dignity and autonomy.
