Introduction:
There are only two medical practitioners who are genuinely generalists. The confirmed generalist is the one who has been trained and credentialed to perform prenatal care, deliver babies and perform c-sections, take care of young children, perform simple surgeries, perform palliative care, and hold a patient's hand and hug the family after the death of a loved one. In the human world, that medical provider is a family practice physician. In the animal world, that provider is the veterinarian, who cares for all species that are not human and covers their medical needs, from preventive care to surgical needs, dentistry to dermatology, internal medicine to cardiology. As such, veterinarians are indeed generalist medical providers. In disasters, veterinarians are often pushed aside by their human medical counterparts. In doing so, there are a lot of learning opportunities missed on both sides.
Method:A literature review was conducted.
Results:n/a
Conclusion:In learning the skills that are unique and overlapping, physicians and veterinarians will be better able to respond to disasters anywhere and will be positioned to help the displaced and injured get better so they may return to normalcy as quickly as possible. It is time that disaster teams and planning sessions stop being siloed and think about how medical generalists can team up and work together.
Geomorphological, chronological, and paleoenvironmental context of the Mousterian site at Roca San Miguel (Arén, Huesca, Spain) from the penultimate to the last glacial cycle

The Roca San Miguel (RSM) archaeological site was occupied during Mousterian times. Here we present a geoarchaeological and paleoenvironmental reconstruction of the site. Five stratigraphic units (A to E) formed by different archaeological levels are identified. Three optically stimulated luminescence (OSL) ages show that Unit A dates to between 169.6 ± 9.1 and 151.9 ± 11.1 ka, during the penultimate glacial period (PGP), and contains numerous signs of recurring hearths. Unit B is unexcavated. Unit C dates to between 118.9 ± 11.5 and 103.4 ± 6.9 ka (late Eemian–marine isotope stage (MIS) 5d) and shows an abundance of lithic remains as well as some faunal elements. Unit C is covered by Unit D, which incorporates materials moved downslope, and is dated at 81.2 ± 4.7 ka. These OSL ages concur with U/Th ages (129.3 ± 1.5 and 123.6 ± 0.6 ka) derived from a flowstone covered by both -C and D- post-flowstone units. Finally, Unit E covers the archaeological site, which was partially eroded during MIS2. The robust and well-constrained chronology of the RSM site and surroundings enables the establishment of its evolutionary model from the PGP to the last glacial cycle. The RSM site is the oldest Neanderthal occupation accurately dated in the Pre-Pyrenean region.
Relationship Between O6-methylguanine-DNA Methyltransferase Levels and Clinical Response Induced by Chloroethylnitrosourea Therapy in Glioma Patients

Background:
Adjuvant nitrosourea chemotherapy fails to prolong survival significantly as many tumors demonstrate resistance to these drugs. It has been documented in cell lines that O6-methylguanine DNA methyltransferase (MGMT) plays an important role in chloroethylnitrosourea (CENU) drug resistance.
Methods:We evaluated MGMT expression in 22 glioma specimens by using an immunofluorescence assay and compared the results with clinical responses of the patients to CENU-based chemotherapy.
Results:Eight tumor samples had no detectable MGMT, whereas other samples had from 9,989 to 982,401 molecules/nucleus. In one group (12 patients), the tumor decreased in size or was stable (effective group), whereas in the other group (10 patients), the tumor demonstrated continuous growth during chemotherapy (progressive group). The Mer- patients (MGMT < 60,000 molecules/nucleus) appeared to have more chance of stable disease or response to CENU therapy than the Mer+ patients (MGMT > 60,000 molecules/nucleus) (X2 = 4.791, p = 0.0286). In patients with glioblastomas multiforme (GBMs), the median time to progression (TTP) of Mer+ patient was shorter than that of Mer- patient (t = 2.04, p = 0.049). As a corollary, the MGMT levels were significantly higher in GBM tumors from the progressive group than those from the effective group (t = 2.26, p = 0.029). However, there was no significant correlation between MGMT levels and either the survival time (r = 0.04, p = 0.8595) or TTP (r = 0.107, p = 0.6444).
Conclusion:This study suggests that being MGMT positive is indicative of a more aggressive disease that progresses more rapidly with CENU therapy. However, MGMT negative tumors are not always sensitive to CENU agents, suggesting that other factors are also important.

Summary
In December 2010, a major storm erupted in Saturn’s northern hemisphere near 37° planetographic latitude. This rather surprising event, occurring at an unexpected latitude and time, is the sixth “Great White Spot” (GWS) storm observed over the last century and a half. Such GWS events are extraordinary, planetary-scale atmospheric phenomena that dramatically change the typically bland appearance of the planet. Occurring while the Cassini mission was on orbit at Saturn, the Great Storm of 2010–2011 was well suited for intense scrutiny by the suite of sophisticated instruments onboard the Cassini spacecraft as well by modern instrumentation on ground-based telescopes and onboard the Hubble Space Telescope. This GWS erupted on 5 December close to the peak of a westward jet and generated a major dynamical disturbance that affected the whole latitude band from 25° to 48°N. At the upper cloud level, following the rapid growth of the bright outbreak spot, a blunt aerodynamic-shaped head formed due to interaction of the spot with the westward zonal jet, with the winds reaching velocities of 160 m s−1 along the periphery of the arc. Eastward of the head, the disturbance progressed in the following months forming a turbulent wake or tail with growing vortices, one of them a major enduring anticyclone (called AV) with a size of ~11,000 km. Lightning events were prominent and detected as outbursts and flashes at the head and along the disturbance at both optical and radio wavelengths. The activity of the head ceased after about seven months when AV reached it, leaving the cloud structure and ambient winds perturbed. The tops of the optically dense clouds of the head reached the 300-mbar altitude level (~50 km below tropopause), where a mixture of ices was detected, including (1) a component of water ice lofted over 200 km altitude from its 10-bar condensation level, (2) ammonia ice as the predominant component and (3) a component that might be ammonium hydrogen sulfide ice. The energetics of the frequency and power of lightning, as well as the estimated power generated by the latent heat released in the water-based convection to create the observed dynamical three-dimensional flows, both indicate that the power released for much of the 7-month lifetime of the storm (~1017 Watts) was a significant fraction of Saturn’s total radiated power (~2.2 1017 W). A post-storm depletion of ammonia vapour was also measured in the upper troposphere. The effects of the storm propagated into the stratosphere, forming two warm air masses at the ~0.5- to 5-mbar pressure level altitude that later merged into a so-called “beacon” because of its 80 K temperature excess relative to its surroundings. Related to the stratospheric disturbance, hydrocarbon composition excesses were found, in particular for ethylene (C2H4), in the high stratosphere at the ~0.1- to 0.5-mbar altitude level. Numerical models of the storm dynamics explain the major observed features that essentially result from two processes: (1) a huge and sustained, moist, convective storm at the water clouds (altitude level 10–12 bar, or ~250–275 km below the tropopause) and (2) the interaction of the updraft columns with the ambient winds that generates the turbulent wake consisting of vortices and waves. Model simulations of the GWS require a low vertical shear of the zonal winds and low static stability across the weather layer where the disturbance develops. Its upward propagation into the stratosphere involves Rossby waves and their breaking and energy deposition to form the beacon and induce chemical changes.
The decades-long interval between storms is probably related to the insolation cycle and the long radiative time constant of Saturn’s atmosphere, and several theories for temporarily storing energy have been proposed.
The Resuscitative and Pharmacokinetic Effects of Humeral Intraosseous Vasopressin in a Swine Model of Ventricular Fibrillation

Introduction
The American Heart Association (AHA; Dallas, Texas USA) and European Resuscitation Council (Niel, Belgium) cardiac arrest (CA) guidelines recommend the intraosseous (IO) route when intravenous (IV) access cannot be obtained. Vasopressin has been used as an alternative to epinephrine to treat ventricular fibrillation (VF).
Hypothesis/ProblemLimited data exist on the pharmacokinetics and resuscitative effects of vasopressin administered by the humeral IO (HIO) route for treatment of VF. The purpose of this study was to evaluate the effects of HIO and IV vasopressin, on the occurrence, odds, and time of return of spontaneous circulation (ROSC) and pharmacokinetic measures in a swine model of VF.
MethodsTwenty-seven Yorkshire-cross swine (60 to 80 kg) were assigned randomly to three groups: HIO (n=9), IV (n=9), and a control group (n=9). Ventricular fibrillation was induced and untreated for two minutes. Chest compressions began at two minutes post-arrest and vasopressin (40 U) administered at four minutes post-arrest. Serial blood specimens were collected for four minutes, then the swine were resuscitated until ROSC or 29 post-arrest minutes elapsed.
ResultsFisher’s Exact test determined ROSC was significantly higher in the HIO 5/7 (71.5%) and IV 8/11 (72.7%) groups compared to the control 0/9 (0.0%; P=.001). Odds ratios of ROSC indicated no significant difference between the treatment groups (P=.68) but significant differences between the HIO and control, and the IV and control groups (P=.03 and .01, respectively). Analysis of Variance (ANOVA) indicated the mean time to ROSC for HIO and IV was 621.20 seconds (SD=204.21 seconds) and 554.50 seconds (SD=213.96 seconds), respectively, with no significant difference between the groups (U=11; P=.22). Multivariate Analysis of Variance (MANOVA) revealed the maximum plasma concentration (Cmax) and time to maximum concentration (Tmax) of vasopressin in the HIO and IV groups was 71753.9 pg/mL (SD=26744.58 pg/mL) and 61853.7 pg/mL (SD=22745.04 pg/mL); 111.42 seconds (SD=51.3 seconds) and 114.55 seconds (SD=55.02 seconds), respectively. Repeated measures ANOVA indicated no significant difference in plasma vasopressin concentrations between the treatment groups over four minutes (P=.48).
ConclusionsThe HIO route delivered vasopressin effectively in a swine model of VF. Occurrence, time, and odds of ROSC, as well as pharmacokinetic measurements of HIO vasopressin, were comparable to IV.

Objective
Transparent evidence-based decision making has been promoted worldwide to engender trust in science and policy making. Yet, little attention has been given to transparency implementation. The degree of transparency (focused on how uncertain evidence was handled) during the development of folate and vitamin D Dietary Reference Values was explored in three a priori defined areas: (i) value request; (ii) evidence evaluation; and (iii) final values.
DesignQualitative case studies (semi-structured interviews and desk research). A common protocol was used for data collection, interview thematic analysis and reporting. Results were coordinated via cross-case synthesis.
SettingAustralia and New Zealand, Netherlands, Nordic countries, Poland, Spain and UK.
SubjectsTwenty-one interviews were conducted in six case studies.
ResultsTransparency of process was not universally observed across countries or areas of the recommendation setting process. Transparency practices were most commonly seen surrounding the request to develop reference values (e.g. access to risk manager/assessor problem formulation discussions) and evidence evaluation (e.g. disclosure of risk assessor data sourcing/evaluation protocols). Fewer transparency practices were observed to assist with handling uncertainty in the evidence base during the development of quantitative reference values.
ConclusionsImplementation of transparency policies may be limited by a lack of dedicated resources and best practice procedures, particularly to assist with the latter stages of reference value development. Challenges remain regarding the best practice for transparently communicating the influence of uncertain evidence on the final reference values. Resolving this issue may assist the evolution of nutrition risk assessment and better inform the recommendation setting process.
Predicting response to cognitive behavioral therapy in contamination-based obsessive–compulsive disorder from functional magnetic resonance imaging

Background
Although cognitive behavioral therapy (CBT) is an effective treatment for obsessive–compulsive disorder (OCD), few reliable predictors of treatment outcome have been identified. The present study examined the neural correlates of symptom improvement with CBT among OCD patients with predominantly contamination obsessions and washing compulsions, the most common OCD symptom dimension.
MethodParticipants consisted of 12 OCD patients who underwent symptom provocation with contamination-related images during functional magnetic resonance imaging (fMRI) scanning prior to 12 weeks of CBT.
ResultsPatterns of brain activity during symptom provocation were correlated with a decrease on the Yale–Brown Obsessive Compulsive Scale (YBOCS) after treatment, even when controlling for baseline scores on the YBOCS and the Beck Depression Inventory (BDI) and improvement on the BDI during treatment. Specifically, activation in brain regions involved in emotional processing, such as the anterior temporal pole and amygdala, was most strongly associated with better treatment response. By contrast, activity in areas involved in emotion regulation, such as the dorsolateral prefrontal cortex, correlated negatively with treatment response mainly in the later stages within each block of exposure during symptom provocation.
ConclusionsSuccessful recruitment of limbic regions during exposure to threat cues in patients with contamination-based OCD may facilitate a better response to CBT, whereas excessive activation of dorsolateral prefrontal regions involved in cognitive control may hinder response to treatment. The theoretical implications of the findings and their potential relevance to personalized care approaches are discussed.
