Over the years, so-called univentricular hearts represented one of the greatest challenges for surgical correction. All this changed with the advent of the Fontan procedure,1 along with the realization that it could become the final stage of the sequence of procedures used to correct lesions such as those included in the hypoplastic left heart syndrome,2 which previously had been beyond surgical repair. The overall group of lesions also posed significant problems in adequate description and categorization. Even these days, many continue to describe patients with a double inlet left ventricle as having a single ventricle, despite the fact that, with the availability of clinical diagnostic techniques producing three-dimensional datasets, patients with this lesion can be seen to have two chambers within their ventricular mass, one being large and the other small (Figure 9.1.1). The semantic problems with description can now be resolved by the simple expedient of describing the patients as having functionally univentricular hearts.3

In this chapter, a case is made for the inclusion of computational approaches to linguistics within the theoretical fold. Computational models aimed at application are a special case of predictive models. The status quo in the philosophy of linguistics is that explanation is scientifically prior to prediction. This is a mistake. Once corrected, the theoretical place of prediction is restored and, with it, computational models of language. The chapter first describes the history behind the emergence of explanation over prediction views in the general philosophy of science. It’s then suggested that this post-positivist intellectual milieu influenced the rejection of computational linguistics in the philosophy of theoretical linguistics. A case study of the predictive power already embedded in contemporary linguistic theory is presented through some work on negative polarity items. The discussion moves to the competence–performance divide informed by the so-called Galilean style in linguistics that retains the explanatory over prediction ideal. In the final sections of the chapter, continuous methods, such as probabilistic linguistics, are used to showcase the explanatory and predictive possibilities of nondiscrete approaches, before a discussion of the contemporary field of deep learning in natural language processing (NLP), where these predictive possibilities are further amplified.

Chapter 8 analyzes the marketing of inkiko gacaca; that is, the RPF’s effort to create demand for its invented tradition. By revealing a series of tactics related to this marketing strategy, the book here sheds light on the manufacturing of consent about the meaning of transitional justice in post-genocide Rwanda. The focus is on the presentation of law in everyday life, with particular reference to select localities.

The linchpin of colonial administration under regulatory imperialism was the royal governor. The crown relied on royal governors to control the colony’s political agenda, especially on fiscal matters. Yet, in most colonies, assemblies managed to invert this plan and claim almost complete fiscal power for themselves – one of the most important developments under imperial administration in the New World. To explain this, the present chapter develops a strategic logic of bargaining between assemblies and governors and the accountability of governors to the crown. The logic shows how governors were able to make major political concessions to assemblies, yet evade accountability to the crown for them. As a result, regulatory imperialism was unable to restrain the independence of colonial legislatures.

NaCl 0.9% is an established cause of metabolic acidosis. The underlying mechanism, a reduction in plasma strong ion difference [SID], is comprehensibly explained by the principles of the Stewart approach. Both fluid-induced hyperchloremia and metabolic acidosis can be avoided using various balanced solutions that do not cause alterations in plasma [SID]. Many balanced solutions are commercially available, using various alkalinizing agents such as lactate, acetate and gluconate. Their only drawback when compared with NaCl 0.9% is their higher cost. Although there no definitive evidence of their clinical benefit in low volumes in patients with a low severity of illness, some data suggest kidney injury under treatment with NaCl 0.9%, an effect that seams to appear in a dose-dependent manner. Awaiting definitive conclusions, the use of balanced solutions in sicker patients who are in need of large amounts of fluids seems to be a pragmatic choice.

All humans experience needs.1 At times needs cut deep, inhibiting persons’ abilities to act as agents in the world, to live in distinctly human ways, or to achieve life goals of significance to them. In considering such potentialities, several questions arise: Are any needs morally important, meaning that they operate as morally relevant details of a situation? What is the correct moral stance to take with regard to situations of need? Are moral agents ever required to tend to others’ well-being by meeting their needs? What justification or foundation, if any, can be given for requiring moral agents to respond to others’ needs?2

Understanding the anatomy of septal defects is greatly facilitated if the heart is thought of as having three distinct septal structures: the atrial septum, the atrioventricular septum, and the ventricular septum (Figure 8.1.1). The normal atrial septum is relatively small. It is made up, for the most part, by the floor of the oval fossa. When viewed from the right atrial aspect, the fossa has a floor, surrounded by rims. As we have shown in Chapter 2, the floor is derived from the primary atrial septum, or septum primum. Although often considered to represent a secondary septum, or septum secundum, the larger parts of the rims, specifically the superior, antero-superior, and posterior components, are formed by infoldings of the adjacent right and left atrial walls.1 Infero-anteriorly, in contrast, the rim of the fossa is a true muscular septum (Figure 8.1.2).

Cerebral oxygen consumption accounts for around 20% of the whole-body oxygen consumption. In the perioperative period,monitoring the adequacy of cerebral perfusion and oxygen delivery is desirable, since they can be compromised by several factors related to surgery and anaesthesia Cerebral oximeters are monitoring devices based on near infrared spectroscopy (NIRS) providing a continuous and non-invasive assessment of cerebral oxygenation. Cerebral oximetry is commonly adopted in cardiac surgery, with the widest application in aortic arch procedures and pediatric congenital heart surgery.There is growing interest in its application in non-cardiac surgery settings. Whilst the evidence for a role in impacting patient outcomes is still not completely clear even in cardiac surgery, cerebral oximetry can be a useful adjunct in perioperative monitoring and in hemodynamic management, provided that it is interpreted in the context of other physiological variables.

In ‘The Stoics on Conceptions and Concepts’, Katerina Ierodiakonou offers an account of the Stoics’ distinction between ennoiai and ennoēmata (‘conceptions’ and ‘concepts’), and also of the distinctions suggested by the standard Stoic terminology of concepts also mentioned above: notably, prolēpseis (‘preconceptions’), phusikai ennoiai (‘natural conceptions’), and koinai ennoiai (‘common conceptions’). All these terms appear intended to point to general notions that play a central role in the acquisition of human knowledge, but it remains puzzling how exactly the Stoics understood them or why they introduced them into their doctrine in the first place. Ierodiakonou addresses these issues, as well as further questions debated in the secondary literature. These include whether all human beings necessarily possess concepts or just have the ability to possess them, what is the content of conceptions and how it is determined, what is the ontological status of conceptions and concepts, and what are their epistemological functions.
