  • Transcatheter closure has become a common treatment method for patent ductus arteriosus in premature infants at many centres; however, many remain uncertain about the ability to perform the procedure in the catheterisation laboratory for infants requiring high-frequency ventilation. This study presents our centre’s experience following the implementation of neonatal ventilatory guidelines, which resulted in 100% procedural success without any procedural or respiratory adverse events.

  • Summary

    This chapter highlights the role of capnography as a monitoring tool with the different adjuncts to mechanical ventilation that are currently used in critically ill patients. The application of positive end-expiratory pressure (PEEP) is used to increase lung volume and improve oxygenation in patients with acute lung injury (ALI). Studies of unilateral lung injury demonstrate that the consolidated lung regions do not expand to total lung capacity during inflation. Tracheal gas insufflation (TGI) is an adjunct to mechanical ventilation that allows ventilation with small tidal volumes while CO2 is satisfactorily eliminated. High-frequency ventilation (HFV) techniques have three essential elements in common: a high-pressure flow generator, a valve for flow interruption, and a circuit for connection to the patient. Measurement of deadspace fraction early in the course of acute respiratory failure may provide clinicians important physiologic and prognostic information.

  • Despite recent advances in intensive care medicine, acute lung injury and its more severe form, acute respiratory distress syndrome pose major therapeutic problems. While mechanical ventilation is integral to the care of these patients, its adverse consequences including ventilator-induced lung injury are determinants of disease progression and prognosis. Among several important ventilator parameters, the use of low tidal volumes is probably the most important feature of lung-protective mechanical ventilation. Intensivists should be trained to recognize acute lung injury and acute respiratory distress syndrome and encouraged to use low-tidal-volume ventilation in clinical practice. Alternative modes of ventilation such as high-frequency ventilation and prone position should be reserved for selected patients in whom conventional lung-protective ventilation strategies have failed.

  • Background

    The objective of this study was to determine the intracranial, cardiovascular and respiratory changes induced by conversion to high-frequency oscillator ventilation from conventional mechanical ventilation at increasing airway pressures.

    Methods

    In this study, 11 anaesthetized sheep had invasive cardiovascular and intracranial monitors placed. Lung injury was induced by saline lavage and head injury was induced by inflation of an intracranial balloon catheter. All animals were sequentially converted from conventional mechanical ventilation to high-frequency oscillator ventilation at target mean airway pressures of 16, 22, 28, 34 and 40 cm H2O. The mean airway pressure was achieved by adjusting positive end expiratory pressure while on conventional mechanical ventilation, and continuous distending pressures while on high-frequency oscillator ventilation. Cerebral lactate production, oxygen consumption and venous oximetry were measured and analysed in relation to changes in transcranial Doppler flow velocity. Transcranial Doppler profiles together with other physiological parameters were measured at each airway pressure.

    Results

    Cerebral perfusion pressure was significantly lower during high-frequency oscillator ventilation than during conventional mechanical ventilation (CMV: 45, 34, 22, 6, 9 mmHg vs. HFOV: 33, 20, 19, 5, 5 mmHg at airway pressures mentioned above, P = 0.02). Intracranial pressure and cerebrovascular resistance increased with increasing intrathoracic pressures (P = 0.001). Cerebral metabolic indices demonstrated an initial increase in anaerobic metabolism followed by a decrease in cerebral oxygen consumption progressing to cerebral infarction as intrathoracic pressures were further increased in a stepwise fashion. Arterial PaCO2 increased significantly after converting from conventional mechanical ventilation to high-frequency oscillator ventilation (P = 0.001). However, no difference was observed between conventional mechanical ventilation and high-frequency oscillator ventilation when intracranial pressure, metabolic and transcranial Doppler indices were compared at equivalent mean airway pressures.

    Conclusions

    The use of high positive end expiratory pressure with conventional mechanical ventilation or high continuous distending pressure with high-frequency oscillator ventilation increased intracranial pressure and adversely affected cerebral metabolic indices in this ovine model. Transcranial Doppler is a useful adjunct to intracranial pressure and intracranial venous saturation monitoring when major changes in ventilation strategy are adopted.

  • Respiratory support with high-frequency jet ventilation has been advocated during airway surgery or to improve pulmonary mechanics and gas exchange in patients with bronchopleural fistulae or pulmonary insufficiency. Despite a large body of published evidence describing its benefits as an alternative ventilatory approach in anaesthesia and intensive care medicine, its application has not gained widespread acceptance and is restricted to specialized centres. To review the literature on high-frequency jet ventilation in European and North American institutions, we performed a search in a computerized database (MEDLINE) covering the period from 1990 until the present time, describing the use of high-frequency jet ventilation in over 7000 patients. Various modes to apply high-frequency jet ventilation during airway surgery have been established, but its value in intensive care is controversial. We report our experience with combined high-frequency jet ventilation and provide guidelines for its safe application.
