The impact of food industry reformulation, innovation and consumer preference on dietary intakes in Ireland: a probabilistic intake model

Strong evidence exists linking poor diet to increased risk of overweight, obesity and non-communicable diseases. Reformulation initiatives, whereby the food industry gradually reduces energy, sodium, sugar, fat and saturated fat content of their products, are identified as important strategies to improve dietary intakes. The analysis aimed to examine the impact of voluntary reformulation, changes to products on the market and changes in consumer preferences on dietary intakes in Ireland from 2005 to 2017. Data on composition and volume sales (kg/ year) of products available on the market in 2005 and 2017 were collected from 15 Food Drink Ireland (FDI) member companies via online templates. These products were assigned to appropriate food and beverage groups identified in four Irish University Nutrition Alliance (IUNA) surveys of preschool children (1–4 years), children (5–12 years), teenagers (13–17 years) and adults (18–90 years). Assignment of FDI products to IUNA foods and beverages was carried out using weighted distributions for a given group of foods. The weightings were taken from the sales volumes of similar products relative to one another in a given category in a given year. Monte Carlo simulations were used to run the IUNA survey consumption data with both sets of weighted composition data from 2005 and 2017. The Creme Global intake model was used to estimate daily energy and nutrient intakes for all four populations during 2005 and 2017. The Wilcoxon-signed rank test was used to test for differences between the two years. Changes in both the products available on the market and market share of these products were observed from 2005 to 2017. The nutrient with the greatest intake reduction between the two years for all ages was sugar. Children and teens were the most affected, where total sugar intakes reduced by 3.2g/d and 2.7g/d, respectively. This reduction was primarily driven by the beverage category. There were modest saturated fat intake reductions observed for teens and adults (0.2g/d and 0.5g/d, respectively). Energy, total fat and sodium intakes for all ages remained relatively stable between the two years. This analysis highlights the impact of not only food industry efforts but also consumer choices on nutrient intakes in Ireland. It is worth noting that the data collected predates the sugar tax on sugar-sweetened beverages in Ireland. Reductions in sugar intakes were not compensated by total fat or energy increases.
Integrating mental health and disaster preparedness in intervention: a randomized controlled trial with earthquake and flood-affected communities in Haiti

Background
Given the frequency of natural hazards in Haiti, disaster risk reduction is crucial. However, evidence suggests that many people exposed to prior disasters do not engage in disaster preparedness, even when they receive training and have adequate resources. This may be partially explained by a link between mental health symptoms and preparedness; however, these components are typically not integrated in intervention.
MethodsThe current study assesses effectiveness of an integrated mental health and disaster preparedness intervention. This group-based model was tested in three earthquake-exposed and flood-prone communities (N = 480), across three time points, using a randomized controlled trial design. The 3-day community-based intervention was culturally-adapted, facilitated by trained Haitian lay mental health workers, and focused on enhancing disaster preparedness, reducing mental health symptoms, and fostering community cohesion.
ResultsConsistent with hypotheses, the intervention increased disaster preparedness, reduced symptoms associated with depression, post-traumatic stress disorder, anxiety, and functional impairment, and increased peer-based help-giving and help-seeking. Mediation models indicated support for the underlying theoretical model, such that the effect of the intervention on preparedness was mediated by mental health, and that effects on mental health were likewise mediated by preparedness.
ConclusionsThe community-based mental health-integrated disaster preparedness intervention is effective in improving mental health and preparedness among community members in Haiti vulnerable to natural hazards. This brief intervention has the potential to be scaled up for use with other communities vulnerable to earthquakes, seasonal flooding, and other natural hazards.

Summary
Executive Summary
Disasters are most acutely experienced at the local level (high agreement, robust evidence). The reality of disasters in terms of loss of life and property occurs in local places and to local people. These localized impacts can then cascade to have national and international consequences. In this chapter, local refers to a range of places, social groupings, experience, management, institutions, conditions, and sets of knowledge that exist at a sub-national scale. [5.1]
Developing strategies for disaster risk management in the context of climate change requires a range of approaches, informed by and customized to specific local circumstances (high agreement, robust evidence). These differences and the context (national to global, urban to rural) in which they are situated shape local vulnerability and local impacts. [5.1]
The impacts of climate extremes and weather events may threaten human security at the local level (high agreement, medium evidence). Vulnerability at the local level is attributed to social, political, and economic conditions and drivers including localized environmental degradation and climate change. Addressing disaster risk and climate extremes at the local level requires attention to much wider issues relating to sustainable development. [5.1]
While structural measures provide some protection from disasters, they may also create a false sense of safety (high agreement, robust evidence). Such measures result in increased property development, heightened population density, and more disaster exposure. Current regulations and design levels for structural measures may be inadequate under conditions of climate change. [5.3.2]

An Outbreak of Acute Gastroenteritis in a Geriatric Long-Term–Care Facility: Combined Application of Epidemiological and Molecular Diagnostic Methods

Objective:
To assess possible transmission modes of, and risk factors for, gastroenteritis associated with Norwalk-like viruses (NLVs) in a geriatric long-term-care facility.
Methods:During a prolonged outbreak of acute gastroenteritis, epidemiological data on illness among residents and employees were collected in conjunction with stool, vomitus, and environmental specimens for viral testing. NLVs were identified by electron microscopy in stool and vomitus specimens, and further characterized by reverse-transcriptase polymerase chain reaction and nucleotide sequencing. Potential risk factors were examined through medical-record review, personal interview, and a self-administered questionnaire sent to all employees.
Results:During the outbreak period, 52 (57%) of 91 residents and 34 (35%) of 90 employees developed acute gastroenteritis. Four case-residents were hospitalized; three residents died at the facility shortly after onset of illness. A point source was not identified; no association between food or water consumption and gastroenteritis was identified. A single NLV strain genetically related to Toronto virus was the only pathogen identified. Residents were at significantly higher risk of gastroenteritis if they were physically debilitated (relative risk [RR], 3.5; 95% confidence interval [CI95], 1.0-12.9), as were employees exposed to residents with acute gastroenteritis (RR, 2.6; CI95, 1.1-6.5) or ill household members (RR, 2.3; CI95, 1.4-3.6). Adherence to infection control measures among the nursing staff may have reduced the risk of gastroenteritis, but the reduction did not reach statistical significance.
Conclusions:In the absence of evidence for food-borne or waterborne transmission, NLVs likely spread among residents and employees of a long-term-care facility through person-to-person or airborne droplet transmission. Rapid notification of local health officials, collection of clinical specimens, and institution of infection control measures are necessary if viral gastroenteritis transmission is to be limited in institutional settings
Morphology of a Blend of Zinc Neutralized Sulfonated Poly(Phenylene Oxide) or Polystyrene and an Amino Silicone

A polymer blend of an ionomer based on zinc neutralized sulfonated poly(phenylene oxide) or polystyrene with a silicone copolymer containing 6.45% propylamine groups in place of one of the methyl groups on a backbone silicon was prepared. Carbon-13 magic angle spinning spectra show coordination of the amine by the zinc ions. Morphological characterization was made by NMR spectroscopy based on proton spin diffusion, by small angle x-ray scattering and by energy filtered transmission electron microscopy. All experiments show domains in the range of 1 to 1000 nm and domain size can be controlled by the extent of coordination of the amine groups by the zinc ions and by thermal history. The different morphological experiments lead to an apparent hierarchy of domain sizes. The NMR experiment yields the smallest domains, 2 to 10 nm, where contrast is produced by differences in chain mobility. Small angle x-ray scattering indicates domains of 16 nm while electron microscopy indicates domains in the range of 100 to 1000 nm. The variation of the domain size between experiments may reflect the different sources of contrast in each case.
