Physician-based transparency approaches have been advanced as a strategy for informing patients of the likely financial consequences of using services. The structure of health care pricing and insurance coverage, and the low uptake of existing tools, suggest these approaches are likely to be unwieldy and unsuccessful. They may also generate new ethical challenges.

The development of small-scale self-focusing in a nonlinear Kerr medium after preliminary self-filtering of a laser beam propagating in free space is studied numerically. It is shown that, under definite conditions, due to self-filtering, filamentation instability (beam splitting into filaments) either occurs at significantly larger values of the B-integral, or does not occur at all. In the latter case, there develops the honeycomb instability revealed in this work. This instability is the formation of a random honeycomb structure in the beam cross-section. It is shown that self-filtering can significantly increase the permissible values of the B-integral, at which the beam quality remains acceptable.

Glucagon-like peptide-1 receptor agonists are effective for treating obesity, but the high cost of these medications endangers the financial viability of our health care system. To ensure that these drugs are available to Medicare beneficiaries, pharmaceutical manufacturers must lower their prices.

Tonic immobility is considered an anti-predator defence, wherein prey adopts a motionless state in a characteristic posture elicited by external stimuli. The marine isopod Cleantiella isopus exhibits tonic immobility with an arch-like posture and motionless state lasting several seconds or minutes in response to external stimuli such as predatory attacks by fish. In this study, we describe tonic immobility by wild-caught C. isopus and examine the influence of body size, sex, and colour morph on the frequency and duration of tonic immobility. All individuals exhibited tonic immobility regardless of body size, sex, or colour morph, suggesting that the behaviour plays a major role in predator avoidance following detection by a predator. In males, smaller individuals exhibited more prolonged tonic immobility than larger individuals, whereas the relationship between the duration of tonic immobility and body size was unclear in females. Colour morph had no effect on the duration of tonic immobility. These findings provide a detailed documentation of tonic immobility in C. isopus and suggest that the factors affecting tonic immobility differ between males and females.
