  • A Storm, An Explosion, and Flying Rocks - An Unusual Injury due to a Lightning Strike in the Mountains

  • Background:

    Most injuries observed in victims of lightning strikes can be explained by electrothermal phenomena. Blast penetrating injuries caused by a lightning-strike-induced explosion of a nearby structure are rarely reported.

    Case Presentation:

    Here reported is the case of a patient with numerous mixed injuries caused by a lightning strike, including deep lacerations of both hips and thighs with rock fragments embedded in the wounds. Surgical removal of rock fragments from deep areas of the right hip and right lower leg was necessary. The cause of the formation of rock missiles was the lightning-strike-induced explosion of rock. Rapid evaporation of water enclosed in rock crevices was presumably the main force underlying the explosion.

    Conclusion:

    Blast penetrating injuries should be considered and excluded in all patients struck by lightning, particularly when occurring in rocky terrain. The diagnosis and treatment of such injuries can be difficult and require special preparation.

  • Summary

    Traumatic brain injury (TBI) is a leading type of epilepsy with significant repercussions for the quality of life of patients, due to the associated injury, consequent epilepsy and cognitive, behavioral or neuropsychiatric sequelae. There have been intense efforts to generate better strategies and methods to treat these patients better. This chapter reviews the advances in animal models of posttraumatic epilepsy (PTE), focusing on rodents, presenting an update on models, their phenotype, findings on neurobiology of TBI and PTE and future directions. The value of models, like the fluid percussion injury, controlled cortical impact, blast, penetrating TBI, weight drop TBI, in this process in being discussed as well as efforts to accelerate progress in the field through the use of collaborative research and infrastructure.

  • A 20-year-old male was impaled through the chest, abdomen, and right upper thigh by three 1.5 cm (0.59 in) diameter rods, each 2 m (6.56 ft) in length. The first rod entered below his right nipple, the second through the right hypochondrium, and the third through the right upper thigh. He was transported to the hospital with the rods in situ. This paper provides insight as to how these unusual injuries were managed in a limited-resource environment. Even in a developing country, the challenges posed by multiple impalement injuries can be managed successfully by rapid prehospital transfer, along with an adequate and coordinated hospital team effort.

  • Hockey is enjoyed by millions of people around the world and is a sport in which aggression is encouraged and injuries are common. Although body-checking is the most common cause of injury in hockey today, hockey sticks are associated with up to 14% of injuries. We report a case of chest trauma requiring surgical intervention secondary to the penetration of a composite hockey stick into a player's thoracic cavity.
