Newborn male with symptomatic bradycardia initially diagnosed with complete atrioventricular block. Isoproterenol drip was initiated, and the patient was scheduled for pacemaker implantation. During the hospital course, repeat electrocardiogram and Holter monitor revealed evidence of near continuous blocked atrial bigeminy with occasional aberrantly conducted premature atrial contractions. Flecainide was started, resulting in normal sinus rhythm, and the pacemaker implantation was cancelled.

In view of the increasing complexity of both cardiovascular implantable electronic devices (CIEDs) and patients in the current era, practice guidelines, by necessity, have become increasingly specific. This document is an expert consensus statement that has been developed to update and further delineate indications and management of CIEDs in pediatric patients, defined as ≤21 years of age, and is intended to focus primarily on the indications for CIEDs in the setting of specific disease categories. The document also highlights variations between previously published adult and pediatric CIED recommendations and provides rationale for underlying important differences. The document addresses some of the deterrents to CIED access in low- and middle-income countries and strategies to circumvent them. The document sections were divided up and drafted by the writing committee members according to their expertise. The recommendations represent the consensus opinion of the entire writing committee, graded by class of recommendation and level of evidence. Several questions addressed in this document either do not lend themselves to clinical trials or are rare disease entities, and in these instances recommendations are based on consensus expert opinion. Furthermore, specific recommendations, even when supported by substantial data, do not replace the need for clinical judgment and patient-specific decision-making. The recommendations were opened for public comment to Pediatric and Congenital Electrophysiology Society (PACES) members and underwent external review by the scientific and clinical document committee of the Heart Rhythm Society (HRS), the science advisory and coordinating committee of the American Heart Association (AHA), the American College of Cardiology (ACC), and the Association for European Paediatric and Congenital Cardiology (AEPC). The document received endorsement by all the collaborators and the Asia Pacific Heart Rhythm Society (APHRS), the Indian Heart Rhythm Society (IHRS), and the Latin American Heart Rhythm Society (LAHRS). This document is expected to provide support for clinicians and patients to allow for appropriate CIED use, appropriate CIED management, and appropriate CIED follow-up in pediatric patients.

Introduction: Transcutaneous cardiac pacing (TCP) is recommended for the treatment of symptomatic bradycardia, a life-threatening condition. Although TCP is taught in ACLS (advanced cardiac life support) courses, it is a difficult skill to master for junior residents. The main objective of this study is to measure the impact of having access to a checklist on successful TCP implementation. Our hypothesis was that the availability of a CL would improve performance of junior residents in the management of symptomatic bradycardia by facilitating TCP. Methods: We conducted a prospective, randomized, single-site study. First-year residents entering postgraduate programs and taking a mandatory ACLS course were enrolled. Students had didactic sessions on the management of symptomatic bradycardia followed by hands-on teaching on a low-fidelity manikin (ALS® simulator, Laerdal) using a CL conceived for this project as a teaching tool. Study participants were then assessed with a simulation scenario requiring TCP. Participants were randomly assigned to groups with and without CL accessibility. Performances were graded on six critical tasks. The primary outcome was the successful use of TCP, defined as having completed all tasks. Participants then completed a post-test questionnaire. Sample size estimation was based on a previous project (Ranger et al., 2018). Accepting an alpha error of 0.05 and a power of 80%, 45 participants in each group would permit the detection of 26.5% in performance gain. Results: Of 250 residents completing the ACLS course in 2017, 85 voluntary participants were randomized to a control group (no CL available during testing, n = 42) or an experimental group (CL available during testing, n = 43). Six participants in the experimental group adequately used TCP compared to five participants in the control group (p = 0.81, chi-squared test). Out of the 43 participants who had access to the CL, only 2 (5%) used it. Reasons why the CL was infrequently used were stated as the following: 24 participants (56%) mentioned not realizing it was available, 8 (19%) considered it was of little to no utility and 5 (19%) forgot a CL existed. Conclusion: Availability of a checklist previously used during simulation teaching did not increase junior residents’ capacity to correctly apply TCP. Non-recognition of CL availability and decreased perceived need for it were the main reasons for marginal use. Our results suggest that there are many limiting factors to CL effectiveness.

The vast majority of cardiac pacemakers are implanted in older people. Pacing is a safe and effective treatment for patients with symptomatic bradyarrhythmias. This review provides a clinician's guide to the essential aspects of modern pacemaker therapy in older patients, including aetiology and epidemiology of bradycardia. We discuss some barriers to access to pacing. We review indications for pacemakers, and describe pacing system function, including the different types of pacemakers used. We also review clinical outcomes following pacing, follow-up requirements and problems encountered in clinical practice. Finally, we discuss ethical and end of life considerations with these devices.

Cardiac performance of the Chinese black sleeper (Bostrichthys sinensis) was examined at early stages of development (the heartbeat, blood circulation and newly hatched larval stages) under different hypoxic oxygen concentrations. Exposure of the embryos at 25°C to 0.2 mg/l dissolved oxygen (DO) indicated periodic heartbeat at both the heartbeat stage and the blood circulation stage, but it started sooner after exposure at the blood circulation stage than at the heartbeat stage. With the appearance of the gill slits, the tolerance of the newly hatched larvae to hypoxia was improved. The larvae did not display periodic heartbeat at 0.2 mg/L DO, but periodic heartbeat occurred at 0.1 mg/L DO and 0.02 mg/L DO after 23.3±6.2 minutes and 13.2±3.4 minutes of exposure. With exposure of the embryos to hypoxia, the occurrence of periodic heartbeat of excised embryos was earlier than that of whole egg embryos at both the blood circulation and heartbeat stages. These results indicated that the tolerance of embryos to hypoxia decreased with the progress of embryogenesis and suggested that the perivitelline liquid can protect the embryos by conserving oxygen. In this study, we also found that a breakdance performance (an arrhythmic heart beat) occurred in the newly hatched larvae at 0.02 mg/l DO. The breakdance performance lasted about 10 minutes, and then the heartbeat became normal.

Second degree atrioventricular block is uncommon in children. In order to evaluate its outcome, and to find early prognostic factors, we reviewed the history and evolution of 21 children with this arrhythmia discovered on their surface electrocardiogram. Twenty-four-hour monitoring displayed variations in the conduction patterns in almost all children, from long PR interval to complete heart block alternating in the same patient. At follow-up (nine months to 19 years), 13 children (62%) had received implantation of a pacemaker because of progression either to complete atrioventricular block or to severe ventricular bradycardia. Atrioventricular conduction improved on exercise in 11 children, but this did not predict a favorable outcome since four of them required pacing. In contrast, deterioration during sinus acceleration in four patients predicted further aggravation. A supra-Hisian location of the block did not protect against the occurrence of syncope in the two patients who underwent His recordings. Finally, the outcome was not linked to age at diagnosis. Second degree atrioventricular block, therefore, should be considered a serious disease because of its trend to worsen towards complete block and/or severe complications. Close follow-up, including repeated 24-hour monitoring, is the most effective way to unmask such a progression and to avoid its clinical consequences.

Objective: To evaluate the origin of transient episodes of sinus bradycardia, atrial escape rhythm, and atrioventricular nodal escape rhythm in preterm infants. Material and methods: The study was observational, and was carried out in a third level neonatal intensive care unit. We studied 19 spontaneously breathing infants born healthy but prior to term, the examinations being carried out between the ages of 3 and 28 days. The mean gestational age was 29.2 ± 1.9 weeks, and the mean birth weight was 1154 ± 264 g. Transient episodes of bradycardia were defined as a decline in heart rate equal to or greater than 25% from baseline, lasting for at least 3 successive RR-intervals. To discriminate between different types of escape mechanisms, we used the P wave and the P axis of the electrocardiogram. Sinus bradycardia was diagnosed when the P axis was from +0 to +90 degrees; atrial escape rhythm when it was from +91 to +359 degrees, and atrioventricular nodal escape rhythm when the P wave was absent, hidden, or followed the QRS complex. Results: The mean P axis was +50 ± 11 degrees. We observed 60 transient episodes of bradycardia in 11 of the 19 infants. Of these, 34 (57%) were classified as sinus bradycardia, and 13 (22%) as atrial escapes. Atrioventricular nodal escapes developed during 6 episodes (10%), while 7 episodes (11%) could not be classified. Conclusions: Atrial excitation as evidenced by the P axis during sinus rhythm is similar in very preterm infants to that seen in children and adults. Escape rhythms with different origins occur during transient episodes of bradycardia in healthy preterm infants. In at least one third, the episodes are due to atrial or atrioventricul ar nodal escape.

Two cases of bradycardia following the intra-operative use of ketorolac given intravenously (i.v.) to children are reported. We examine why this may not have been previously reported and make recommendations regarding its use.
