  • Manic episodes: the direct cost of a three-month period following hospitalisation

  • Introduction.

    Few data exist to estimate the burden of manic episodes on healthcare systems or the therapeutic strategies used. This study was undertaken to identify treatment strategies chosen, and to assess the “real-world” direct medical cost of treating manic episodes necessitating hospitalisation.

    Method.

    Case record data were reviewed during the three months following hospitalisation for a manic episode in France. Healthcare resource utilisation was assessed, direct costs calculated, and treatment strategies analysed. A total of 137 patients files (51.8% female; mean age: 35 years) were reviewed and data on 185 hospitalisations collected.

    Results.

    The mean duration of hospitalisation was 47 days over the study period. The most common treatment strategy during hospitalisation was the combination of a mood stabiliser with a neuroleptic drug (64% of patients at day 30). Anticonvulsants including valproate (39%) and carbamazepine (20%) were more common than lithium (42%). Treatment received during hospitalisation was generally continued after discharge, with a trend away from neuroleptics and towards mood stabilisers. The mean direct costs incurred over the three-month study period was Euro 22 297, with 98.6% of those costs due to hospitalisation.

    Conclusion.

    These results confirm that the costs of treating a manic episode are high, and overwhelmingly due to the cost of hospitalisation.

  • Since the prevalence of many chronic health conditions increases with age, we might anticipate that as the population ages the proportion with one or more such conditions, and the cost of treatment, would rise. How much would the overall prevalence of chronic conditions increase in a quarter century if age-specific rates of prevalence did not change? How much would the requirements for health care resources increase? How much difference would it make to those requirements if people had fewer chronic conditions? The overall prevalence rates for almost all conditions associated mostly with old age would rise by more than 25 per cent, and health care requirements would grow more rapidly than the population – more than twice as rapidly in the case of hospital stays – if the rates for each age group remained constant. Even modest reductions in the average number of conditions at each age could result in substantial savings.

  • Summary

    It is much easier to make an exact calculation of the direct treatment costs of intervention than the often hidden indirect costs of nonintervention. This chapter reviews cost-effectiveness studies that explicitly compare benefits in relation to costs and risks in other areas of medicine. It examines a number of research initiatives currently underway to study the indirect costs of psychiatric disorders and the extent to which these costs can be reversed with intervention. The chapter recommends that mechanisms to assure the quality of interventions have to be developed as part of cost-effectiveness studies if we hope to persuade policy makers to increase the allocation of health care resources to psychiatric disorders. It argues that while process assessments have to play a prominent role in any plan for the quality assurance of psychiatric interventions, special features of psychiatric disorders make them especially appropriate for outcomes assessment.
