Objectives
The article aims to investigate the feasibility, acceptability, and initial efficacy of a short-term 3-day art therapy group for children who have experienced parental death to cancer.
MethodsThe study utilized a pretest–posttest design and included children (n = 20) aged 7–12 years. The feasibility of the intervention was measured by recruitment ability, study compliance, and intervention adherence, while acceptability was assessed using a child-reported satisfaction survey. Efficacy was examined using the child-reported Pediatric Quality of Life Inventory (PedsQL), while the emotional, social, and behavioral functioning of children was measured using the parent-reported Strengths and Difficulties Questionnaire. Paired sample t-tests were used for analyses.
ResultsThe intervention was found to be feasible (80% recruitment rate and 100% session adherence). Acceptability was high, and all participants were satisfied and found the intervention to be helpful. While results did not reach statistical significance, improvements in psychosocial and physical quality of life were reported by all the children post-intervention and at the 3-month follow-up. Parent-reported a decrease in behavioral difficulties scores and an increase in prosocial behavior scores at post-intervention and at the 3-month follow-up.
Significance of resultsThe 3-day art therapy group intervention was shown to be feasible to conduct and acceptable to the recipients. The intervention shows promise in improving post-death adjustment and quality of life outcomes of children bereaved by parental death due to cancer that were maintained after 3 months. The use of art therapy groups to ameliorate difficulties associated with parental loss and to assist children in coping day-to-day difficulties should be further investigated.

Introduction
This chapter explores policy and practice in Hong Kong, and their impact on people with intellectual disabilities and their families, in three phases: before the 1990s (‘Early developments’); pre- and post-resuming sovereignty over Hong Kong (‘The Golden Era’); and from the Millennium to the present (‘Divergence’).
Before the 1990s: Early developments
Laura's story I: A parent's experience
Laura, the mother of a 43-year-old daughter with Down's Syndrome, said:
“I still remember the moment, just after my labour, when I heard and learned from the dialogue among the medical staff in the delivery room that my daughter had Down's Syndrome; no one came up to me and informed me directly. I felt helpless and did not know where to seek help or get more information. Afterwards, no social worker came to me to give me any support or counselling. The hospital also did not follow up on my situation after we were discharged. I took some time to accept this news, and I had to seek information from other organisations on my own. The days were full of tears, as I recall.” (A scenario from the year 1974)
Laura's experience reflects the general situation of families with a child with intellectual disability in the early 1970s. This period marked the beginning of the development of disability services (then conceived of as ‘rehabilitation services’) in Hong Kong. From the 1970s to the 1990s, the field was in a developmental phase in ideology, policy direction, service provision and people's understanding.
Morninghill School was the first special school in Hong Kong for pupils with intellectual disability. It was established in 1964 by a group of parents. The school had one class, with four pupils: one each from the USA, Canada, Australia and Scotland. From 1968 the school was subsidised by the government and run by the Hong Kong Association for the Mentally Handicapped (now called Hong Chi Association) (Hong Chi Association, 2018). The enactment by the government of six-year compulsory education in 1971 and nine-year free and compulsory education in 1978 (Poon-McBrayer and Lian, 2002) gradually provided children with intellectual disability the opportunity of schooling.

Summary
Introduction
The term kernicterus was originally used to describe the deposition of bilirubin in the basal ganglia. It was first described in 1903 by Schmorl. More recently, the term has also been used in reference to the chronic and permanent clinical sequelae of bilirubin toxicity. For the acute manifestations of bilirubin toxicity, the term acute bilirubin encephalopathy or ABE has been adopted. Another acronym, BIND, has been adopted to describe any bilirubin-induced neurologic dysfunction. Although technically the diagnosis of kernicterus can only be confirmed at autopsy, brain magnetic resonance imaging (MRI) studies may now aid in the confirmation of the diagnosis in a living child with severe jaundice. The MRI signature for kernicterus includes high signal intensity on T1-weighted (T1W) images in the globus pallidus, internal capsule, thalamus, and hippocampi (Fig. 27.1). The associated T2W images have abnormal increased signal in the globus pallidus and thalamus in the same regions as the high signal on the T1W images (Fig. 27.2). Loss of demarcation between globus pallidus, internal capsule, and the anterior thalamus was the major finding. The source of these abnormal signals has not been definitively identified, and therefore the MRI findings should not be considered diagnostic in themselves, but only consistent with the diagnosis of kernicterus in the context of severe neonatal jaundice and the acute and chronic clinical features of bilirubin toxicity (Table 27.1).
