Long-term outcomes of trauma-focused treatment in psychosis

We present 12-month follow-up results for a randomised controlled trial of prolonged exposure and eye movement desensitisation and reprocessing (EMDR) therapy in 85 (78.8%) participants with psychotic disorder and comorbid post-traumatic stress disorder (PTSD). Positive effects on clinician-rated PTSD, self-rated PTSD, depression, paranoid-referential thinking and remission from schizophrenia were maintained up to 12-month follow-up. Negative post-traumatic cognitions declined in prolonged exposure and were stable in EMDR. A significant decline in social functioning was found, whereas reductions in interference of PTSD symptoms with social functioning were maintained. These results support that current PTSD guidelines apply to individuals with psychosis.

Background
Post-traumatic stress disorder (PTSD) is highly prevalent in patients with a psychotic disorder. Because a PTSD diagnosis is often missed in patients with psychosis in routine care, a valid screening instrument could be helpful.
AimsTo determine the validity of the Trauma Screening Questionnaire (TSQ) as a screening tool for PTSD among individuals with psychotic disorders.
MethodAmong 2608 patients with a psychotic disorder, the rate of trauma exposure was determined and the TSQ was administered to screen for PTSD. PTSD status was verified in 455 patients using the Clinician-Administered PTSD Scale (trial registration: ISRCTN 79584912).
ResultsTrauma exposure was reported by 78.2% of the 2608 patients. PTSD prevalence was estimated at 16% (95% CI 14.6–17.4%) compared with 0.5% reported in the patients' clinical charts. A TSQ cut-off score of six predicted PTSD with 78.8% sensitivity, 75.6% specificity, 44.5% correct positives and 93.6% correct negatives.
ConclusionsThe TSQ seems to be a valid screening tool for PTSD in patients with a psychotic disorder.

OBJECTIVE
Manual surveillance of healthcare-associated infections is cumbersome and vulnerable to subjective interpretation. Automated systems are under development to improve efficiency and reliability of surveillance, for example by selecting high-risk patients requiring manual chart review. In this study, we aimed to validate a previously developed multivariable prediction modeling approach for detecting drain-related meningitis (DRM) in neurosurgical patients and to assess its merits compared to conventional methods of automated surveillance.
METHODSProspective cohort study in 3 hospitals assessing the accuracy and efficiency of 2 automated surveillance methods for detecting DRM, the multivariable prediction model and a classification algorithm, using manual chart review as the reference standard. All 3 methods of surveillance were performed independently. Patients receiving cerebrospinal fluid drains were included (2012–2013), except children, and patients deceased within 24 hours or with pre-existing meningitis. Data required by automated surveillance methods were extracted from routine care clinical data warehouses.
RESULTSIn total, DRM occurred in 37 of 366 external cerebrospinal fluid drainage episodes (12.3/1000 drain days at risk). The multivariable prediction model had good discriminatory power (area under the ROC curve 0.91–1.00 by hospital), had adequate overall calibration, and could identify high-risk patients requiring manual confirmation with 97.3% sensitivity and 52.2% positive predictive value, decreasing the workload for manual surveillance by 81%. The multivariable approach was more efficient than classification algorithms in 2 of 3 hospitals.
CONCLUSIONSAutomated surveillance of DRM using a multivariable prediction model in multiple hospitals considerably reduced the burden for manual chart review at near-perfect sensitivity.

Executive summary
Nature of the problem
The future effects of nitrogen in the environment will depend on the extent of nitrogen use and the practical application techniques of nitrogen in a similar way as in the past. Projections and scenarios are appropriate tools for extrapolating current knowledge into the future. However, these tools will not allow future system turnovers to be predicted.
Approaches
In principle, scenarios of nitrogen use follow the approaches currently used for air pollution, climate, or ecosystem projections. Short-term projections (to 2030) are developed using a ‘baseline’ path of development, which considers abatement options that are consistent with European policy. For medium-term projections (to 2050) and long-term projections, the European Nitrogen Assessment (ENA) applies a ‘storyline’ approach similar to that used in the IPCC SRES scenarios. Beyond 2050 in particular, such storylines also take into account technological and behavioral shifts.
Key findings/state of knowledge
The ENA distinguishes between driver-oriented and effect-oriented factors determining nitrogen use. Parameters that cause changes in nitrogen fixation or application are called drivers. In a driver-based approach, it is assumed that any variation of these parameters will also trigger a change in nitrogen pollution. In an effect-based approach, as the adverse effects of nitrogen become evident in the environment, introduction of nitrogen abatement legislation requiring the application of more efficient abatement measures is expected. This approach needs to rely on a target that is likely to be maintained in the future (e.g. human health). Nitrogen abatement legislation based on such targets will aim to counter any growth in adverse environmental effects that occur as a result of increased nitrogen application.

Introduction
What is the probability that Sweden will win next year's world championships in ice hockey? If you're a hockey fan, you probably already have a good idea, but even if you couldn't care less about the game, a quick perusal of the world championship medalists for the last 15 years (Table 7.1) would allow you to make an educated guess. Clearly, Sweden is one of only a small number of teams that compete successfully for the medals. Let's assume that all seven medalists the last 15 years have the same chance of winning, and that the probability of an outsider winning is negligible. Then the odds of Sweden winning would be 1:7 or 0.14. We can also calculate the frequency of Swedish victories in the past. Two gold medals in 15 years would give us the number 2:15 or 0.13, very close to the previous estimate. The exact probability is difficult to determine but most people would probably agree that it is likely to be in the vicinity of these estimates.
You can use this information to make sensible decisions. If somebody offered you to bet on Sweden winning the world championships at the odds 1:10, for instance, you might not be interested because the return on the bet would be close to your estimate of the probability. However, if you were offered the odds 1:100, you might be tempted to go for it, wouldn't you?
