Environmental factors associated with freshwater recreational water quality in Niagara Region, Ontario, Canada: A path analysis

Escherichia coli concentration levels in recreational water are used by beach managers to evaluate the risk of gastrointestinal illness among beachgoers. We examined the relationship between specific environmental factors and E. coli concentration in recreational beaches in the Niagara Region. We analysed E. coli geometric means collected from eight beaches from two of the Great Lakes in the Niagara Region in Ontario, between 2011 and 2019. We applied path analysis to evaluate the relationship between the environmental factors and E. coli concentrations, including whether effects were direct or indirect via a mediator. Turbidity was found to be an important mediator for the indirect effect of environmental variables overall and in beach-specific models. Rainfall and streamflow had a positive indirect effect on E. coli via turbidity and a direct effect in five out of seven beach models. Streamflow was also a mediator for the indirect effect of previous day air temperature in five out of seven models. In three subset models, outfall E. coli concentration was a mediator for the effect of the environmental factors. Using a novel methodological approach, this study identifies important relationships and pathways that predict beach E. coli concentration in freshwater beaches located on two of the Great Lakes.

Objective
To evaluate whole-genome sequencing (WGS) as a molecular typing tool for MRSA outbreak investigation.
DesignInvestigation of MRSA colonization/infection in a neonatal intensive care unit (NICU) over 3 years (2014–2017).
SettingSingle-center level IV NICU.
PatientsNICU infants and healthcare workers (HCWs).
MethodsInfants were screened for MRSA using a swab of the anterior nares, axilla, and groin, initially by targeted (ring) screening, and later by universal weekly screening. Clinical cultures were collected as indicated. HCWs were screened once using swabs of the anterior nares. MRSA isolates were typed using WGS with core-genome multilocus sequence typing (cgMLST) analysis and by pulsed-field gel electrophoresis (PFGE). Colonized and infected infants and HCWs were decolonized. Control strategies included reinforcement of hand hygiene, use of contact precautions, cohorting, enhanced environmental cleaning, and remodeling of the NICU.
ResultsWe identified 64 MRSA-positive infants: 53 (83%) by screening and 11 (17%) by clinical cultures. Of 85 screened HCWs, 5 (6%) were MRSA positive. WGS of MRSA isolates identified 2 large clusters (WGS groups 1 and 2), 1 small cluster (WGS group 3), and 8 unrelated isolates. PFGE failed to distinguish WGS group 2 and 3 isolates. WGS groups 1 and 2 were codistributed over time. HCW MRSA isolates were primarily in WGS group 1. New infant MRSA cases declined after implementation of the control interventions.
ConclusionWe identified 2 contemporaneous MRSA outbreaks alongside sporadic cases in a NICU. WGS was used to determine strain relatedness at a higher resolution than PFGE and was useful in guiding efforts to control MRSA transmission.

Summary
Aging-related decline in hand function is ubiquitous and inexorable, beginning at about age 60 years. This decline disproportionately impacts dexterous grasp and manipulation. Many potential explanations for this decline have been offered, but the causes remain poorly understood. Here we report observations from our laboratory on timed tasks demonstrating that the forces and kinematics of dexterous grasp and manipulation in old adults differ from young adults, even at “comfortable” performance speeds. These observations support recent suggestions that controlling the moments of force applied to grasped objects is a fundamental problem in old age. Possible explanations lead to a review of contemporary issues related to the sensorimotor control of the aging hand. These topics include: sensory deterioration (both peripheral and central); reduced ability to coordinate muscle forces to control force vectors at the fingertip of a single digit and across digits; increased moment-to-moment force fluctuations; and loss of independent control of the right and left hands. On an optimistic note, training and practice appears to slow or reverse declining hand function in healthy aging.
Introduction
Hand function deteriorates unequivocally in healthy aging. Well-known decreases in muscle strength, mostly from sarcopenia (Holloszy, 1995; Hughes et al., 2001; Doherty, 2003), can account for increased difficulty in accomplishing some daily living skills, such as opening containers (Sperling, 1960; Shiffman, 1992). Manual dexterity also declines in old age, but in a manner that is dissociated from decreasing muscle strength.

Wheat germ agglutinin–horseradish peroxidase conjugate (WGA–HRP) was injected into the dorsal root ganglia (L5–S1) of the cat and used as an anterograde tracer substance for intra-axonal labelling of peripheral nerve endings in joint capsule and cranial (anterior) cruciate ligament (CCL). We believed that the high specificity of WGA–HRP for neural tissue along with the high visibility of its reaction product could help resolve controversies concerning the sensory innervation of the cruciate ligaments. Substantial amounts of WGA–HRP were transported in tibial nerve axons to the level of the knee. However, using standard HRP histochemistry we found that the capsular tissue and ligament synovia disintegrated during the incubation reaction. This problem was avoided by air drying the tissue slices on glass slides prior to reaction. Abundant labelling occurred in the posterior capsule with dense filling of axons and terminal endings. Sensory endings displayed features consistent with Ruffini endings and pacinian corpuscles. Sensory endings were located throughout the CCL in its sagittal plane, in the subsynovial layers and between collagen fascicles. In each CCL we observed 5–17 ovoid and elongated endings with dense terminal arborisations. These endings were between 100 and 150 μm long, were encapsulated, and gave rise to 1 or 2 axons. Large (up to 1.5 mm in maximum extent) elongated regions of dense, inhomogeneous labelling were found in the body of several CCLs. These resembled Golgi tendon-like endings, with the exception of their large size. We conclude that anterograde transport of HRP to the knee is a useful technique for labelling mechanoreceptors and axons in knee tissue. However, recently developed immunohistochemical analysis of peripheral tissue using protein gene product 9.5 appears to be the method of choice and should be employed for further study of human and animal cruciate ligament innervation.
