Depression is a common psychiatric disorder of the aged. This article briefly reviews the literature on the use of cognitive behavioral therapy (CBT) with the elderly, and suggests some changes in using CBT with the elderly based on the authors' clinical experiences. Recommended changes in technique and common themes when dealing with the frail elderly are described, including the use of “supportive CBT” for patients with mild cognitive impairment. The authors' experiences suggest that CBT is an effective treatment for depression and other affective disorders of the frail elderly, and is especially useful when somatic treatments are contraindicated or produce intolerable side effects.

Treatment studies of delusional major depression demonstrate a poor response to standard antidepressant medications. Longitudinal studies demonstrate high relapse rates, even in patients receiving postdischarge antidepressants. The poor medical and psychiatric outcomes for late-life delusional depression and the increased risk for adverse medication reactions in this population underscore the importance of developing effective postrecovery treatments.

Studies of mixed-age adults demonstrate the effectiveness of acute treatment with either electroconvulsive therapy or combination pharmacotherapy with high doses of neuroleptics and antidepressants. In considering these results in relation to the treatment of late-life delusional depression, attention must be given to the particular vulnerabilities to medication side effects of elderly patients.

The potential effectiveness of continuation treatment with combined antidepressant-neuroleptic therapy is discussed. Clinical and methodologic issues related to studying the effectiveness of combination treatment in elderly patients are emphasized.

We set out to investigate the possible beneficial effects on cognitive function of demented patients with cobalamin deficiency after cobalamin replacement. A total of 181 consecutive, demented (DSM-III or DSM-III-R criteria and score below 24 on the Mini-Mental State Examination [MMSE] outpatients (mean age 77.5 years) were prospectively evaluated and had their vitamin B12 level measured by radioimmunoassay. The frequency of vitamin B12 deficiency (less than 200 pg/mL) was 25% (46 patients). Treatment outcome was obtained in 19 patients (19 of 46). Despite cobalamin replacement, 16 of 19 patients persisted in showing progressive decline during follow-up visits (3 to 24 months). The nonresponse to vitamin B12 replacement in most cases seems to reflect the presence of associated irreversible dementia or a follow-up of shorter duration in a few patients. All of the patients who showed some improvement (MMSE returned to normal values) had mild dementia with a history of less than 2 years. Thus, screening for B12 deficiency should be considered in patients with recent onset of mild mental status changes.

Although individuals aged 65 and over accounted for 12% of the total population of Japan in 1990, suicides in this age group consisted of 29% of all suicides. The elderly population of Japan is expected to grow rapidly to 24% of the total population by the year 2020, and suicide prevention for the elderly is an urgent mental health problem. Among a total of 1,216 elderly patients who were admitted to the Department of Psychiatry at Tokyo Metropolitan Geriatric Hospital between April 1980 and March 1990, 50 were hospitalized immediately after suicide attempts in order to study their psychosociomedical problems. Because early diagnosis of depression and initiation of proper treatment are indispensable—even if patients do not show obvious depressive symptoms—those who develop persistent somatization and/or delirium should be considered highly suicidal and given special attention. With the cooperation of mental health professionals, it is necessary to educate general practitioners, the public, and the elderly themselves about characteristics of psychiatric disorders and various problems associated with aging. Further research on other elderly Japanese populations, as well as research that examines suicide completion, is needed to confirm the findings of the present study.

This is a comparative study of the prevalence of dementia among elderly Chinese and Malay persons living in the community in Singapore. The subjects, aged 65 years and over, were all living in public housing estates. Two hundred elderly Chinese and 149 elderly Malay persons were interviewed. They were first screened for any cognitive deficit with the Elderly Cognitive Assessment Questionnaire (ECAQ). All those who scored 5 or fewer points on the ECAQ were then assessed with the Geriatric Mental State schedule.

The results showed an overall dementia prevalence of 4.0% among the Malay elderly and 2.5% among the Chinese elderly. The rate of dementia of the Alzheimer's type was 1.8% for Chinese women and 1.5% for Malay women; for multi-infarct dementia the rate for Malay women was 4.4% and for Chinese women 0.9%. For Chinese and Malay men, the prevalence of Alzheimer's disease and multi-infarct dementia was quite similar (1.1% and 1.2%).

Depression in the geriatric population is a frequents, serious, and potentially reversible disorder, yet relatively few blinded, controlled, antidepressant trial have been reported. A number of age-related issues complicate safe and effective pharmacotherapy. In a 6-week, double-blind trial in moderately to severely depressed (nonpsychotic) outpatients over age 60, fluoxetine (N = 335) was statistically significantly more efficacious than placebo (N = 336) in overall response (43.9% vs. 31.6%, p = .002) and remission (31.6% vs. 18.6%, p < .001) rates. Analyses of early discontinuations because of an adverse drug event revealed no statistically significantly greater rate with fluoxetine (n = 39; 11.6%) than was seen with placebo (n = 29; 8.6%). These results corroborate that major depression in an older population is responsive to antidepressant pharmacotherapy. Specifically, fluoxetine, at a conventional 20-mg dose, was both safe and effective relative to placebo in this special population.

Two hundred participants (mean age = 80 years) from five senior day-care centers were included in a study of agitation. Staff members at the centers and participants' relatives rated the frequency with which participants displayed agitated behaviors, via an expanded version of the Cohen-Mansfield Agitation Inventory. The most frequent behaviors noted were general restlessness, repetitious sentences, verbal interruptions, and pacing. A three-factor solution for staff members' ratings included (a) physically nonaggressive behaviors, including general restlessness and pacing; (b) verbally agitated behaviors, including complaining and constant requests for attention; and (c) aggressive verbal behaviors, including cursing and temper outbursts. A three-factor solution for relatives' ratings included (a) physically nonaggressive behaviors, including general restlessness and pacing; (b) verbally agitated behaviors, including constant requests for attention and related interruptions; and (c) aggressive behaviors, including cursing, grabbing, kicking, and pushing. The syndromes of both models showed similarity to the factors found in a nursing home population, although differences were also apparent.

In a randomized 6-week trial comparing fluoxetine with placebo, the Medical Outcomes Study 36-Item Short-Form Health Status Survey (SF-36) scales were used to measure the effects of treatment on functional health and well-being among elderly (age ≥ 60 years) outpatients with major depression. In the fluoxetine and placebo groups, 261 and 271 patients, respectively, completed the SF-36 before treatment and at Weeks 3 and 6. Compared with national norms for individuals over age 60, study patients before treatment exhibited baseline decrements on the following SF-36 scales: mental health, role limitations due to emotional problems, social functioning, vitality, role limitations due to physical problems, and bodily pain. Analyses of SF-36 changed scores from baseline to Week 6 revealed that the fluoxetine group improved more than the placebo group across all scales. Differences in changes of scores between groups were significant (p < .05), favoring the fluoxetine group for the scales of mental health, role limitations due to emotional problems, physical functioning, and bodily pain. Improvements observed in the fluoxetine group were both clinically and socially significant.

It is estimated that in the year 2020, approximately 25% of the Japanese population will be over 65 years of age. Moreover, suicide is a significant public health problem in Japan, where more than 6,000 elders take their lives each year. The authors compare late-life suicide in urban Kawasaki with suicide among the elderly in rural Higashikubiki over a 12-year period, from 1979 through 1990. The suicide rates in Kawasaki were lower than for Japanese elders as a whole, whereas those in rural Higashikubiki were extraordinarily high. The most frequent method used in both areas was hanging, and none of the victims died of gunshot wounds. In Higashikubiki, almost two thirds of victims lived in a three-generation family and none lived alone. The change of the family system from the traditional extended family to the nuclear family is accelerating, especially in rural areas. The authors suggest that the greatly elevated suicide rates among the elderly in Higashikubiki, and in rural regions of Japan more generally, result from these rapidly occurring changes in traditional social structure.

Because of the growing number of elderly people, the incidence of dementia is increasing. The disease represents a heavy burden for patients and their relatives and has growing social and financial implications. Unforunately, research and therapeutic progress is slowed by mehodological differences in the assessment of dementia and a therapeutic nihilism toward antidementia drugs. However, the limited efficacy of some antidementia drugs should not obscure the fact that a definite placebo-verum difference has been proved for other drugs and that the combination of these drugs with other therapeutic methods, like cognitive training, does bring positive results. Their use also seems meaningful in terms of economics.

This study examined some aspects of psychogeriatric care in Münster, Germany. The diagnostic and therapeutic attitudes of 94 general practitioners/internists and neurologists/psychiatrists toward demented patients were investigated by questionnaire. This figure represents a return rate of approximately 55% on the questionnaires. As is typical in Germany, no physicians were specializing in geriatric patients, and fewer general practitioners/internists than neurologists/psychiatrists had undergone supplementary psychogeriatric training. A higher percentage of the latter group estimated more than 10% of their elderly patients were demented. Both estimated Alzheimer's disease as less frequent than multi-infarct dementia. Physicians with training in psychogeriatrics claimed to treat more elderly patients than physicians with out it. With regard to therapeutic procedure, physicians with psychogeriatric training prescribed nootropics more guardedly. The data from this pilot study suggest that there are no eminent discrepancies between the different medical groups with regard to psychogeriatric care, but there is a great need for supplementary training.

This article reviews British government policies regarding the reduction of elderly suicide. Epidemiologic trends in elderly suicide indicate males aged 75 and older have the highest rates, with frequent methods being hanging and overdose. In contrast, rates among elderly who have immigrated from the Indian subcontinent in the past 10 to 30 years are significantly lower. Characteristics of elderly suicides and deliberate self-harm in the United Kingdom suggest that depression, social isolation and loss, and physical illness all are risk factors. Because most elderly suicides have contact with a primary healthcare provider in the month prior to the suicide, an effort to educate general practitioners and psychiatrists about treatable depression in the elderly is being made. Other policies also are being implemented: reduction of means of suicide, better linkage among services for the elderly for enhanced appropriate care, and clinical audits of suicide. Research associated with these policy efforts may further elucidate clinically relevant risk factors.

Late-life depression is an immense public health problem for which research on treatment interventions is very much needed. Between 1950 and 1990, average longevity worldwide increased from 46.6 years to 64.7 years (United Nations, 1993). With this surge in the elderly population has come an increased prevalence of late-life depression and its sometimes severe consequences. Older people with depressive symptoms have a threefold increase in mortality (Ashby et al., 1991). As many as 15% to 25% of nursing home residents in the United States have major depression (NIH Consensus Development Panel, 1992), and between 50% and 80% of elderly people who commit suicide have major depression (Clark, 1991; Conwell et al., 1991; Finkel & Rosman, 1995).

The purpose of this study was to investigate the prevalence and comorbidity of current mental disorders defined by DSM-III-R among elderly suicide victims and to compare them with the mental disorders among younger victims. Using a psychological autopsy method, we collected comprehensive data on all suicides in Finland during 1 year. Retrospective Axis I-III consensus diagnoses were assigned to a random sample consisting of 43 victims aged 60 years or over and 186 victims aged under 60 from the nationwide suicide population. At least one Axis I diagnosis was made for 91% of the elderly victims. Major depression as the principal diagnosis was more common among the elderly victims. Almost all elderly female victims were major depressives. Psychiatric comorbidity was more common among elderly male than among elderly female victims. More of the elderly victims (88%) than the younger (36%) received Axis III diagnoses. Suicide among the elderly without a diagnosable mental disorder and somatopsychiatric comorbidity seems to be rare.

To assess the economic burden of Alzheimer's disease (AD), we carried out a cross-sectional prevalence cost-of-illness study in France. Fifty-one probable AD patients (NINCDS-ADRDA) actually treated in ambulatory care were recruited in two university outpatient centers. Demographic, clinical (including actual Mini-Mental State Examination scores), and economic data were collected by clinical investigators and trained interviewers. Total costs included actual expenditures such as direct medical costs and direct nonmedical costs, as well as indirect costs (loss of earnings due to loss of productivity). Cost valuation was based on the societal perspective using an opportunity costing approach. We found that indirect costs represented a significant portion of total costs (36%-40%). In terms of expenditures, patients and caregivers were found to bear the major part of AD total costs. We found a positive and significant correlation between disease severity and costs. Our findings support the hypothesis of a relationship between disease evolution and healthcare costs.

Sixteen patients with dementia were studied 4 years after admission to group-living (GL) units, an intermediate level of dementia care. Of eight patients who were still alive, four lived in the GL units and four had been institutionalized. The eight patients who had died had spent 89% of their survival time in GL. Aggression was the most frequent cause of institutionalization.

The primary objectives of this study were to examine recent stressful life events and to explore possible social interaction factors across age groups in a population of suicides. Life events during the last 3 months preceding suicide (32 items) and factors related to social interaction (6 items) among 219 suicide victims aged 60 years and older were compared with those of 803 victims aged 20 to 59 years. The study population comprised all adult suicides who had life events interview data assessed as reliable by the interviewers in a nationwide total suicide population (1,397) of 1 year in Finland.

Differences in life events were found across age groups: Family discord, loss (separation, death), financial trouble, job problems, unemployment, and residence change were more common among younger victims, and somatic illness was more common among elderly victims. Age groups were similar with regard to proportions of persons living alone, availability of confidants and friends with common interests, and reports of loneliness. Similarly, few differences were found among persons aged 60–74 compared to age 75+ with regard to rates of life events, living alone, opportunities for social interaction, and complaints of loneliness. Loneliness was reported for persons who experienced loss among both younger and older suicides. In terms of age differences, younger men were more likely to have experienced job-related events,whereas older men were more likely to have had a somatic illness.

Somatic illness appeared to be the most important stressor in elderly suicides, particularly for men. Living alone and diminished opportunity for social interaction were not common factors in late-life suicides. Controlled studies with age matched comparisons are needed to further investigate how life events and social interaction factors vary in their risk for suicide across the life course.

Recent major political changes in Eastern European countries might have affected their suicide rates. For this article, suicide statistics available to the World Health Organization were used to compare data from eight Eastern European countries to those from seven countries in Northern or Western Europe. Comparisons were made between 1987 and 1991/92 data using total suicide rates for each country, rates by gender, and rates for the elderly (age 75 and older). The total rates indicated an increase in suicide in Eastern European countries and a decrease in other European countries. The ratio of male-to-female suicides in the Eastern European countries increased during this time as well, more than in other European countries. Among those over the age of 75, however, rates of suicide in Eastern European countries decreased; this pattern was less clear in the European countries chosen for comparison. The article discusses the role of economic, cultural, and health service factors affecting these trends.

In the cosmopolitan city of Singapore the annual suicide rates in the general population from 1985 to 1991 remained fairly constant, with a mean of 15.3 per 100,000. It was highest among Indians (19.5 per 100,000), followed by Chinese (16.2 per 100,000) and Malays (2.3 per 100,000). The suicide rates were higher in elderly people (aged 65 years and over) than in younger age groups (10 to 64 years) and in males than in females. For the elderly, the mean annual suicide rate for this period was 52.0 per 100,000. However, it was highest among Chinese, with 59.3 per 100,000, followed by Indians at 33.9 per 100,000, and, again, lowest among Malays, with 3.0 per 100,000. Possible sociocultural factors are proposed to account for differences in suicide rates among these ethnic groups.
