The Clinical and Translational Science Award (CTSA) program aims to enhance the quality, efficiency, and impact of translation from discovery to interventions that improve human health. CTSA program hubs at medical research institutions across the United States develop and test innovative tools, methods, and processes, offering core resources and training for the clinical and translational research (CTR) workforce. Hubs have developed services across different domains, such as informatics and pilot studies, to provide ad hoc expertise and staffing for local research teams. Although these services can provide efficient, cost-effective ways to cover skills gaps and implement rigorous studies, three CTSAs of varying size found the majority of investigators were single domain service users, likely missing opportunities to further enhance their work.

Through interviews with CTSA service users and a survey of CTSA service managers, this exploratory study aims to identify barriers to using services from multiple modules and solutions to overcome those barriers.

Barriers include challenges in finding information about services, unclear or unknown user needs, and users’ lack of funding to engage in services. More issues were identified for the largest CTSA.

Although this study represents a small subset of CTSA hubs, we anticipate that our findings and proposed solutions will be relevant to the broader CTSA community. This study provides foundational information can use in their own efforts to increase service utilization and methods that can be used for more comprehensive studies that focus on explaining the relationship between CTSA features and rates of single versus cross-module service use.

Existing literature suggests that mental health literacy is positively associated with mental health services utilization. Despite an aging population that faces significant mental health concerns in Korea, the role of mental health literacy on mental health services utilization is not known among older adults in Korea. This study aimed to (1) identify whether mental health literacy mediates the association between population characteristics and mental health services utilization and (2) identify an optimal path model for mental health services utilization among Korean older adults.

Using a cross-sectional survey with a quota sampling strategy, we collected and analyzed responses from 596 community-dwelling individuals ages 65 years and older. We used structural equation modeling (SEM) to estimate the effect of mental health literacy as a mediator.

When controlling for other relevant covariates in the optimal path model, mental health literacy mediated the relationships between three socio-demographic factors (education, general literacy, and health status) and mental health services utilization. The model fit index shows that the SEM fits very well (CFI = 0.92, NFI = 0.90, RMSEA = 0.07).

Efforts to improve mental health literacy through community-based education programs may need to particularly target Korean older adults with the relevant socio-demographic characteristics to enhance their utilization of appropriate mental health services.

II rapporto tra prevalenza annua non ospedaliera e ospedaliera è nel 1990 di 2,5 a 1. II tasso di lungoassistiti e di 130/100000 adulti. La spesa globale del Dipartimento è aumentata di circa il 25%, con un incremento da parte di tutte le componenti dell'assistenza.

The principles, structure, Psychiatric Case Register (PcR) and catchment-area of the Community Psychiatric service of South-Verona are described, using the same methodology employed in 4 other Italian PCRs. The monitoring of the demand over 4 years shows one-year prevalence rates (1010/100000 adult inhabitants), one-day prevalence rates (306/100000 adult inhabitants) and incidence rates (219/100000 adult inhabitants) lower than those of other European PCRs. South-Verona PCR records data of the activities made in a comprehensive psychiatric service, which has multiple structures and services, offering different degrees of care: residential in and outside the hospital, semiresidential, in outpatient clinics, domiciliary, etc. The 1990 ratio between non-hospitalized and hospitalized users is 2.5 to 1. The rate of long-term patients is 130/100000 adult inhabitants. All the components of the treatment contributed to the 25% increment of the costs of the South-Verona Psychiatric Service.

Scopo - Analizzare l'attività del Servizio Psichiatrico di Diagnosi e Cura di Pavia nel periodo compreso tra il 1978 (anno dell'apertura del servizio) ed il 1990. Disegno - Analisi retrospettiva dei dati contenuti in un archivio computerizzato, nel quale, a partire dal 1978, sono state raccolte informazioni relative alle caratteristiche socio-demografiche e nosologiche dei pazienti ricoverati, nonché alia modalità ed all'andamento dei ricoveri. Setting - Il Servizio Psichiatrico di Diagnosi e Cura di Pavia, il cui bacino di utenza è costituito dall'ULSS 77, comprendente la città di Pavia e 61 comuni limitrofi. Principali indici utilizzati - Per ciascun anno considerato sono stati ricavati: indicatori d'uso (primi ricoveri, ricoveri complessivi, giornate di degenza, pazienti ricoverati); presenza media giornaliera e media annuale dei massimi mensili di pazienti ricoverati; indicatori d'intensita d'uso o di flusso (degenza media, indice di occupazione media dei posti-letto, indice di turnover, indice di rotazione dei posti-letto). La produttivita del servizio è stata indagata utilizzando il monogramma di Barber. Risultati - Tra il 1978 ed il 1983 si è verificato un progressivo incremento nel numero dei pazienti ricoverati, dei ricoveri complessivi e delle giornate di degenza; è seguito un periodo stazionario protrattosi fino al 1987; infine, durante gli ultimi tre anni il numero dei ricoveri complessivi e quello dei pazienti ricoverati sono leggermente diminuiti, mentre le giornate di degenza sono rimaste sostanzialmente stabili. Il numero annuale dei pazienti entrati in contatto per la prima volta con il servizio ha segnato due picchi negli anni 1979 e 1983 (N = 250), diminuendo poi progressivamente fino ai valori di 139 nel 1989 e di 152 nel 1990. Le frazione percentuali dei primi ricoveri rispetto ai ricoveri complessivi e rispetto ai pazienti ricoverati si sono ridotte nel corso del periodo considerato: nel 1990 esse erano pari a 30% e 50% rispettivamente. Presenza media giornaliera e media annuale dei massimi mensili di pazienti ricoverati sono aumentate tra il 1979 ed il 1983, passando da 12 a 21 pazienti/die, e da 14 a 25 pazienti/die, rispettivamente; negli ultimi otto anni i due parametri si sono stabilizzati su valori di 20-21 e 24-25 pazienti/die rispettivamente. II nomogramma di Barber, costruito sulla base dei valori assunti dagli indicatori d'intensita d'uso nel corso di ciascun anno considerato, ha mostrato che tutti gli anni in esame si sono distribuiti entro la zona di produttivita accettabile, ad eccezione del 1980 e del 1981. Conclusioni - Dopo una fase iniziale di progressiva espansione, il servizio sembra aver raggiunto una fase di equilibrio dinamico con il suo bacino di utenza, fornendo per la maggior parte del periodo considerato un livello di produttività accettabile.

Objective – To analyze the activity of a General Hospital Psychiatric Inpatient Service in Pavia between 1978 (when the service was set up) and 1990. Design - Retrospective analysis of data stored in a computerized database, in which, since 1978, information has been collected on sociodemographic and nosological characteristics of patients admitted to the service, as well as on clinical aspects of all admissions. Setting – A General Hospital Psychiatric Inpatient Service in Pavia, whose catchmentarea (ULSS 77) includes the town of Pavia and 61 close municipalities. Main outcome measures -For each yearover the period considered we reported on: the indices of service utilization (first admissions, total admissions, days of stay, number of admitted patients); the mean daily number of inpatients, and the mean of the largest daily numbers of inpatients by month; the indices of intensity of service utilization (mean lenght of stay, index of bed occupation, index of turnover, index of bed rotation). The service productivity was determined according to the Barber's nomogram. Results – Between 1978 and 1983 patients admitted to the service, total admissions, and days of stay gradually increased; a steady period then followed until 1987; over the last three years, total admissions and the number of admitted patients slightly decreased, while daysof stay remained substantially stable. The annual number of patients admitted for the first time to the service marked two peaks in 1979 and 1983 (N = 250), and then gradually decreased to 139 in 1989 and 152 in 1990. The percent ratio of first admissions by total admissions and that of first admissions by the number of admitted patients showed a reduction over the period considered: in 1990 they were 30% and 50% respectively. The mean daily number of inpatients and the mean of the largest daily numbers of inpatients by month increased between 1979 and 1983, from 12 to 21 and from 14 to 25 patients/day respectively; over the last eight years the values remained rather constant, around 20-21 and 24-25 patients/day respectively. The Barber's nomogram, obtained on the basis of the indices of intensity of service utilization over the period considered, showed that each year fell within the area of acceptable productivity, with the exception of 1980 and 1981. Conclusions – After an initial phase of gradual expansion, the service then reached asteadystate, providing an acceptable productivity over most of the period considered.

II rapporto tra prevalenza annua non ospedaliera ed ospedaliera è nel 1990 di 5,5 a 1. II tasso di lungoassistiti è di 161/100000 adulti. La spesa globale del Dipartimento è lievemente diminuita dal 1987 al 1990.

Portogruaro PCR records data of the activities made in a comprehensive psychiatric service, which has multiple structures and services, offering different degrees of care: residential in and outside the hospital, semiresidential, in outpatient clinics, domiciliary, etc. The 1990 ratio between non-hospitalized and hospitalized users is 5.5 to 1. The rate of long-term patients is 161/100000 adult inhabitants. The costs of the Community Psychiatric Service slightly decreased during 1987-90 period.

I tassi di incidenza (281/100000 residenti adulti) indicano un afflusso di nuovi casi appartenenti a tutte le categorie diagnostiche. II RPC di Caltagirone monitora l'attività svolta in strutture e servizi a differente gradiente assistenziale: residenziale ospedaliera, residenziale non ospedaliera, semiresidenziale, ambulatoriale, domiciliare, ecc. II rapporto tra prevalenza annua non ospedaliera e ospedaliera è nel 1990 di 4,4 a 1. II tasso di lungoassistiti è di 122/100000 residenti adulti, con una tendenza ad un accumulo per quelli che vivono nel territorio, mentre vi è un calo di quelli che sono in Comunità. II costo del Dipartimento, che è andato riducendosi dal 1987 al 1990, è in buona parte da addebitare alle giornate di assistenza in Comunità.

The monitoring of the demand over 4 years shows that the year prevalence (763'100000 adult inhabitants) and day prevalence (248'100000 adult inhabitants) rates are lower than those of other European PCRs. Incidence rate (248'100000 adult inhabitants) shows that new patients belong to all diagnostic categories. The PCR records data of the activities made in multiple structures and services, offering different degrees of care: residential in and outside the hospital, semiresidential, care in outpatient clinics, domiciliary care, etc. The ratio between non-hospitalized and hospitalized users is 4.3 to 1. The rate of long-term patients is 122'100000 adult inhabitants; the rate of long-term patients living in the community is increasing over the years, whereas the similar rate of patients living in the sheltered apartments is decreasing. The costs of the Service (which have decrease from 1987 to 1990) have to be attributed mainly to the costs of the treatment in sheltered apartments.

II rapporto tra prevalenza annua non ospedaliera ed ospedaliera è, mediamente, di 3,8 a 1. II tasso dei lungoassistiti è di 195/100000 residenti adulti, con un accumulo di quelli che vivono nel territorio, ma non di quelli che sono in Comunita. II costo dei servizi psichiatrici di Legnano è diminuito nella componente ospedaliera e semiresidenziale, mentre è aumentato in quella territoriale.

The monitoring of the demand over 4 years shows that the mean year prevalence (530/100000 adult inhabitants) rates are lower than those of other European PCRs. Incidence rate (152/100000 adult inhabitants) have diminished, showing a difficulty of the psychiatric services in accepting new patients. The ratio between non-hospidalized and hospidalized users is 3.8 to 1. The rate of long-term patients is 195/100000 adult inhabitants; the rate of long-term patients living in the community is increasing over the years, whereas the similar rate of patients living in the community is increasing over the years, whereas the similar rate of patients living in the scheltered apartments is stable. The costs of the hospital-based and the semiresidential agencies decreased, while the cost of the community agencies increased.
