Several scientific communities and international health organizations promoting an interdependent human-nature health perspective are calling upon healthcare professionals (HCP) to integrate this vision into their practice and become role models. However, rising cases of stress, burnout, and depression, among this group jeopardize this potential and their self-care. Therefore, we conducted an exploratory qualitative study focusing on how HCP relate to their self-care, their relationship with nature and its implementation into their professional practice. Semi-structured interviews with 16 HCP were executed, transcribed and imported into NVivo. Using the six-step framework, we conducted a thematic analysis, followed by two-step member-checking. Three main findings arose. First, participants employ various self-care strategies outside of work whenever possible. Second, their nature experiences can be drawn along a continuum ranging in level of intensity, attitude and reciprocity, which does not seem to be disclosed during consultation. Third, the reflexive interviews may have led to increased awareness and agency on the former topics, which we have termed ‘nature-connected care awareness’. A preliminary framework to stimulate nature-connected care awareness could support HCP in becoming a role model.

To better understand and improve the rehabilitation process of older adults with sensory losses in both hearing and vision or dual sensory impairment (DSI), this study explored the perspectives of health care professionals who work with this population. Thirteen individuals, with varied professional backgrounds, were interviewed about their experiences in working with older adults with DSI. We transcribed and coded the interviews, then conducted content analysis. Regardless of their professional backgrounds, the participants reported additional roles that they perceived they fulfilled: (a) counsellor, (b) navigator, and (c) trainer and re-trainer. These roles involved helping individuals with DSI, and their family, with depression, acceptance, repeat consultations, and way-finding through the health system. From the professionals’ perspective, these additional roles increase workload and place them in situations they were not trained for. They suggest education for all professionals and for family members working with people with DSI; moreover, they suggest a multidisciplinary team rehabilitation approach.

This study aimed to evaluate the effects of an intervention including nutritional telemonitoring, nutrition education, and follow-up by a nurse on nutritional status, diet quality, appetite, physical functioning and quality of life of Dutch community-dwelling elderly. We used a parallel arm pre-test post-test design with 214 older adults (average age 80 years) who were allocated to the intervention group (n 97) or control group (n 107), based on the municipality. The intervention group received a 6-month intervention including telemonitoring measurements, nutrition education and follow-up by a nurse. Effect measurements took place at baseline, after 4·5 months, and at the end of the study. The intervention improved nutritional status of participants at risk of undernutrition (β (T1)=2·55; 95 % CI 1·41, 3·68; β (T2)=1·77; 95 % CI 0·60, 2·94) and scores for compliance with Dutch guidelines for the intake of vegetables (β=1·27; 95 % CI 0·49, 2·05), fruit (β=1·24; 95 % CI 0·60, 1·88), dietary fibre (β=1·13; 95 % CI 0·70, 1·57), protein (β=1·20; 95 % CI 0·15, 2·24) and physical activity (β=2·13; 95 % CI 0·98, 3·29). The intervention did not have an effect on body weight, appetite, physical functioning and quality of life. In conclusion, this intervention leads to improved nutritional status in older adults at risk of undernutrition, and to improved diet quality and physical activity levels of community-dwelling elderly. Future studies with a longer duration should focus on older adults at higher risk of undernutrition than this study population to investigate whether the impact of the intervention on nutritional and functional outcomes can be improved.

Introduction: The right to live is the supreme human right and according to Article 3 of the Universal declaration of human rights everyone has a right to life, liberty and security. In Karachi, Pakistan huge numbers of health care professionals (HCP) have been subjected to violence inclusive of money extortion, kidnapping, mental & physical torture, murders etc. A recent study from Karachi’s four major hospitals reported that 72.5% of HCP have experienced abuse (verbal and physical) in the past 12 months. The goal of this study was to develop strategies for preventing violence against health care after collecting baseline data. The objectives of the study were to: Identify the magnitude, threshold and impact of violence against HCPs. Methods: This was a mixed methods study design with a QUAN-QUAL approach. Structured questionnaire was used after pilot testing and filled by the surveyors. Focus group discussion and In-depth interviews were conducted with HCPs, NGOs, Law enforcement agencies, ambulance services, hospital administrators and LHW programs. Frequencies and proportions were compared for different cadres of HCPs for the quantitative data analysis. Thematic content analysis with inductive and deductive reasoning was used for analysing qualitative data. Results: Data on 822 HCPs revealed that 33% had faced violence and 49 % had experienced it, 89% was verbal and 43% was physical abuse, 2% had died and 22% were injured. Ambulance services and physicians were the most common victims of violence. The main reasons were grouped as institutional, behavioral (victims and perpetrators) and general situation of the city. There is high acceptance of violence among HCPs and lack of training in dealing with it was the most common reason given. Sequlae included effects on victim, family, institution and the reporting agencies. The final paper will have complete details on the baseline and the recommendations proposed by the stakeholders. Conclusion: Violence faced by HCP’s is a multifactorial complex issue. There is a dire need to design interventions which can help in addressing the behavioral, Institutional and sociopolitical factors promoting violence among HCP’s. The interventions based on recommendations by the respondents have been developed and implementation has started as a pilot in the city of Karachi.

This article focuses on the micro-narratives of two individuals whose responses to AIDS were mediated by their sexual identity, AIDS activism and the political context of South Africa during a time of transition. Their experiences were also mediated by well-established metanarratives about AIDS and ‘homosexuality’ created in the USA and the UK which were transplanted and reinforced (with local variations) into South Africa by medico-scientific and political leaders.The nascent process of writing South African AIDS histories provides the opportunity to record responses to AIDS at institutional level, reveal the connections between narratives about AIDS and those responses, and draw on the personal stories of those who were at the nexus of impersonal official responses and the personal politics of AIDS. This article records the experiences of Dennis Sifris, a physician who helped establish one of the first AIDS clinics in South Africa and emptied the dance floors, and Pierre Brouard, a clinical psychologist who was involved in early counselling, support and education initiatives for HIV-positive people, and counselled people about dying, and then about living. Their stories show how, even within government-aligned health care spaces hostile to gay men, they were able to provide support and treatment to people; benefited from international connections with other gay communities; and engaged in socially subversive activities. These oral histories thus provide otherwise hidden insights into the experiences of some gay men at the start of an epidemic that was initially almost exclusively constructed on, and about, gay men’s bodies.

In this article I analyze two brief case studies to propose that a “spiritual strength story” has five defining characteristics: (1) it is brief; (2) it is ontological; (3) it uses symbols and metaphors; (4) it is a “big story” or meta-narrative with a positive spiritual and/or religious focus that informs other narrative data; and (5) most conspicuously of all, it repeats. Cultivating awareness of the “spiritual strength” narrative type can help to improve the quality of inter-professional patient-centered care teamwork and understanding, especially in regard to the reflexive, embodied, and relational aspects of palliative and end-of-life care.

Nutritional genomics has reached the public through applications of the Human Genome Project offered direct to consumers (DTC). The ability to pursue nutrigenetic testing without the involvement of a health care professional has received considerable attention from academic and policy commentators. To better understand the knowledge and attitudes of Canadian health care professionals regarding nutritional genomics and nutrigenetic testing, qualitative research in the form of focus group discussions was undertaken. Four key themes emerged: (1) concerns over DTC testing; (2) lack of health care professional competency; (3) genetic scepticism and inevitability; (4) expectation of regulation. Together, they indicate that health care professionals have little knowledge about nutritional genomics and hold contradictory attitudes towards genomics in general, and to nutritional genomics in particular. Respondents argue in favour of a delivery model where health care professionals act as intermediaries. They are also aware of their lack of competency to provide such services. To ensure greater public protection, respondents cite the importance of more stringent regulatory oversight of DTC genetic testing. Whether such an approach is necessary to address the various ethical and social issues raised by nutrigenetic testing remains an open debate.

Using a case scenario involving a marginally competent elderly woman living alone at risk, we assessed the care decisions made by older adults (n = 82) and health care professionals (HCPs, n = 87), and identified differences in the values underlying the care decisions. Overall, participants did not place a high value on independence when they appraised the risk to the client as high and safety as low. Under these conditions, elderly respondents tended to be more paternalistic in their decisions about care, while HCPs tended to be more beneficent. If the values of HCPs differ from those of elderly people, how likely is it that the care provided to marginally competent elderly people will be congruent with their wishes? The care provided by HCPs might be improved by incorporating knowledge of the values and perspectives of other older adults.

The purpose of this feminist narrative study was to examine the experiences of women in four different health professions (nursing, medicine, physiotherapy, and social work) who provided care to elderly relatives. Although caring is a central and common feature of the personal and professional lives of many women (Baines, Evans, & Neysmith, 1991; Baines, 2004), the separation of professional, paid caregiving from family, unpaid caregiving among health care providers is problematic. Study findings suggest that female health professionals who assume familial responsibilities continually negotiate the boundaries between their professional and personal caring work. Despite the use of a variety of strategies for managing their double-duty caregiving demands, many women experienced a dramatic blurring or erosion of these boundaries, resulting in feelings of isolation, tension, and extreme physical and mental exhaustion. These findings suggest that women who are double-duty caregivers, especially those with limited time, finances, or other tangible supports, may experience poor health, which warrants further study.
