    Elder mistreatment, defined as action or negligence against a vulnerable older adult that causes harm or risk of harm, either committed by a person in a relationship with an expectation of trust or when an older person is targeted based on age or disability, is common and may have serious medical and social consequences. Elder mistreatment includes physical abuse, sexual abuse, neglect, emotional/psychological abuse, financial exploitation, and self-neglect.

  • Summary

    Elder mistreatment is common and often overlooked by health-care providers. It may include physical abuse, psychological and emotional abuse, sexual abuse, financial exploitation, and neglect. While any older adult may be susceptible, particular mitigating factors discussed in this chapter include cognitive impairment and dementia, multimorbidity, substance use, socioeconomics and culture, and social isolation. Strategies for approaching clinical cases, identifying signs and symptoms, and developing interventions are explored using illustrative cases and selected findings from the growing literature on elder mistreatment across care settings.

  • Elder abuse is a serious public health concern requiring immediate intervention; however, the under-reporting of elder abuse by victims to formal and informal networks remains a major obstacle. This scoping review aims to identify barriers to help seeking that older adults experiencing abuse confront. The goal is to inform public policies and practices in the Canadian context and identify research gaps in the extant literature. Seven scholarly databases were searched from which 12 articles met the inclusion criteria and were extracted for analysis. The findings from this scoping review revealed three levels at which barriers exist: individual focused, abuser/family focused, and community/culture focused barriers. The results suggest that there are several complex obstacles that older adults face when contemplating disclosure of abuse. Future research into help seeking in the Canadian context should more readily incorporate the voices of elder abuse victim-survivors to develop effective assessment strategies and responsive service provisions.

  • Objectives:

    To explore the prevalence of EM in an older Chinese population and examine the mediating role of three psychosocial variables – psychological vulnerability, housework involvement, and financial independence – in the relationship between physical frailty and EM.

    Design:

    Cross-sectional analysis.

    Setting:

    The data source was the Third Survey on Chinese Women’s Social Status (SCSSW), which is a nationwide decennial survey conducted in 2010.

    Participants:

    Community-dwelling adults aged 60 and older who participated in SCSSW (N = 3516).

    Measurements:

    The past-year prevalence of EM and its seven subtypes, physical frailty, psychological vulnerability, housework involvement, financial independence, and demographic characteristics.

    Results:

    The past-year prevalence of EM was 4% among Chinese older adults, with psychological abuse being the most common subtype (3.9%). A higher level of physical frailty had a direct influence on EM. Older adults with higher levels of physical frailty were more likely to have higher levels of psychological vulnerability (anxiety, loneliness, and uselessness) and lower levels of housework involvement, which further correlated with increased risk of EM. Frail Chinese older adults were less likely to have financial independence, which in turn, surprisingly predicted a lower probability of EM.

    Conclusions:

    In this nationally representative sample, we provided the first evidence of the prevalence of EM among Chinese older adults and expanded the global understanding of EM by examining the mediating role of three psychosocial variables. Future studies are warranted to corroborate our findings and identify factors contributing to the complex mechanism of EM.

  • The elder abuse and neglect phenomenon in the ultra-Orthodox Jewish society: social workers’ perspectives

  • Background:

    In the last 30 years, elder abuse and neglect has been recognized as a social and health-related problem. The aim of this paper is to describe the phenomenon of elder abuse and neglect in a separatist faith-based society (ultra-Orthodox Jewish society—UOJS).

    Methods:

    A qualitative-phenomenological study with 28 social workers who underwent in-depth semi-structured interviews based on an interview guide consisting of the following items: visibility of the elder abuse and neglect phenomenon in the ultra-Orthodox society, and dilemmas and sensitive issues that arise when working with this population.

    Results:

    Three main themes emerged: (1) Between the commandment to honor one's parents and concealment patterns: Cultural barriers to exposing the abuse and neglect phenomenon; (2) “Life is demanding:” The unique expression of abusive and neglectful behavior in the UOJS; (3) Culturally related dilemmas when intervening with cases of elder abuse and neglect.

    Conclusions:

    Ultra-Orthodox Jewish cultural belief is a differentiating component in the context of elder abuse and neglect. Social workers need to develop a deep understanding of the unique characteristics of the phenomenon and cultural sensitivity to cope with it to address the well-being of older ultra-Orthodox Jews.

  • Background: Elder mistreatment, social ageism, and human rights are increasingly powerful discourses in positioning older people in society, yet the relationship between them has rarely been subjected to critical investigation. This perceived relationship will have implications for how mistreatment is understood and responded to.

    Method: Critical gerontological approach based on narrative and textual analysis.

    Results: Reports of public attitudes toward mistreatment suggest that it is thought to be more common than scientific evidence would suggest; however, reporting is much lower than prevalence. While the discourse over mistreatment has tended to focus on interpersonal relationships, ageism has emphasized social attitudes, and human rights have concentrated on relations between the state and the individual.

    Conclusions: In this paper, a series of models have been examined which mark a tendency to restrict and then attempt to reintegrate individual, interpersonal, and social levels of analysis. It is concluded that a focus on the processes of transaction across boundaries rather than contents would facilitate both integrative modeling and deeper understanding of the qualities of abusive situations.
