Summary
A 65-year-old male is admitted to the emergency department and found to have an acute appendicitis. His past medical history is significant for rheumatoid arthritis. The patient has taken prednisone 10 mg/day for several years. After an uneventful appendectomy, the patient becomes hypotensive with a blood pressure of 76/40 mm Hg in the post-anesthesia care unit. The patient received a 2 L normal saline IV bolus without improvement. Would you recommend that the patient receive stress dose steroids in this situation?
